1. Thoái hóa khớp gối như một quy luật?
Tình trạng thoái hóa khớp gối hay gặp ở người lớn tuổi, triệu chứng đau nhức
khiến người ta đi lại rất khó khăn.
Sụn khớp bị mòn theo thời gian là quy luật tự nhiên của các bộ phận cơ thể già đi theo
tuổi tác. Tại buổi nói chuyện về căn bệnh này ở TP.HCM mới đây, TS-BS Phạm Chí
Lăng cho biết tình trạng mòn sụn khớp của đầu dưới xương đùi và đầu trên xương
chày gây đau đớn và làm giảm chức năng vận động là bệnh lý thường gặp. Nếu sụn
khớp này mòn nhiều sẽ xuất hiện triệu chứng đau khi ngồi xuống, đứng lên, đau khi đi
lại, khi lên cầu thang. Nếu đau cả khi đi lại bình thường thì tình trạng bệnh đã nặng. Có
người đi bộ chừng 50 hay 100 m là phải dừng lại vì đau quá; có người đau không thể
nhấc chân lên cầu thang được.
Nhiều nguyên nhân
Theo TS-BS Phạm Chí Lăng, bình thường sụn khớp trơn láng, sắc cạnh, nó lấp đầy
khoảng trống giữa hai xương, nhưng khi bị mòn nhiều thì khoảng cách giữa hai xương
trống ra. Nhìn bằng mắt thường có thể thấy khớp gối cong hình chữ C, nặng hơn thì
cong hình chữ O. Khi sụn khớp mòn nhiều quá, sụn tróc để lộ xương ra, lúc đó hai
xương sẽ chạm vào nhau nên khi cử động đau rất dữ. Thường thì khớp gối sẽ mòn bên
trong nhiều hơn mòn ở mặt ngoài.
2. Về nguyên nhân, theo TS-BS Phạm Chí Lăng, nếu là nguyên phát thì do tuổi tác, dư
trọng lượng cơ thể; về thứ phát là do viêm đa khớp dạng thấp, chấn thương (gãy
xương, phạm khớp, đứt dây chằng chéo, rách sụn chêm).
Từng giai đoạn
Khi bệnh ở giai đoạn sớm, biểu hiện là: người bệnh đi trên đất bình thường, đau khớp
gối khi ngồi xổm đứng lên, khi lên xuống cầu thang, khớp gối không bị sưng, không
biến dạng, X-quang khớp gối cho thấy thoái hóa độ 1, độ 2, còn chụp MRI cho thấy gần
như khớp gối bình thường.
Ở giai đoạn trung bình, khả năng đi bộ sẽ giảm so với trước do đau, lên xuống cầu
thang phải nhích từng bước do đau, có những đợt viêm khớp gối (đau, tràn dịch khớp),
khớp gối có thể bị biến dạng (vẹo), X-quang cho thấy khớp gối thoái hóa độ 3, chụp
MRI thì thấy khớp gối rách sụn chêm, tổn thương mặt sụn.
Giai đoạn bệnh nặng, khả năng đi bộ giảm nhiều, lên xuống cầu thang rất khó khăn (có
người không thể đi được), khớp gối biến dạng nhiều...
Nếu phát hiện ở giai đoạn sớm (khi khớp gối chưa mòn) thì kết hợp 3 phương pháp
điều trị là: điều trị nội (uống thuốc giảm đau, chỉ uống khi có triệu chứng đau, thuốc
kháng viêm); tập thể dục nhẹ như chạy xe đạp, bơi lội - nhằm tăng sức cho bắp cơ đùi,
đầu gối; và cần phải giảm cân (giảm cân nặng là biện pháp không thể thiếu). Giai đoạn
trung bình thì điều trị nội khoa, nếu không đỡ sẽ phẫu thuật. Giai đoạn nặng có thể phải
phẫu thuật thay khớp gối.
