2. Log in using your UAMS username and password.

3. •Inbox - Items that require action by you to move your submission
through the review process.
•My COI - Displays your COI related items.

4. •To start new COI submission, click “New COI” button.
•Don’t see the New COI button? Be sure that your role is “Academic
Staff Member”.

5. •Save at any time and return later.
•Select Fiscal Year from drop down menu.
•Use “Continue” and “Back” buttons, not browser buttons

6. •If nothing to disclose, hit “Continue.”
•If something to disclose, check box(es), click on “Add.”
•If company is not included in drop down menu, click on “Enter
New Company.”

8. •When you are finished with your disclosures, click “Continue”
to move to next section.

10. •You must respond either “Yes” or “No” to continue. Otherwise, works
the same as previous pages.

11. •Check “I agree with the above statement.”
•Click “Finish.”

12. •Shows the current state of one COI submission.
•History – actions and states for that particular COI
submission.

13. •If you receive notification that changes are requested by an
approver, you can make edits to your COI form.
•Click here to view the COI form.

14. •Requested changes appear under the “Activity” heading.
•Click “Edit COI” to make requested changes.

15. •Make requested changes.
•Click Continue and sign the Attestation Page.
