community. This year also brings a
change to the Liberty Area Chamber of
Commerce. Our offices have movedfrom
the historic building at 9 South Leonard,
near downtown Liberty.
Now our offices are convenient to all
Liberty area businesses. We moved into
our new offices March 1st, which are
located in the Westowne Center near the
intersection of Clayview Dr. & Kansas
St./152 Hwy.
Some of the new features in the office
include artwork of the Liberty area and
we have had all of our Hamil prints
reframedsothattheyarecomplementary
to our new decor. Also available is
Chamber WiFi to those attending
meetings in the office.
Our Ribbon Cutting ceremony and Open
House will be held, Wednesday, March

An innovative solution
Offering an innovative blend of short- and long-term
disability coverage, Assurant Employee Benefits’
Serious Disability plan offers benefits from the first
day up to six months for claimants who are initially
hospital confined for 72 hours or longer and unable
to work at any gainful occupation. Benefits can con-
tinue up to age 65 when a more seriously disabled
claimant qualifies under our patent-pending impair-
ment rating definition of disability.
Disability can be devastating financially as well. Just
think of all of the bills you pay every month. Would

additional medical bills as a result of their disability?

What sets Serious Disability apart
from other disability coverage?
New to the industry, the Serious Disability Benefit
offers a very different approach to disability than
we’ve historically taken. Traditionally, disability
coverage may have been viewed as complex.
With Serious Disability, we’ve created a simple ap-
proach to disability claims that allows us to quickly
and objectively measure whether or not the em-
ployee is eligible for benefit payments. We make
every effort to process claims as quickly as possible
to minimize financial strain on your employees.
For particular companies, disability coverage can
be expensive due to high-risk occupations, existing
health conditions in the employee group or prob-
lems with maintaining participation levels.
With Serious Disability, our definitions of disability
allow us to contain the cost, so these other factors are
no longer part of the mix. Rather than limiting ben-
efits, we’ve limited the claims that qualify to only
those conditions considered more serious, using an
initial hospital confinement period, followed by our
patent-pending impairment rating definition.
A real benefit for the truly disabled
While the objective definitions of disability used
with Serious Disability mean fewer individuals
will qualify for benefits, the truly disabled receive
the financial assistance needed when seriously
impaired.

Clear, objective qualification
for benefits
Initial qualification for the first six months of benefits
requires:
• continuous hospital confinement of 72 hours or more
OR
• being diagnosed with a terminal condition
(12 months or less life expectancy)
Qualification for benefits after the first six months
requires:
• the condition must result in a 51% whole person
impairment rating (or extreme impairment for
psychiatric conditions)
OR
• being diagnosed with a terminal condition
(12 months or less life expectancy)
Claimants meeting the above qualifications must also
be unable to do one of the material duties of each
gainful occupation for which they are qualified.
Common conditions frequently
allow qualification for benefits
While 72-hour continuous hospital confinement may
seem a rare occurrence, many conditions would enable
a claimant to qualify for benefits.
Any condition meeting the 72-hour confinement quali-
fication becomes payable from the first day of hospital
confinement. Given the relative cost of Serious Dis-
ability coverage, internal data shows that a minimum
72-hour stay will return 128% of the annual premium
for those 72 hours of benefits. Those claimants who
continue to qualify for benefits during the recovery
period following hospitalization may realize even
greater value.
Common events and conditions that can require ex-
tended hospital stays for adults include:
• Injuries
• Poisoning
• Fractures
• Heart disease
• Cancer
• Pneumonia
• Mental Illness
• Alcohol and/or drug abuse
The impairment rating
definition of disability
After the initial six months of disability, our patent-
pending impairment rating definition of disability is
a unique and objective way to determine qualification
for continued disability benefits. It places the focus
on the actual sickness or injury, rather than a person’s
occupation, other types of work that could be done, or
how much money a person can or cannot earn.
Impairment ratings reflect the severity of medical con-
ditions and the degree to which they limit an individ-
ual’s ability to function. A rating is determined by the
results of a “whole person impairment rating”
calculation, which must be performed by a certified
physician using “The Guides to the Evaluation of Per-
manent Impairment”, Fifth Edition, published by the
American Medical Association (“The AMA Guides”).
Using medical records, clinical, and examination
results, The AMA Guides require a defined process
and exacting standards for determining the numeri-
cal whole person impairment rating. This results in a
clear and objective benchmark for benefits qualifica-
tion. When the rating meets or exceeds our require-
ment of 51%, indicating a very serious disability (or
extreme impairment for psychiatric conditions),
benefits are paid to age 65 or for two years, whichever
is longer.
A clear need for employees
Nearly one in every three Americans will be disabled for three months or longer before
reaching age 65.1
Yet less than 20 percent of full-time employees of small employers
(1-99 employees) have access to long-term disability coverage.2
Perhaps their employ-
ers have been unable to afford disability coverage or they work in an industry to which
disability coverage has not been made readily available.
Fifty-eight year old Sam*
was one of our first Serious
Disability claimants. His claims analyst describes him
as a “dedicated 30-year employee at a lumber mill
who, because of his high-risk industry and limited
salary, probably wouldn’t have had the opportunity for
group coverage any other way. He continued to work
when he was very ill but, because of Serious Disability,
took the time off from work to get treatment and then
returned to work after a seven-day hospital stay and a
short recovery.”

