This document outlines the procedures and guidelines for an assessment on welding carbon steel plates and pipes using shielded metal arc welding (SMAW). The assessment will include a demonstration, oral questioning, and written test to evaluate core competencies. Examinees are reminded of facility locations, assessment rules, rights and appeals processes, and occupational health and safety practices to ensure a fair and secure assessment.
5. Reminders
For your personal needs the
comfort rooms and the canteen is
located outside of this building.
During the assessment, you are not
allowed to leave the premises of the
assessment center.
6. Dos and Donts
Before the assessment, any questions or
matter you want to clarify will be
entertained.
Once the assessment has started, please
refrain from asking questions, it is no
longer allowed.
If you have physical difficulties, please call
my attention so that we can address it
immediately.
7. Rights and Appeal System
In case you have questions
regarding the assessment result,
you have the right to appeal. Just
submit your appeal to the TESDA
provincial office where the
assessment center and the assessor
is accredited.
8. Issuance of National Certificate
The National Certificate can be claim at
TESDA provincial office within 3 to 5
working days after assessment. Bring
passport size picture with white
background, and also 100 pesos for
processing fee.
10. Occupational Safety and Health
Make sure to check the tools, materials
and equipment before using it.
Observe safety as you perform different
task.
Please call my attention in case of
accidents.
If you need some medicine, you can get it
on our medicine cabinet located at the
corner in this room.

If you found Not competent do worry about it because you can be re-assess anytime at your own convenience. If you failed full qualification, you need to undergo the whole process of the assessment.
But in COC, you are only going to be assess on the competencies that you were found not yet competent.
Reminder, Fail to pass the assessment 2 consecutive times, you will be advised to undergo a refresher course before applying a re-assessment.

We stablish strict confidentiality, to ensure your integrity. The result is only made available to authorized personnel and to the institution where you had your actual training. In relation to this, the assessment package is kept in a source place to guarantee the integrity of the assessment package. For this, may I ask all the candidates to regram from giving away any form of information that took place during the assessment process.
