This document contains medical records for two patients. The first patient, Nama Nuraini, was born on 27-02-1985 and has a diagnosis of left eye keratopathy post amniotic membrane transplantation with a soft contact lens bandage. The planned procedure is left eye affection. The second patient, Nama Andi Halija, was born on 31-12-1951 and has a diagnosis of right eye keratopathy bullosa, meibomian gland syndrome, and severe dry eye disease in both eyes.
