Dr. Benedetta Allegranzi is a specialist in infectious diseases, tropical medicine, infection prevention and control and hospital epidemiology. She currently works at the World Health Organization HQ (Service Delivery and Safety department), leading the "Clean Care is Safer Care" programme. Since 2013, Dr Allegranzi has gathered the title of professor of infectious diseases in the official Italian professorship list and is adjunct professor attached to the Institute of Global Health at the Faculty of Medicine, University of Geneva, Switzerland. She closely collaborates with the team at the IPC and WHO Collaborating Center on Patient Safety, University of Geneva Hospitals (Geneva, Switzerland), as well as with the Armstrong Institute for Patient Safety and Quality, John Hopkins University, (Baltimore, USA) for clinical research projects. She is currently involved in the leadership on the WHO Ebola Response in the field of IPC and supervises IPC activities in Sierra Leone and Guinea. She has experience in clinical management of infectious diseases and tropical medicine, and clinical research in healthcare settings in both developing and developed countries. She has thorough skills and experience in training and education.

While the global burden of HCAI is unknown due to a lack of reliable data collection, studies in developed countries show that 5-15% of hospitalized patients and 9-37% of intensive care unit patients acquire HCAI. Common HCAI include surgical site infections, pneumonia, bloodstream infections, and gastrointestinal infections. Hand hygiene is considered one of the most effective measures for reducing the transmission of harmful pathogens and preventing HCAI.

Health care-associated infections (HCAI) affect hundreds of millions of patients worldwide each year. HCAI prolong hospital stays, induce long-term disabilities, increase costs for patients and health systems, and often result in tragic loss of life. While the global burden of HCAI is unknown due to a lack of reliable data collection, studies in developed countries show that 5-15% of hospitalized patients and 9-37% of intensive care unit patients acquire HCAI. Common HCAI include surgical site infections, pneumonia, bloodstream infections, and gastrointestinal infections. Hand hygiene is considered one of the most effective measures for reducing the transmission of harmful pathogens and preventing HCAI.

Abstract
Background: Adequate hand hygiene is considered the most effective measure to reduce transmission of nosocomial
pathogens.
Aims: To determine the effectiveness of infection control intervention to improve compliance with hand hygiene in the
Emergency Department, Al-Leith General Hospital, Saudi Arabia, and evaluate bacterial load on hands as a possible indi-
cator of improvement.
Methods: The study consisted of 3 phases: Phase I, measurement of basal hand hygiene compliance level; Phase II, mul-
timodal hand hygiene educational programme was initiated; and Phase III, hand hygiene compliance level was measured
again. Data were collected by direct observation of healthcare workers in the emergency department between October
2016 and March 2017, using the standardized World Health Organization method for direct observation, “Five Moments
for Hand Hygiene”. The intervention comprised health education sessions using direct personal contact. Hands of health-
care workers were sampled during Phases I and III by sterile bag method, and bacterial load was determined.
Results: A total of 1374 opportunities for hand hygiene were observed during the triphase study. Implementation of the in-
terventional hand hygiene educational programme significantly improved compliance with hand hygiene guidelines from
30.7% to 45.5% (P < 0.01). Log10 bacterial load per hand dropped from 4.97 (standard deviation = 0.32) to 4.57 (0.47) (P < 0.05).
Conclusions: Hand hygiene educational programmes were effective in improving compliance in the emergency depart-
ment, and bacterial load on hands of healthcare workers could be used as an indicator of improvement in hand hygiene
compliance.

However, global compliance rates remain low, especially in developing countries where 38% of facilities lack improved water sources, 19% lack improved sanitation, and 35% lack handwashing stations. Barriers to proper hand hygiene include inadequate infrastructure, lack of resources and time, inconsistent practices among different roles, and cultural and behavioral factors. The WHO's "Five Moments for Hand Hygiene" model outlines critical times for cleaning hands to prevent pathogen transmission.

Background
Hygiene is a very crucial factor in prevention of infection in any health care facility. Also, compliance of handwashing ensures patients safety, aids in the treatment and recovery of hospitalized patients. Hand hygiene is important action performed by healthcare works to prevent transmission of healthcare associated infection (Smiddy, O’Connell & Creedon, 2015). Health care professionals such as doctors, nurses, physical therapist and laboratory technicians, take the responsibility of providing efficient, effective and quality care that will improve the health of their patients.
The purpose of this paper is to discuss the change proposal project components the author has been working on throughout the course. The goal of health care works rendering a quality, effective and efficient care to their patient in the health care setting will be difficult to achieve if the rate handwashing adherence is below expectation. Unclean healthcare environments harbor germs that can cause disease, thereby placing the patient at risk of developing infection instead of recovering from their present health condition. Healthcare providers inability to comply with hand hygiene is one the main reason patient develop hospital acquired infections (HAIs). Healthcare employees have the lives of patient in their hands therefore, hand hygiene should not by any means be neglected or dominated out in any healthcare facility.
Approximately 250 health care specialists in a Metro Detroit facility happened to be watched and assessed directly; prior to the start of the exercise, participants were selected based on their hand washing comprehension and compliance. Partakers expresses that they observed improvement on handwashing practices and that most nurses complied to hand washing guidelines evidenced by some significant reductions in the rate of transmission of HAIs within the healthcare facility. HAIs are the infections a patient acquire during the period of hospitalization. The result of the research showed a huge decrease in the spread of nosocomial infections due to progress of hand hygiene training and nurses’ compliance to handwashing protocols. These infections mostly manifest during or after 48 hours of admission or thirty days after discharge from the hospital or health-care facility. The author of this research study sees HAIs as a dangerous disease with many complications. Because inadequate handwashing practices by healthcare workers are the main cause of spread of hospital acquire infections, it is important to educate staff members on proper hand hygiene, implement plan to encourage hand hygiene compliance in the healthcare settings. Blood-stream, ulcers / surgical wounds, CAUTI and respiratory infections are the most common types of HA.
