  • 4. •To start new COI submission, click “New COI” button. •Don’t see the New COI button? Be sure that your role is “Academic Staff Member”. Questions? Contact the COI Office at 686-5502.
  • 5. •Save at any time and return later. •Select Fiscal Year from drop down menu. •Use “Continue” and “Back” buttons, not browser buttons Questions? Contact the COI Office at 686-5502.
  • 6. •If nothing to disclose, hit “Continue.” •If something to disclose, check box(es), click on “Add.” •If company is not included in drop down menu, click on “Enter New Company.” Questions? Contact the COI Office at 686-5502.
