Mental health has been in the news quite a bit lately. Dozens of U.S. states are currently suing Meta for contributing to the youth mental health crisis by inserting addictive features into their products, while the U.S. Surgeon General is touring the nation to bring awareness to the growing epidemic of loneliness and isolation. The country has endured periods of low national morale, such as in the 1970s when high inflation and the energy crisis worsened public sentiment following the Vietnam War. The current mood, however, feels different. Gallup recently reported that national mental health is at an all-time low, with few bright spots to lift spirits.
To better understand how Americans are feeling and their attitudes towards mental health in general, ThinkNow conducted a nationally representative quantitative survey of 1,500 respondents and found some interesting differences among ethnic, age and gender groups.
Technology
For example, 52% agree that technology and social media have a negative impact on mental health, but when broken out by race, 61% of Whites felt technology had a negative effect, and only 48% of Hispanics thought it did.
While technology has helped us keep in touch with friends and family in faraway places, it appears to have degraded our ability to connect in person. Staying connected online is a double-edged sword since the same news feed that brings us pictures of the grandkids and fluffy kittens also feeds us news about the wars in Israel and Ukraine, the dysfunction in Washington, the latest mass shooting and the climate crisis.
Hispanics may have a built-in defense against the isolation technology breeds, owing to their large, multigenerational households, strong social support systems, and tendency to use social media to stay connected with relatives abroad.
Age and Gender
When asked how individuals rate their mental health, men rate it higher than women by 11 percentage points, and Baby Boomers rank it highest at 83%, saying it’s good or excellent vs. 57% of Gen Z saying the same.
Gen Z spends the most amount of time on social media, so the notion that social media negatively affects mental health appears to be correlated. Unfortunately, Gen Z is also the generation that’s least comfortable discussing mental health concerns with healthcare professionals. Only 40% of them state they’re comfortable discussing their issues with a professional compared to 60% of Millennials and 65% of Boomers.
Race Affects Attitudes
As seen in previous research conducted by ThinkNow, Asian Americans lag other groups when it comes to awareness of mental health issues. Twenty-four percent of Asian Americans believe that having a mental health issue is a sign of weakness compared to the 16% average for all groups. Asians are also considerably less likely to be aware of mental health services in their communities (42% vs. 55%) and most likely to seek out information on social media (51% vs. 35%).

Een rechtstreeks en permanent gevolg van lang-
durige lawaaiblootstelling is de ontwikkeling
van lawaai-traumatisch gehoorverlies dat in de
Angelsaksische literatuur beschreven wordt als
noise-induced hearing loss (NIHL). Dit wordt gety-
peerd door een blijvende beschadiging van de uit-
wendige haarcellen (UHC) in de cochlea, met als
resultaat een verminderde werking van de ‘coch-
leaire versterking’2
en een dip bij 4 kHz op het
conventioneel zuivere toonaudiogram.
Traditioneel gebruikt men zuivere toonaudiome-
trie voor het opsporen van dergelijk gehoorver-
lies. Met de ontdekking van otoakoestische emis-
sies (OAE’s) door David Kemp 1
in 1978 krijgen
onderzoekers en clinici een nieuw instrument in
handen om de integriteit en functie van de UHC
op een objectievere en accuratere manier te eva-
lueren.3,4
Verder onderzoek uit dezelfde periode
suggereert dat de OAE-techniek gehoorschade
ook veel vroeger aan het licht brengt. Veel vroe-
ger dan het klassieke zuivere toonaudiogram.4-7
Deze bevindingen zijn nog steeds onderwerp van
discussie. Daardoor is verwarring ontstaan op het
niveau van de eerstelijnsgezondheidszorg, meer
bepaald in het kader van het voeren van een pre-
ventief georiënteerd gehoorzorgbeleid.
In dit artikel trachten de auteurs op een kritische
manier de mogelijke rol van OAE’s te evalueren
in de context van een arbeidsgeneeskundige
dienst. De argumenten pro en contra die gefor-
muleerd worden, zijn ook van toepassing bij een
beleid ter voorkoming van gehoorschade door
recreatief geluid bij jongeren.
HET FYSIOLOGISCHE DIFFERENTIËLE
EFFECT VAN LAWAAI OP DE
SENSORIËLE STRUCTUREN
VAN DE COCHLEA
De inwendige haarcellen (IHC) en uitwendige
haarcellen (UHC) in het slakkenhuis zijn verant-
woordelijk voor de omzetting van de mechani-
sche stimulus, aangestuurd door de lopende golf,
in een receptorpotentiaal, die op zijn beurt zorgt
voor een verdere activatie van de neurale vezels
van de nervus acusticus (n. VIII).
