Mental health has been in the news quite a bit lately. Dozens of U.S. states are currently suing Meta for contributing to the youth mental health crisis by inserting addictive features into their products, while the U.S. Surgeon General is touring the nation to bring awareness to the growing epidemic of loneliness and isolation. The country has endured periods of low national morale, such as in the 1970s when high inflation and the energy crisis worsened public sentiment following the Vietnam War. The current mood, however, feels different. Gallup recently reported that national mental health is at an all-time low, with few bright spots to lift spirits.
To better understand how Americans are feeling and their attitudes towards mental health in general, ThinkNow conducted a nationally representative quantitative survey of 1,500 respondents and found some interesting differences among ethnic, age and gender groups.
Technology
For example, 52% agree that technology and social media have a negative impact on mental health, but when broken out by race, 61% of Whites felt technology had a negative effect, and only 48% of Hispanics thought it did.
While technology has helped us keep in touch with friends and family in faraway places, it appears to have degraded our ability to connect in person. Staying connected online is a double-edged sword since the same news feed that brings us pictures of the grandkids and fluffy kittens also feeds us news about the wars in Israel and Ukraine, the dysfunction in Washington, the latest mass shooting and the climate crisis.
Hispanics may have a built-in defense against the isolation technology breeds, owing to their large, multigenerational households, strong social support systems, and tendency to use social media to stay connected with relatives abroad.
Age and Gender
When asked how individuals rate their mental health, men rate it higher than women by 11 percentage points, and Baby Boomers rank it highest at 83%, saying it’s good or excellent vs. 57% of Gen Z saying the same.
Gen Z spends the most amount of time on social media, so the notion that social media negatively affects mental health appears to be correlated. Unfortunately, Gen Z is also the generation that’s least comfortable discussing mental health concerns with healthcare professionals. Only 40% of them state they’re comfortable discussing their issues with a professional compared to 60% of Millennials and 65% of Boomers.
Race Affects Attitudes
As seen in previous research conducted by ThinkNow, Asian Americans lag other groups when it comes to awareness of mental health issues. Twenty-four percent of Asian Americans believe that having a mental health issue is a sign of weakness compared to the 16% average for all groups. Asians are also considerably less likely to be aware of mental health services in their communities (42% vs. 55%) and most likely to seek out information on social media (51% vs. 35%).

39. PERCHE’ L’ALCOL FA MALE?
TOLLERANZA: necessità di assumere la sostanza in dosi sempre più alte per
ottenere l’effetto desiderato con marcata riduzione degli effetti quando la sostanza
viene assunta nella dose abituale.
ASTINENZA: sono gli effetti fisici e psicologici che si manifestano quando la
persona smette di assumere la sostanza o ne riduce la quantità.
Le 2 principali caratteristiche dei disturbi da uso di sostanze nel DSM-5 sono:

40. CARATTERISTICHE DELL’USO DELL’ALCOL
L’abuso di alcol si inserisce quasi sempre in
un contesto di poliabuso.
L’80-85% dei soggetti che abusano
dell’alcol sono anche fumatori.
Questo alto livello di comorbilità si spiega
col fatto che l’alcol e la nicotina sono
sostanze con tolleranza incrociata: ovvero
la nicotina può indurre tolleranza per gli
effetti di ricompensa dell’alcol e viceversa.
Il consumo di entrambe le sostanze può aumentare per mantenere inalterato il livello di
gratificazione.
41. QUALI SONO GLI EFFETTI DELL’ALCOL?

GLI EFFETTI A BREVE TERMINE DEL’ALCOL
● Dopo essere stato ingerito, l’alcol (etanolo) inizia a
essere metabolizzato.
● Passa per la maggior parte del tempo nel intestino
tenue e da qui viene assorbito nel sangue.
● L’alcol viene poi degradato principalmente nel
fegato che trasforma l’alcol in acetaldeide, che con il
tempo non riesce più a eliminare e che provoca un
primo stato di ubriachezza.
Le quantità eccedenti restano nel circolo sanguigno.
43. GLI EFFETTI A BREVE TERMINE DELL’ALCOL
Attraverso il circolo sanguigno raggiunge anche
i sistemi neurali del cervello:
● Stimola i recettori GABA → allevia la
tensione.
● Incrementa i livelli di serotonina e dopamina
→ produce effetti piacevoli.
● Blocca i recettori del glutammato →
rallentamento del pensiero e perdita di
memoria (effetti cognitivi dell’intossicazione).
44. GLI EFFETTI A BREVE TERMINE DELL’ALCOL
Lo stato di ubriachezza si amplifica e perdura a
causa della produzione di tetraidropapaverolina
(precursore della morfina) nel cervello partendo
dall’acetaldeide.
Questa sostanza è la causa che provoca
dapprima una sensazione di euforia breve, di
piacere e di aumento di fiducia ma poi a seconda
delle quantità ha un effetto sedativo, riduce le
percezioni, e ti fa entrare in uno stato di
confusione. Tutto questo può portare ad un grave
stato di depressione.

