This document discusses high reliability organizations and their preoccupation with failure, reluctance to simplify situations, sensitivity to operations, and commitment to resilience. It also discusses several human factors lessons learned from wildland firefighting incidents, including underestimating fire behavior and overestimating ability to control fires, reluctance to change plans when needed, importance of personal knowledge and experience, and importance of considering worst-case scenarios.