De drie rijen UHC en de enkele rij IHC (figuur 1)
zijn parallel aan elkaar gelegen op hun hele ver-
loop van het slakkenhuis (van de basale tot aan
de apicale winding).
Lawaai veroorzaakt systematisch meer schade
aan de cellulaire structuur van de UHC dan van
de IHC. De belangrijkste reden voor dit verschil
in impact door lawaai op UHC en IHC, is dat de
verplaatsende kracht van de membrana tectoria
in de eerste plaats opgevangen wordt door de
UHC. Ten tweede zijn UHC, in tegenstelling tot
de IHC, niet omgeven door steuncellen, waar-
door ze ‘onbeschermd’ zijn tegen mechanische
stress. Ten slotte ligt de inplanting van de UHC
op de basilaire membraan veel dichter bij het
maximale uitwijkingspunt van de lopende golf
dan de inplanting van de IHC, waardoor de UHC
systematisch aan een grotere externe kracht
onderhevig zijn. Het gevolg is dat de UHC bij
overdreven en/of langdurige lawaaiblootstel-
ling sneller structurele schade oplopen dan de
IHC. Figuur 2 illustreert het verschil in impact
van lawaai op deze twee types sensoriële struc-
turen.
Dit inzicht maakt duidelijk dat bij screening op
gehoorschade (door industrieel of recreatief
lawaai) het accent veel meer op de integriteit en
goede werking van de UHC moet liggen en niet
zozeer op het registreren van de gehoordrempel
voor zuivere tonen. Zeker wanneer de screening
deel uitmaakt van een preventief georiënteerd
gehoorzorgbeleid voor bedrijven.

Ook de gevolgen voor het ‘spraakverstaan’ ver-
schillen duidelijk naargelang de IHC meer be-
schadigd zijn of de UHC. Uit een studie van
Liberman et al. blijkt dat bij schade van de UHC
het effect op het spraakverstaan beduidend gro-
ter is dan bij schade aan de IHC.8
Het screenen
van de UHC-functie is daarom belangrijk, niet
alleen vanuit een anatomisch-fysiologisch stand-
punt, maar ook en vooral vanuit een functioneel
standpunt.
DE OAE-TECHNIEK ALS MOGELIJK
ALTERNATIEF VOOR EN/OF
AANVULLING OP DE CONVENTIONELE
ZUIVERE TOONAUDIOMETRIE
OAE’s zijn laagniveaugeluiden die ontstaan door
de specifieke niet-lineaire eigenschappen van de
cochlea. Door ‘retrograde propagatie’ lekt een
deel van de akoestische energie, die in de cochlea
omgezet is in trillingsenergie, via het middenoor
terug naar de uitwendige gehoorgang. Dit ‘terug-
gekaatste’ geluid kan met een gevoelige minia-
tuurmicrofoon in het uitwendig oor worden
geregistreerd; dit noemt men een OAE. Met ande-
re woorden: OAE’s zijn een bijproduct (emissie)
van de elektromotiliteit (samentrekking) van de
UHC.
De twee meest gebruikte vormen zijn ‘transiënt’
geëvokeerde OAE’s (TEOAE’s) en ‘distorsiepro-
duct’ otoakoestische emissies (DPOAE’s) die uit-
gelokt worden door verschillende types van sti-
muli: TEOAE’s door een ‘click’-stimulus waarbij
nagenoeg alle UHC worden getriggerd en
DPOAE’s door twee primaire zuivere tonen, aan-
geboden met een gering frequentieverschil waar-
door een beperkte groep UHC gestimuleerd
wordt. Beide geven op een verschillende manier
feedback over de integriteit van de cochlea.
Experimenteel onderzoek in de laatste twee
decennia, zowel op dieren als op mensen, heeft
onomstotelijk bewezen dat de morfologische ver-
anderingen van de UHC door lawaai een afname
van de OAE’s veroorzaken.9-19
Op dit vlak bestaat
er geen enkele twijfel en is er een algemene con-
sensus dat de OAE-techniek het rechtstreeks
effect van lawaai en andere ototoxische agentia
op de UHC-functie van de cochlea registreert.