45. GLI EFFETTI A LUNGO TERMINE DELL’ALCOL
L’alcol causa effetti avversi su quasi tutti i tessuti e gli organi del corpo:
● Fornisce calorie "vuote" (prive di nutrimenti
essenziali) → malnutrizione.
● Compromette i processi digestivi e l’assorbimento
di vitamine (B).
Infatti nelle persone di età più avanzata che hanno
fatto abuso cronico di alcol, la carenza di vitamine B
può provocare la sindrome amnesica →
confabulazione (il loro racconto non è lineare
perché cercano di colmare i vuoti di memoria
inventando «storielle»).
46. GLI EFFETTI A LUNGO TERMINE DELL’ALCOL
CIRROSI EPATICA
La riduzione dell’apporto proteico può portare alla cirrosi epatica: una malattia cronica e
degenerativa del fegato nella quale le cellule epatiche si riempiono di grassi e proteine che
ne impediscono il normale funzionamento. Alcune cellule muoiono e scatenano un
processo infiammatorio. E’ uno dei principali fattori di rischio per l’epatocarcinoma.
Con lo sviluppo di tessuto cicatriziale il flusso ematico nel fegato viene ostruito.
47. GLI EFFETTI A LUNGO TERMINE DELL’ALCOL
LA CIRROSI EPATICA NON SI PUÒ CURARE!
La terapia medica si limita a rallentarne
l'evoluzione, individuando la causa che l'ha
provocata e tentando poi di eliminarla o
controllarla tramite farmaci specifici, interventi
chirurgici e misure comportamentali.
La cirrosi è una delle prime 10 cause di morte
nel mondo occidentale.
Negli USA è al 12° posto con più di 30 000
morti all’anno.
48. ALTRI EFFETTI DELL’ALCOL
Altre manifestazioni comuni:
- Sindrome alcolica fetale (mamme in gravidanza)
→ ritardo mentale, malformazioni (neonato).
- Danni in diverse aree cerebrali molte delle quali
coinvolte nelle funzioni mnestiche (relativi alla
memoria).
- Danni alle ghiandole endocrine e al pancreas.
- Insufficienza cardiaca.
- Ictus.
- Ipertensione.
- Emorragie dei capillari.
Quest’ultime sono responsabili del gonfiore e del colore
rosso del naso tipico degli alcolisti.
49. ALTRI EFFETTI DELL’ALCOL
Delirium tremens
Stato raro di confusione mentale che si
manifesta in un individuo che abbia assunto
forti quantità di alcol per diversi anni e
quando la concentrazione di alcol nel sangue
diminuisce improvvisamente.
Delirio, tremori, allucinazioni visive, uditive e
tattili.
Purtroppo l’alcol non ha più effetto in queste
condizioni ma è il sistema nervoso centrale
malfunzionante il responsabile principale.
50. VALORI DI ALCOLEMIA ED EFFETTI
● 0,2 g/L: aumenta la tendenza ad agire in modo imprudente dal momento che i riflessi sono leggermente
disturbati in virtù di una ridotta percezione del rischio;
● 0,4 g/L: i movimenti o le manovre vengono eseguiti bruscamente con difficoltà di coordinazione; le capacità di
vigilanza, le percezioni e l'elaborazione mentale risultano rallentate;
● 0,5 g/L(LIMITE LEGALE DI GUIDA): riduzione del 30-40% della capacità di percezione degli stimoli sonori,
uditivi e luminosi e della conseguente capacità di reazione; inoltre, il campo visivo si riduce;
● 0,6 g/L: la facoltà visiva laterale è fortemente compromessa di conseguenza, i movimenti e gli ostacoli vengono
percepiti con notevole ritardo;
● 0,7 g/L: l'esecuzione dei normali movimenti attuati alla guida è priva di coordinamento;
● 0,9 g/L: l'adattamento all'oscurità è compromesso, unitamente alla riduzione delle capacità di valutazione delle
distanze, degli ingombri, delle traiettorie dei veicoli e delle percezioni visive simultanee;
● 1 g/L (stato di ebbrezza): conferisce euforia, capacità visiva minima e disturbi motori che rendono precario
l'equilibrio; inoltre, le capacità di attenzione sono alterate, i riflessi inadeguati e sopiti, i tempi di reazione
sono assolutamente inadeguati e pericolosi per se stessi ed eventuali terzi.
