treatment and aftercare plans.
SENIOR SOCIAL WORKER October 2008-May 2015
 Conducted patient/family intake process to obtain information required to formulate a
biopsychosocial evaluation and social work treatment plan for patients living with HIV/AIDS
diagnosis, psychiatric patients, inpatient adult medicine, adult ICU, and emergency room;
 Completed psychosocial evaluations assessing the weaknesses and strengths of individuals
and/or families;
 Reported suspected cases of abuse or neglect to the New York Central Registry; Evaluates
alternate care arrangements for the elderly and other patients, where needed; planned for
institutional or hospital discharge and other post institutional/hospital care;
 Participated in telemanagement program to reduce readmission rate for patients diagnosed with
congestive heart disease to manage their health and improve the quality of their lives through
early symptom recognition, continuity of care, identifying strength and weaknesses in home
environment, diet planning and medication management.
 Provided direct counseling services and/or outreach services; develop treatment plan and
implement referrals to other public agencies or community resources as needed;
 Conferred and consulted with professional and technical personnel in implementing a multi-
disciplinary approach to patient care;
 Provided therapeutic interventions, drawing upon a wide range of treatment modalities, to high-
risk and complex cases, coordinate case conferences as needed;