OAE’S ZIJN GEKENMERKT
DOOR EEN HOGE SENSITIVITEIT
EN SPECIFICITEIT
Vanuit de neurofysiologie is het nodige bewijs-
materiaal geleverd dat audiometrie gehoorscha-
de pas in een laat stadium zichtbaar maakt en dat
deze achterloopt op de realiteit. Figuur 3 toont de
resultaten van een studie van Altschuler et al.
waarbij de differentiële impact van een chemisch

| Uitwendige haarcellen lopen eerder structurele schade op bij
overdreven en/of langdurige lawaaiblootstelling dan inwendige
haarcellen.
| Met otoakoestische emissie (OAE)-techniek kan beschadiging van de
uitwendige haarcellen in cochlea vroegtijdig vastgesteld worden. Het is
dus een goed instrument voor preventieve screening op gehoorschade.
| OAE’s zijn een bijproduct (emissie) van de elektromotiliteit
(samentrekking) van de uitwendige haarcellen.
| Toonaudiometrie is niet gevoelig genoeg om gehoorschade in een
vroegtijdig stadium op te sporen.

ototoxisch agens (kanamycine) op de UHC en
IHC werd aangetoond.20
Uit de figuur blijkt dat ondanks belangrijke scha-
de aan de UHC er geen significante achteruitgang
is op het audiogram. Hieruit kan afgeleid worden
dat ototoxische agentia (zoals lawaai) de UHC
eerder beschadigen dan de IHC en dat het meten
van een drempelverschuiving (lees: audiometrie)
niet gevoelig genoeg is om deze schade op te spo-
ren. Dit doet twijfels rijzen over de validiteit van
toonaudiometrie in het kader van detectie van
ototoxische agentia en bevestigt dat deze tech-
niek niet gevoelig genoeg is om gehoorschade in
een vroegtijdig stadium op te sporen.
Dit werpt twee vragen op.
1. Is de OAE-techniek meer of minder geschikt
dan de klassiek gehanteerde toonaudiometrie
om ‘vroegtijdig’ gehoorschade op te sporen?
2. Welke van beide technieken is het meest
geschikt voor toepassing in een arbeidsge-
neeskundige context en in een preventief
kader?
Deze vragen werden onlangs door de Health &
Safety Executive (HSE) voorgelegd aan een groep
van internationale experts.21
In hun rapport vin-
den we onderstaande bevindingen en conclusies:
| Toonaudiometrie detecteert gehoorschade
pas vanaf het moment dat er aantasting is van
het vermogen om zuivere tonen te horen. Op
dat moment is de schade reeds aanzienlijk,
permanent en onomkeerbaar. Dergelijke
schade is veelal het gevolg van langdurige en
herhaalde blootstelling.
| Bij toonaudiometrie is er een zekere tijds-
spanne (time lag)tussen de blootstelling en
het moment dat de schade geconstateerd
wordt. Dit verklaart waarom maatregelen om
schade te voorkomen systematisch te laat
komen.
| Toonaudiometrie vereist strikte meetcondi-
ties. Niet opvolgen ervan (metingen buiten
audiometrische cabine, niet uitvoeren van
contralaterale maskering enzovoort) leidt tot
vermindering van de reproduceerbaarheid en
betrouwbaarheid, twee parameters die cruci-
aal zijn in een preventiecyclus waarbij metin-
gen met elkaar vergeleken worden (follow-
upstrategie).
| Toonaudiometrie is een subjectieve methode
en vereist een actieve en positieve medewer-
king van de persoon; zonder die medewer-
king is zo’n meting niet bruikbaar.
Op basis van deze conclusies kunnen we ons
terecht afvragen in hoeverre toonaudiometrie de
‘gouden standaard’ kan blijven voor de ‘vroegtij-
dige’ detectie van gehoorschade. Zeker in een
arbeidsgeneeskundige context, waar door tijds-
druk de meetcondities vaak minder strikt opge-
volgd worden, is de effectiviteit van toonaudio-
metrie discutabel.
Wat OAE’s betreft, stelt het rapport dat:
| er sterk bewijs is om OAE’s in te zetten voor
de opsporing en longitudinale monitoring
van risicogroepen;
| er voldoende bewijs is om personen op indi-
viduele basis te monitoren met OAE’s op
voorwaarde dat bij die persoon OAE’s geme-
ten kunnen worden bij de start van de pre-
ventiecyclus;
| het samen gebruiken van DPOAE’s en
TEOAE’s de meest veelbelovende combinatie
is;
| het een objectieve techniek betreft, waarbij
medewerking van de persoon geen rol speelt;
| metingen bij voorkeur uitgevoerd worden in
een stille ruimte, maar dat ze heel goed moge-
lijk zijn buiten een audiometrische cabine,
zonder dat dit afbreuk doet aan de betrouw-
baarheid en kwaliteit van het meetresultaat.
