This document is an application for Stanly County 4-H awards. It requests the applicant's name, date of birth, years in 4-H, and 4-H club. The applicant lists their project books and leadership, community service, and county/state 4-H activities for 2008. On the back, the applicant must write a statement about what 4-H means to them. A 4-H leader must also sign and can attach a statement nominating the applicant for an award. The form will be used to determine Outstanding 4-H'er Awards given at Achievement Night and is due by February 17, 2009.