Eligibility
guidelines
To be eligible for coverage, an
employee must work 30 hours
per week, year-round.
If the insured is not at work, the
effective date of the coverage is
deferred until the date the insured
returns to active full-time work.
Limitations
and exclusions
The Serious Disability policy does
have limitations and exclusions,
including, war, self-inflicted inju-
ries, disability resulting from the
commission of a felony, partici-
pating in professional sports, or
racing, disability while under the
influence of a controlled sub-
stance, or being intoxicated or un-
der the influence of any narcotic
unless administered according
to a doctor. Drug/chemical dis-
abilities are excluded for certain
industries.
In addition, if an employee is of-
fered the opportunity to return to
limited work, and is functionally
capable, but does not return to
work, his or her benefits may be
terminated.
Coverage will not go into effect
unless the minimum participation
requirement is met. We reserve
the right to cancel the policy with
written notice to the policyholder
in the event participation falls
below the required amount or
premiums are not paid.

but rather an overview of avail-
able coverage.

Serious Disability Benefit A product that can protect when it’s needed the most Clear, objective qualification for benefits Initial qualification for the first six months of benefits requires: • continuous hospital confinement of 72 hours or more OR • being diagnosed with a terminal condition (12 months or less life expectancy) Qualification for benefits after the first six months requires: • the condition must result in a 51% whole person impairment rating (or extreme impairment for psychiatric conditions) OR • being diagnosed with a terminal condition (12 months or less life expectancy) Claimants meeting the above qualifications must also be unable to do one of the material duties of each gainful occupation for which they are qualified. Common conditions frequently allow qualification for benefits While 72-hour continuous hospital confinement may seem a rare occurrence, many conditions would enable a claimant to qualify for benefits. Any condition meeting the 72-hour confinement quali- fication becomes payable from the first day of hospital confinement. Given the relative cost of Serious Dis- ability coverage, internal data shows that a minimum 72-hour stay will return 128% of the annual premium for those 72 hours of benefits. Those claimants who continue to qualify for benefits during the recovery period following hospitalization may realize even greater value. Common events and conditions that can require ex- tended hospital stays for adults include: • Injuries • Poisoning • Fractures • Heart disease • Cancer • Pneumonia • Mental Illness • Alcohol and/or drug abuse The impairment rating definition of disability After the initial six months of disability, our patent- pending impairment rating definition of disability is a unique and objective way to determine qualification for continued disability benefits. It places the focus on the actual sickness or injury, rather than a person’s occupation, other types of work that could be done, or how much money a person can or cannot earn. Impairment ratings reflect the severity of medical con- ditions and the degree to which they limit an individ- ual’s ability to function. A rating is determined by the results of a “whole person impairment rating” calculation, which must be performed by a certified physician using “The Guides to the Evaluation of Per- manent Impairment”, Fifth Edition, published by the American Medical Association (“The AMA Guides”). Using medical records, clinical, and examination results, The AMA Guides require a defined process and exacting standards for determining the numeri- cal whole person impairment rating. This results in a clear and objective benchmark for benefits qualifica- tion. When the rating meets or exceeds our require- ment of 51%, indicating a very serious disability (or extreme impairment for psychiatric conditions), benefits are paid to age 65 or for two years, whichever is longer. A clear need for employees Nearly one in every three Americans will be disabled for three months or longer before reaching age 65.1 Yet less than 20 percent of full-time employees of small employers (1-99 employees) have access to long-term disability coverage.2 Perhaps their employ- ers have been unable to afford disability coverage or they work in an industry to which disability coverage has not been made readily available. Fifty-eight year old Sam* was one of our first Serious Disability claimants. His claims analyst describes him as a “dedicated 30-year employee at a lumber mill who, because of his high-risk industry and limited salary, probably wouldn’t have had the opportunity for group coverage any other way. Limitations and exclusions The Serious Disability policy does have limitations and exclusions, including, war, self-inflicted inju- ries, disability resulting from the commission of a felony, partici- pating in professional sports, or racing, disability while under the influence of a controlled sub- stance, or being intoxicated or un- der the influence of any narcotic unless administered according to a doctor. Drug/chemical dis- abilities are excluded for certain industries. In addition, if an employee is of- fered the opportunity to return to limited work, and is functionally capable, but does not return to work, his or her benefits may be terminated. Coverage will not go into effect unless the minimum participation requirement is met. We reserve the right to cancel the policy with written notice to the policyholder in the event participation falls below the required amount or premiums are not paid.
