Jennifer Naylor, an RPN from Guelph, discusses leading from the frontlines to implement best practices for preventing falls in long-term care homes. She describes establishing a community of practice to share knowledge and resources between homes, support challenges, access expertise, adopt evidence-based practices from RNAO guidelines, and problem solve. Naylor also discusses performing a gap analysis, stakeholder analysis, and champions workshop using RNAO tools to help implement practice changes to prevent falls according to the 4 Ps of hourly rounding protocol.

Pain: Does that resident complain of or appear to be in pain? If yes,
inform the nurse.
Personal needs: Does the resident need to use the toilet? Check
incontinent brief. Ensure resident is clean and dry.
Position: Ensure resident is repositioned and placed safely in
bed/chair. Is the bed in the lowest position? Are fall out mats in
place?
Possessions: Ensure that call bell, TV remote, walker/wheelchair are
all within reach. Check that the environment is clean and no fall
hazards are present.
