This document outlines the official rules for a sweepstakes hosted by FAIRWINDS Credit Union from November 10 to December 23, 2014. Seven winners will be selected to receive prizes valued between $84-$500 by random drawing. To enter, individuals must be 18+ Florida residents and can submit online or via mail. Winners will be notified by phone and must claim their prize in person at the credit union within 15 days.

Official Rules
Introduction: During the period beginning on Monday, November 10, 2014 at 12:00 a.m. Eastern Time (“ET”) and concluding on Tuesday, December 23, 2014 at 11:59 p.m. ET (“Sweepstakes Period”), FAIRWINDS Credit Union (“Sponsor”) will sponsor the ‘Tis the Season to Save Sweepstakes (“Sweepstakes”). The Sweepstakes will offer multiple individual sweepstakes, each with a designated start and end date.
Eligibility: The Sweepstakes is open to legal U.S. residents at least 18 years of age residing within the state of Florida who are not minors at the time of Sweepstakes entry under the laws of their state of residence. Proof of eligibility will be required to claim prize(s). The following individuals, regardless of their ability to meet the above qualifications, do not qualify to enter or win the Sweepstakes: employees of Sponsor, immediate family members (spouses, parents, siblings, children, grandparents, grandchildren) of employees of Sponsor, Sponsor’s Board of Directors, and Sponsor’s Board of Directors’ immediate family members (spouses, parents, siblings, children, grandparents, grandchildren).
Prizes: Prizes may not be substituted for cash or another prize. Odds of winning will depend on the number of eligible entries received. Seven (7) winners will be selected to receive prizes valued at $84 to $500. The seven (7) winners will be determined during the Sweepstakes Period by random drawing from all eligible entries through the date of the applicable drawing. Winners are responsible for paying taxes on prizes. If Sweepstakes Period is shortened or lengthened due to start or end dates of Sweepstakes Period changing, this will have no impact on the number of winners selected and prizes issued; winners will be selected from the eligible entries received.
How to Enter: There are two (2) methods by which you can enter the Sweepstakes. You may enter the Sweepstakes in any one of the following ways:
a) Complete the entry form online at facebook.com/fairwinds.
b) Send a handwritten 3” x 5” card with your first name, last name, residential address, daytime telephone number, and email address to:

One entry per applicant is allowed during the Sweepstakes Period (November 10, 2014 through December 23, 2014). Multiple entries by any one individual may, at the sole discretion of the Sponsor, result in all of that individual’s entries being disqualified. No more than one (1) entry per postcard or envelope is allowed. Any attempt to violate these Official Rules will, at the Sponsor’s sole discretion, result in disqualification of any or all entries submitted that are not

done so in accordance with Official Rules. Any and all fees associated with any of the above entry methods are the sole responsibility of the individual entering the Sweepstakes.
General Rules for Entry: No purchase necessary to enter or to win. A purchase will not increase your odds of winning. All federal, state, local, and municipal laws and regulations apply. Void where prohibited. Sponsor’s decisions regarding all aspects of the Sweepstakes are final and non-negotiable, and may be changed only by the Sponsor. Sponsor will not be responsible for typographical, printing, or other inadvertent errors in any materials other than the Official Rules of the Sweepstakes.
Submission Cut-Off. All entries must be received by 11:59 p.m. ET on Tuesday, December 23, 2014 to be eligible. Sponsor will not be responsible for incomplete, lost, late, misdirected, or illegible entries, or for failure to receive entries due to transmission failures or technical failures of any kind, including, without limitation, malfunctioning of any network, hardware or software, whether originating with sender or Sponsor. All entries become property of Sponsor and none will be returned. Sponsor reserves the right to disqualify any entries by persons determined to be tampering with or abusing any aspect of the Sweepstakes.
Winner Selection and Random Drawing. The prize selection will be determined by random drawing at 135 W. Central Blvd. #1230, Orlando, FL 32801. Entrants do not need to be present to win.
Winners will be randomly selected from all eligible entries received during Sweepstakes Period. Random drawing will be conducted by a representative of FAIRWINDS Credit Union, whose decisions are final on all matters relating to the Sweepstakes. Seven (7) Winners will be selected from all entrants during Sweepstakes Period. Sweepstakes Winners will be determined by random selection on approximately:

Winner Notification and Acceptance: Winners will be notified by telephone. Sponsor will call the winner up to five (5) times that day and the following business day during regular business hours at number provided on entry form and will leave no messages. Sponsor reserves the right to use any contact phone number provided by Sweepstakes entrant to Sponsor – whether on 3” x 5” index card or in Sponsor’s current member database. Sponsor is not responsible for incomplete, inaccurate, or illegible phone numbers.
Failure to reach winner during these calls may result in disqualification of winner, forfeiture of his or her interest in all prizes, and selection of substitute winner. Winners must claim prize in

