Risk assessment template

This risk assessment form evaluates the risks associated with a fight scene involving body contact and furniture being used as a weapon. The production will take place at Sunderland College and involve actors Mayank Kumar and Clark Brettwood. Hazards identified include assault by a person from realistic fighting, and objects moving or flying due to being hit with a table multiple times. Controls like trained actors and rehearsals are planned to minimize risks, but risks remain medium for being assaulted and hit with objects. The assessment was completed and authorized by Mayank Kumar.
