1. STEP #1 CREATE LOGON CREDENTIALS.
Open any browser application. Copy and paste or type
the following URL into your browser address field:
www.rehabcarestore.com. (Use your existing credentials
if you already have an account created.) Please save your
credentials so you can retrieve them if you forget them.
STEP #2 CLICK “PERSONAL PURCHASES
STORE” BUTTON in grey (located in left column of
site once user is logged in).
STEP #3 SELECT CATEGORY. The initial screen
will list the categories of products available to you. Simply
select the image to proceed to the product page.
STEP #4 SELECT ITEMS.
STEP #5 ENTER YOUR FACILITY
INFORMATION AND CAREFULLY REVIEW
PROOF (if applicable). The proof that you see and
approve is exactly what will be printed and shipped to
you. Add to order.
STEP #6 PROCEED TO SHOPPING CART
and enter required fields.
STEP #7 ENTER SHIPPING INFORMATION.
STEP #8 ENTER CREDIT CARD
INFORMATION.
STEP #9 CONFIRM AND SUBMIT ORDER.
Complete your order by clicking the “Submit” button.
Your order will be confirmed by the assignment of a
unique order ID. Please reference this number if you have
any inquiries.
You should receive a confirmation e-mail after placing your
order. If you do not receive this, this means the order was
not received and will not be processed.

department has made our professionally designed and written collateral available for our partners to order
on rehabcarestore.com – where they can pay with a credit card. Our partners will only pay for the cost of
printing and shipping, and the materials are mailed directly to their facility or home address.
