NegBio is a tool that detects negation and uncertainty in radiology reports with high accuracy. It first uses MetaMap to map report text to medical concepts, then applies dependency parsing and rules to identify negative and equivocal findings. Evaluated on several datasets, NegBio significantly outperformed previous state-of-the-art methods, achieving F-scores over 95% in some cases. Future work includes exploring its use in other clinical texts beyond radiology reports. NegBio is available as open source software.
Text Mining Radiology Reports for Deep Learning Radiology Images
Although hospitals have accumulated a tremendous number of radiology images and reports, using them to build data-hungry deep-learning models remains challenging. This is partially because manually annotating a dataset in large scale is costly. Here, we propose using text mining to automatically generate a weakly-labeled dataset for detecting thoracic diseases in radiology images. This work represents the first attempt to unify text mining with computer vision for medical imaging analysis in the era of deep learning.

The p-medicine project is presented. It deals with the creation of an integrative infrastructure for Personalised Medicine, which aims to accelerate personalized medicine and personal clinical research. For this purpose p-medicine developed a comprehensive set of software tools, including ObTiMA data management system, Patient Empowerment Tool, data
mining, data warehousing, biobank access, decision support, image annotation (DrEye) and simulation (Oncosimulator). Here we show the evaluation of some of the p-medicine tools for their suitablity to perform clinical trials. Is their usage conform with regulations and standards (GCP, GDPR, GAMP, computer system validation)? Can these tools be integrated into the existing systems (IT infrastructure / organisational framework) of an international clinical trials network (ECRIN)? To perform clinical trials, a legal and ethical framework based on international requirements and approved concepts for data security must be adopted. GCP (Good Clinical Practice) is such an international ethical and scientific quality standard for designing, recording and reporting trials that involve the participation of human subjects.
Evaluation of usability of p-medicine software tools for clinical trials was done with two surveys: (1) survey of p-medicine tools in the ECRIN network and (2) p-medicine developer survey. The tool integration topics contained questions about the employment of the right Clinical Data Management System (CDMS) at the many ECRIN centres. There is competition between different solutions, like VISTA (EORTC) MACRO, secuTrial, RAVE, OpenClinica. CDMS should be usable for all types of trials and the usability in clinical trials must be demonstrated by integration of biobank access / safety functions. Only ObTiMA is able to specifically address the challenges of personal medicine clinical trials. The results of the evaluation was that there exists some compliance gaps for quality management during software development, no complete GCP compliance yet and the missing of a robust business model for software sustainability. To address the latter, a Reciprocal Integration approach was developed to integrate p-medicine tools into clinical research networks.

Use of personalized medicine tools for clinical research networks
Patient-centric clinical trials can gain enormously from the employment of personalised medicine tools. Here we address software tools created by the p-medicine network, which developed thr ObTiMA data management system, Patient Empowerment Tool, data mining, data warehousing, biobank access, decision support, image annotation (DrEye) and simulation (Oncosimulator). We evaluated of some of these tools for their suitablity to perform clinical trials. Is their usage conform with regulations and standards (GCP, GDPR, GAMP, computer system validation)? Can these tools be integrated into the existing systems (IT infrastructure / organisational framework) of an international clinical trials network (ECRIN)?

The p-medicine project is presented. It deals with the creation of an integrative infrastructure for Personalised Medicine, which aims to accelerate personalized medicine and personal clinical research. For this purpose p-medicine developed a comprehensive set of software tools, including ObTiMA data management system, Patient Empowerment Tool, data
mining, data warehousing, biobank access, decision support, image annotation (DrEye) and simulation (Oncosimulator). Here we show the evaluation of some of the p-medicine tools for their suitablity to perform clinical trials. Is their usage conform with regulations and standards (GCP, GDPR, GAMP, computer system validation)? Can these tools be integrated into the existing systems (IT infrastructure / organisational framework) of an international clinical trials network (ECRIN)? To perform clinical trials, a legal and ethical framework based on international requirements and approved concepts for data security must be adopted. GCP (Good Clinical Practice) is such an international ethical and scientific quality standard for designing, recording and reporting trials that involve the participation of human subjects.
Evaluation of usability of p-medicine software tools for clinical trials was done with two surveys: (1) survey of p-medicine tools in the ECRIN network and (2) p-medicine developer survey. The tool integration topics contained questions about the employment of the right Clinical Data Management System (CDMS) at the many ECRIN centres. There is competition between different solutions, like VISTA (EORTC) MACRO, secuTrial, RAVE, OpenClinica. CDMS should be usable for all types of trials and the usability in clinical trials must be demonstrated by integration of biobank access / safety functions. Only ObTiMA is able to specifically address the challenges of personal medicine clinical trials. The results of the evaluation was that there exists some compliance gaps for quality management during software development, no complete GCP compliance yet and the missing of a robust business model for software sustainability. To address the latter, a Reciprocal Integration approach was developed to integrate p-medicine tools into clinical research networks.

This funding allowed Sentara Healthcare to take a systems approach to implementing NHSN surgical site infection (SSI) surveillance. They used $124,000 for information technology assistance to develop an electronic process for exporting surgical procedure data directly into NHSN. They also used $21,000 to train infection prevention and control staff as well as data auditors on SSI surveillance. This funding demonstrated how a large healthcare system can collaborate across departments like IT, infection prevention, and data auditors to efficiently implement NHSN reporting requirements.

During the last two decades Clinical Decision Support (CDS) standards and technologies have progressed significantly to develop them as more robust and scalable systems. However, the current context of medicine sets high demands in aspects such as interoperability to enable the use of EHR data in CDS systems, the need to establish communication challenges to include the patient as an active component in decision making, collaborative learning and sharing CDS systems across institutional borders, to name a few.
In this thesis I tackle some of these challenges. In particular, I evolve previous conceptual computerized decision support frameworks and I postulate a CDS systems environment where different models interact to enable:
• Secondary use of data for CDS systems: The dissertation presents a model to leverage different developments in data access and standardization of medical information. The result is an openEHR-based Data Warehouse architecture that enables access, standardization and abstraction of clinical data for CDS systems. The architecture allows: a) to access heterogeneous data sources; b) to standardize data into openEHR to grant interoperability of data; and c) to exploit an openEHR repository as a Data Warehouse that allows querying data in a technology-independent format (the Archetype Query Language).
• CDS systems semantic specification: The semantic model proposed exploits the paradigm of Linked Services to unambiguously describe CDS systems in a machine- understandable fashion. This grants ontological descriptions of functional, non- functional and data semantics. These descriptions facilitate to overcome some of the barriers in CDS functionality sharing. In particular, the semantic model proposed allows using expressive queries to discover CDS services in health

networks, and analyzing CDS systems interfaces to understand how to interoperate with
them.
• Effective patient-CDS systems interaction: the dissertation proposes a method to
evaluate the communication process between patients and consumer-oriented CDS systems. The method aims for detecting if important human-computer interaction barriers that could lead to negative outcomes are present in CDS systems user interfaces.
Patient Privacy Control for Health Care in Cloud Computing System
This document describes a cloud-based healthcare system that aims to provide secure sharing of patient health information between healthcare providers while maintaining patient privacy. The system uses authentication and access control schemes along with encryption techniques like attribute-based encryption to restrict access to patient data based on user attributes. It presents the design of the system architecture, which includes patient, doctor and lab units. It also outlines the main algorithms used in the system for key generation, signing data, verifying signatures, and simulating transcript data to protect patient identities. The goal of the system is to enable secure telemedicine and remote diagnosis capabilities while ensuring patient privacy in distributed cloud healthcare computing.
