This case report describes a 39-year-old man who presented with fever, cough, and shortness of breath for one week and was found to have COVID-19 pneumonia as well as spontaneous pneumomediastinum and subcutaneous emphysema. A CT scan showed COVID-19 pneumonia affecting 60% of the lung parenchyma along with pneumomediastinum and emphysema in the neck, chest, and arms. The patient was treated conservatively and showed improvement, with a repeat CT scan after one week showing significant regression of the pneumomediastinum.
