The Efficacy of Mobile ESWL Service (One year review)
Abstract Objective. Extracorporeal shockwave lithotripsy (ESWL) is the management of choice for ureteric and renal stones 20 mm or smaller, with a stone clearance rate of up to 89%. This study determined whether such a high success rate could apply to centres using mobile ESWL, by reviewing the performance at one centre that provides such a service. Material and methods. Between July 2011 and July 2012, 222 patients (median age 51 years, range 18-90 years) underwent one to five sessions of ESWL for ureteric and renal stones (mean size 15 mm, range 4-22 mm). Stone clearance was regarded as residual fragments 2 mm or smaller after completion of sessions. Results. In total, 110 out of 222 patients (49%) were clear of stones. Stones were radiopaque in 198 (89%) and radiolucent in 24 patients (11%), with clearance rates of 48% and 63%, respectively. Regarding size, 36 (16%) were 1-5 mm, 144 (65%) 5-10 mm, 28 (12%) 10-15 mm, eight (4%) 15-20 mm and six (3%) larger than 20 mm, with clearance rates of 61%, 55%, 18%, 13% and 50%, respectively. In total, 173 (78%) were renal stones and 49 (22%) ureteric, with respective clearance rates of 49% and 51%. For kidney stones, 15 (9%) were in the upper, 32 (18%) in the mid, 75 (43%) in the lower pole and 51 (30%) in the pelvis, with clearance rates of 52%, 59%, 49% and 41%; for ureteric stones, 32 (65%) were in the upper, 10 (20%) in the mid and seven (15%) in the lower ureter, with clearance rates of 47%, 70% and 43%, respectively. Conclusion. The performance of mobile ESWL was significantly poorer than expected, and this may be related to a lack of clinical ownership. The authors believe that such a service should be permanently placed on site.

I had been raised up as a proud black girl. My mother and Father made sure that would help me reaffirm my sense of identity through photos of my great great- grandmother a full Indian. I could see myself through her. Thus, I was a very proud black girl and would often identify myself as African as opposed to African American. However, it was a while before I accepted who I was. Growing up as a black girl in a predominantly white neighborhood and school was not easy for me. Many of the Caucasian children of my age in the neighborhood approached me with general courtesy such as why is your nose so big? Why do you have big lips? In the beginning as a child I felt that I was like everyone else until others started pointing out my ethnic features. As I got older is started noticing that I was different. Expressed by both my parents that I was beautiful, it was not enough for validation. It did not help that both my dad and step mom had been very light skinned and could almost pass as Puerto Rican or mixed. The type of light skin that could burn and not get dark. They did not have to be the only black child on the volleyball team in middle school, or the only black girl in Girl Scouts. I started to gain a better picture of how they perceived black people in general. I often wished that I could go to a school that had more black people who were sharing my experience. Perhaps if there were more of us then we could confide in each other and show these other girls that there is absolutely nothing wrong with or awkward about being black (Sen, 2013).
Even in the black community and family no ones wants to be known as the dark one. Our culture brainwashed us to believe not to even marry another dark individual if yourself was dark. “Please don’t bring home a black ashy baby!”. Hearing that all the time, I am wondering but what about love? As a result of these thoughts, feelings I started feeling like I was an incomplete black girl. I kept all these issues to myself until I was an adult with two kids entering the military. There was a different type of discrimination. Gender discrimination.
In a dominate male organization it is very hard to achieve success in the right way. I have never been discriminated as a women and women of color until I entered the Army. TALK ABOUT THE FEMALES BEING IN THE ARMY
As an adult I now recognize that there were elements of white supremacy in the undertone of the comments being made by these girls. They thought that white girls were beautiful and naturally perfect. In the same breadth they thought much less of black girls. In effect they associated all the prominent features of black girls with ugliness such as the kinky hair. This clearly points to white supremacy. They believed in the superiority of features associated with white girls without any factual backing whatsoever (McIntosh, 2000).

Identity Development in Adolescence
"Why are all the Black kids sitting together
in the cafeteria?"
Walk into any racially mixed high school cafeteria at lunch time and
you will instantly notice that in the sea of adolescent faces, there is an
identifiable group of Black students sitting together. Conversely, it
could be pointed out that there are many groups ofWhite students
sitting together as well, though people rarely comment about that.
The question on the tip of everyone's tongue is "Why are the Black
kids sitting together?" Principals want to know, teachers want to
know,White students want to know, the Black students who aren't sit-
ting at the table want to know.
How does it happen that so many Black teenagers end up at the
same cafeteria table? They don't start out there. If you walk into
racially mixed elementary schools, you will often see young children
of diverse racial backgrounds playing with one another, sitting at the
snack table together, crossing racial boundaries with an ease uncom-
mon in adolescence. Moving from elementary school to middle
school (often at sixth or seventh grade) means interacting with new
children from different neighborhoods than before, and a certain
degree of clustering by race might therefore be expected, presuming
that children who are familiar with one another would form groups.
But even in schools where the same children stay together from
kindergarten through eighth grade, racial grouping begins by the
sixth or seventh grade. What happens?
One thing that happens is puberty. As children enter adolescence,
they begin to explore the question of .

“Start Seeing Diversity: Race/Ethnicity”
Program Transcript
NARRATOR: Bias based on race, or racism, and ethnic bias, or ethnocentrism,
are any attitude, action, or institutional practice that subordinates people based
on the color of their skin or on their ethnic background. Racism and
ethnocentrism and are deeply interwoven. We are all affected by the racial bias
in our society, whether our classrooms or communities are mostly white, racially
mixed, or mostly people of color.
One teacher did not believe preschoolers in her all white group were affected by
environmental bias, such as constant TV news stories linking crime to men of
color. Then she showed each child a series of photographs of diverse people and
asked the children to tell her about the people in the pictures. In response to this
photograph, one child said, he's a robber, he has a brown face like a robber. The
teacher asked, why do you think people with brown faces are robbers? He said
he had seen them on TV.
This story shows the bias the children are developing whether or not they come
in contact with people who are different from them. A color blind approach that
does not acknowledge skin color ignores this, and also teaches children that
something is wrong with the differences they do see. Children do notice skin
color-- we all do. And young children are specifically being taught to notice and

In the video, the group leader instructed those with brown eyes to be cruel to those with blue eyes and was the main one being cruel to those with a different eye color. She was attempted to show how people who are not white are treated in the modern civilization, and while I do not agree with the extent she showed in terms of cruelty it is foolish to not believe racism does not exist and is not present. In the video, the instructor made one girl cry and asked if she was yeling or if it was simply her tone, as she attempted to make the girl feel stupid and inadequate which she excelled at. I do believe that there is racism still present however not to the extent portrayed, people are generally although not entirely nice to others. In the modern generation there is such an intermingling of races we have become desensitized and uncaring about ethnicity, however people still stick to preconceived racial stereotypes although they are incorrect and wrong. People are not generally rude or hurtful because of your skin tone, from my experience. As a child growing up in an almost all African-American neighborhood I was told I could not play with my friends due ti the color of my skin. White girls and black girls could not play together, and I still do not understand why. Two four year old children did not see race, or color, we saw best friends and playmates however adults perpetuating racism began to force feed hatred to their
- 2. children until they saw racial stereotypes and cruelty. This experiment was beneficial in showing people the other half of the spectrum however was a slap in the face to those who are not racist, or mean to others because of the color of their skin.