person, during regular business hours, at FAIRWINDS Credit Union Corporate Tower (135 W. Central Blvd., Suite 1230, Orlando, FL 32801), during regular business hours within fifteen (15) calendar days of being notified to claim prize. At the sole discretion of Sponsor, exception(s) may be made in the event a randomly selected winner does not live or work within twenty (20) miles of FAIRWINDS Credit Union’s Corporate Tower. If an exception is made, and delivery of prize is determined to be an acceptable substitute to winner claiming prize in person, winner will be solely responsible for any and all expenses Sponsor incurs in relation to packing, shipping, or delivering prize. Winners must provide a valid government-issued photo identification card or license when claiming prize. Winners may be required to sign an affidavit of eligibility, a liability and publicity release and acceptance of these Official Rules at the time the prize is claimed. Failure of Winners to sign and return any required affidavits and releases, claim prize or comply with any term or conditions of these Official Rules, will result in drawing Winner’s disqualification and the forfeitures of his or her interest in the prize. Winners may waive their right to receive prizes. Winner is solely responsible for reporting and payment of any taxes on prizes. Except where prohibited, acceptance of any prize constitutes Winner's consent to the publication of his or her name, biographical information, and likeness in any media for any commercial or promotional purpose, without limitation or further compensation. Prizes not won and not claimed by eligible winner in accordance with these Official Rules will not be awarded and will remain the property of Sponsor, or at the Sponsor’s discretion may be awarded to another randomly-selected Sweepstakes entrant.
Sponsor is responsible only for the issuance of prizes, not for the use, utility, quality, or otherwise. Once prize is claimed, Sponsor has no further responsibility or liability, stated or otherwise implied, to Winners in regards to Sweepstakes Prize. When claiming prize, Winners may be required to provide a completed W-9 form to Sponsor. W-9 form is required for Sponsor to issue Prize Winner a 1099-MISC form for the Fair Market Value of the prize, as required by the U.S. Government. Prize Winner is required to provide Sponsor with all information necessary to issue a 1099-MISC form including, but not limited to, Winner’s full legal name, social security number, and address.
Participation: By participating in Sweepstakes, entrants agree to be bound by these Official Rules and the decisions of the Sponsor including publication of name, biographical information, likeness, and photos. Winners agree and consent, without further authorization, compensation or remuneration of any kind, to the use of such person’s name, likeness, biographical information, and photograph in any and all advertising, promotions, and other publicity in any media, worldwide conducted by Sponsor.
Winners List: To obtain the list of winners, send a self-addressed, stamped envelope to: FAIRWINDS Credit Union, ‘Tis the Season to Save Sweepstakes, 135 W. Central Blvd., Suite 1230, Orlando, FL 32801. Requests must be received by January 5, 2015. The list will be sent after selection of winners.
Dispute Resolution: Prior to filing any arbitration, the party intending to file such a proceeding shall notify the other party in writing of the existence and the nature of the dispute. The parties each agree that within ten (10) business days of the other party's receipt of such notice, a representative of each party shall meet for a minimum of one (1) eight (8) hour day, or such

shorter period of time if the dispute is more quickly resolved, in Orlando, Florida in order to attempt to amicably resolve the dispute. If such informal dispute resolution attempts prove to be unsuccessful, the notifying party may initiate the dispute resolution procedures available as set forth herein. Each party shall bear its own costs associated with the meeting and in no event shall either party be liable to the other for any costs or expenses incurred as a result of such meeting. Except as expressly set forth below, by entering this Sweepstakes, you specifically agree to waive your right to bring a court action and have a jury trial, that any controversy or claim arising out of or related to this Sweepstakes shall be resolved by final and binding arbitration conducted in accordance with the Commercial Arbitration Rules, including the Supplementary Procedures for Consumer-Related Disputes, of the American Arbitration Association (“AAA”), and that there will be no class claims in arbitration. Any such controversy or claim (whether a contract, tort, statutory, or regulatory claim or claim based on any other legal theory) initiated by you or the Sponsor against the other, or against any employee, agent, or assign of the other, shall be submitted to and resolved on an individual basis by binding arbitration in accordance with the Federal Arbitration Act. The arbitration proceeding shall be conducted before one arbitrator from the AAA under the Commercial Arbitration Rules, including the Supplementary Procedures for Consumer-Related Disputes, of the AAA. Information regarding arbitration, the AAA and the rules of the AAA may be obtained by calling (800) 778-7879 or online at www.adr.org. You and Sponsor agree that any such arbitration shall be held and conducted in Orlando, Florida. Any court having jurisdiction thereof may enter judgment upon any award rendered by the arbitrator. The arbitrator shall determine the prevailing party, and the costs and expenses of the arbitration proceeding, including the arbitrator's fees, shall be borne by the non-prevailing party, unless otherwise required by law or the Commercial Arbitration Rules, including the Supplementary Procedures for Consumer-Related Disputes, of the AAA. Arbitration claims shall be submitted and resolved on an individual basis, and the arbitrator will not consider or address class claims brought by or on behalf of other persons, or award punitive or any other monetary damages other than the prevailing party’s actual damages. The parties acknowledge that violation of these Official Rules would cause irreparable harm that would not be adequately compensable by monetary damages. In addition to other relief, it is agreed that preliminary and permanent injunctive relief will be available to prevent any actual or threatened violation of such provisions.
