DELINQUENCY LETTER
Dear (Patient),
Thank you for your patronage at XYZ Family Dental! We appreciate you choosing our practice
for your dental health needs.
In order to lower our patients’ cost of extended payments, we have partnered with CareCap, a
payment management company. CareCap is now managing all of our Accounts Receivable
(A/R). They are a reliable, customer service oriented company. With their economies-of-scale,
they are able to offer our XYZ Family Dental patients extended payment plans for only a 9%
administrative fee. This is much lower than the rate we need to charge to cover our expenses
for managing A/R.
Payment to CareCap is easy and automatic. You may choose either your checking account or
major credit card to use for payment. To get started, please contact our financial coordinator,
Susie to assist you in enrolling in CareCap. All payments are then automatically transferred
monthly from your chosen credit/ debit card or bank account to CareCap.
We appreciate your immediate attention. Please call our office, 123.456.7890, Monday
through Thursday, 8:30 to 4:30.

Please see the attached statement. Your balance is in excess of 90 days overdue. You are
welcome to pay this off completely with receipt in our office no later than November 7, 2013.
Or, if you prefer, you may transition this balance to a CareCap account for a short-term
payment plan. All transitions must be made by November 13, 2013. Unfortunately, all
accounts not transitioned by that date will be sent to Collections.
