WHY?
Creating opportunities and supports for learners to
independently continue their growth and learning.
Building a transition plan is creating a scaffold that allows
learners to reach their height of learning independence.
The scaffold is made up of a collaboration of individuals and
school networks.
The amount of supportive/intervention structure depends
on the need of the individual and varies over time.

WHY?
Effective transition plans address the individual's
strengths, speak to their desired level of learning
autonomy, is mindful of the potential to learn new
strengths, and consider that support may be reduced as
a learner gains progress.
