This document summarizes software testing issues from three cases: the Therac-25 radiation therapy machine which killed patients due to inadequate safety software; the Intel FDIV bug where a floating point division instruction was incorrectly implemented, leading to wrong results; and issues with assuming symmetry in the Intel FDIV bug that led to an error in the lookup table and inadequate testing to catch the bug before it was shipped.