Wat de reproduceerbaarheid en betrouwbaarheid
van de OAE-techniek betreft, willen de auteurs
opmerken dat er, met name bij toepassing in een
arbeidsgeneeskundige context, nog andere facto-

5. ren een rol spelen, zoals het al of niet aanwezig
zijn van een temporary emission shift(TES), het
opleidingsprofiel en de technische vaardigheid
van de onderzoeker, de aanwezigheid van inter-
fererende pathologie (bijv. middenoorpathologie).
Het zou binnen de scope van dit artikel te ver lei-
den om deze in detail te bespreken, maar we wil-
len hiermee duidelijk maken dat, net zoals voor
andere technieken, ook de OAE-techniek alleen
in handen van een ‘professional’ zijn nut bewijst.
HUIDIGE BEPERKINGEN
VAN DE OAE-TECHNIEK
IN EEN PREVENTIEVE SETTING
De introductie van een nieuwe techniek gaat
meestal gepaard met beperkingen enthings to do.
Op dit moment ontbreken nog specifieke leef-
tijdsafhankelijke normen om tot een ISO-1999-
model te komen, zoals we dat kennen voor audio-
metrie. Hiervoor moeten er nog veel meer klini-
sche data verzameld worden. Deze beperking is
echter geen belemmering voor een follow-upstra-
tegie, zoals een gehoorzorgprogramma, waar
men een effectevaluatie uitvoert door resultaten
van een persoon (of een groep personen) te ver-
gelijken met een vorige meting.
Een tweede aspect is reeds aangehaald, namelijk
de vereiste kennis en kunde van de onderzoeker
om kwalitatief correcte metingen uit te voeren.
In het geval van de OAE-techniek zijn een correc-
te plaatsing van de meetmicrofoon in het gehoor-
kanaal, ervaring met otoscopie en zogenoemd
ear-canal managementvan groot belang. Bij voor-
keur doet men hiervoor beroep op degelijk opge-
leide professionals, zoals audiologen.
Een laatste beperking is dat er op dit moment nog
geen consensus is over de terminologie, en over
welke testparameters bij voorkeur gehanteerd
moeten worden, bijvoorbeeld de sterkte van de
stimulus.
BESLUIT
Otoakoestische emissies werden ontdekt in 1978.
Sinds hun ontdekking hebben ze een enorme ver-
andering in en aanpassing van het diagnostisch
beleid van vele klinische centra tot gevolg gehad,
met name op het gebied van neonatale gehoor-
screening en de differentiaaldiagnostiek van
ziektebeelden, zoals auditieve neuropathie.
Het objectief, niet-invasief en fysiologisch pre-
ventief karakter van de OAE-techniek is weten-
schappelijk sterk onderbouwd. Daarnaast zijn
OAE’s in de praktijk zeer goed toepasbaar als
screeningstechniek, vanwege:
| hoge test-hertestbetrouwbaarheid;
| hoge sensitiviteit om kleine veranderingen in
UHC-schade te detecteren (meet op de plaats
waar de schade ontstaat);
| snelheid (snelle meting en dus kostenbespa-
rend);
| eenvoud (patiënt hoeft zelf niets te doen);
| geen speciale infrastructuur (audiocabine)
vereist;
| economisch verantwoord (vergelijkbaar met
audioapparatuur).
De OAE-techniek is echter geen gehoortest! Het
evalueert de integriteit en functie van de UHC
van de cochlea en monitort dus een heel ander
deel van het auditieve systeem dan de zuivere
toonaudiometrie (corticale respons op zuivere
toonstimulatie). Vanuit deze optiek spreekt het
voor zich dat de OAE-techniek:
| uitermate betrouwbaar (hoge sensitiviteit) is
om gehoorschade door lawaai vroegtijdig op
te sporen;
| het toonaudiogram niet vervangt, maar zijn
eigen plaats verdient als ‘gouden standaard’
voor ‘vroegtijdige detectie’ van ototoxische
gehoorschade;
| in een preventief georiënteerd gehoorzorgbe-
leid complementair is aan het audiogram.
Een gehoorzorgbeleid met beide technieken in
een getrapte benadering lijkt de meest aangewe-
zen route. Een mogelijk model dat hierbij kan
worden gehanteerd, is een jaarlijkse monitoring
en follow-up met OAE. Bij significante achteruit-
gang van de UHC-functie wordt de werknemer
doorverwezen voor eendiagnostischaudiogram
(is niet gelijk aan een screening audiogram).
Het is evident dat de huidige beperkingen van de
OAE-techniek dienen te worden aangepakt, zoals
het bepalen van leeftijdsafhankelijke normen in
relatie tot blootstellingsniveau en blootstellings-
duur.
Het, omwille van deze beperking, niet implemen-
teren van deze methode zou een historische fout
zijn in de preventieve audiologie.
