This document is a pledge form for donations to support the Lakeview High School Tennis Building Project. Donors can pledge a total donation amount and choose to pay it over one to five years, agreeing to pay the full pledge by a specified date. Donors will be recognized on a plaque at the new tennis building for donations of $1,000 or more, or can sponsor an individual tennis court for $12,000 with the option to include multiple names. The pledge form requests the donor's contact information and preferred name format for recognition.
