This document introduces NUTAS, a non-invasive ultrasound tumor ablation system developed through a joint venture between an Indian and Chinese company. NUTAS provides a breast-conserving alternative to surgery for treating breast cancer tumors. It uses high-intensity focused ultrasound to coagulatively necrosis tumor cells without damaging surrounding tissue. Over 138 patients in Delhi have been successfully treated with NUTAS, which can treat tumors across stages and offers benefits like no scarring or risk of infection compared to surgery.

23. I. Principle of NUTAS
• Under the guidance of Color Doppler Ultrasound images, NUTAS focuses
high intensity ultrasound beams generated in vitro on target lesions
inside body. High energy deposition appears in the focal area which
destroys tumor cell structure, and results in irreversible coagulative
necrosis of tumor tissues.

24. • According to the size, shape and boundary of tumor, NUTAS automatically generates treatment
scheme by using tumor tomography recognition and 3D reconstruction technologies. Then, by
controlling the movement of focused ultrasound, it cover the whole tumor, the complete tumor
ablation is accomplished.
• The temperature of the focal volume of an Ultrasound Beam is rapidly raised to cytotoxic levels
to selectively ablate a targeted tumor at depth cell-by-cell & line-by-line layers of cancerous
cells present in the tumor to ablate, without any damage to the surrounding or overlying
healthy tissues.
•
Coagulative necrosis is a type of accidental cell death typically caused by ischemia or infarction.
In coagulative necrosis the architecture of dead tissue is preserved for at least a couple of
days.[1]
It is believed that the injury denatures structural proteins as well as lysosomal enzymes
thus blocking the proteolysis of the damaged cells. The lack of lysosomal enzymes allows it to
maintain a "coagulated" morphology for some time. Like most types of necrosis if enough viable
cells are present around the affected area regeneration will usually occur.
• Coagulative necrosis can also be induced by high local temperature; it is a desired effect of
treatments such as high intensity focused ultrasound applied to cancerous cells.
25. Indications
• Breast Tumors: Benign / Malignant, Suggest choosing indications and
perform comprehensive treatment;
• Liver Tumors: Benign and malignant liver tumors. The tumor which is
blocked by rib can be treated by ablating rib or other method to improve
the ultrasound path;
• Bone Tumors: Original and metastatic tumors except skull and spine;
• Pancreatic Tumors: Non-icteric pancreatic tumors or pancreatic tumors
which have received jaundice reducing treatment;
• Kidney Tumors: Benign and malignant kidney tumors except those with
renal vein tumor emboli or inferior vena cava tumor emboli. The tumor
which is blocked by rib can be treated by ablating rib or other method to
improve the ultrasound path;
• Soft Tissue Tumors: Benign or malignant tumors in soft tissue;
• Uterine Fibroid;
• Benign Prostatic Hyperplasia and Prostatic Cancer;
• Retroperitoneal, abdominal and pelvic solid tumors with good
ultrasound path.
26. Contraindications
• Tumors in gassy and hollow viscera;
• Tumors in the central nervous system;
• Skin with ulcers or infections in the treated region;
• Skin in the treated region that has been treated by radiotherapy over
45Gy;
• Tumor thrombi in the vena cava system in the path of an ultrasound
wave;
• Apparent calcification on an arterial vessel wall in the path of an
ultrasound wave;
• Patients with major organ failure;
• Patients with severe coagulation disorders or definite bleeding
tendency;
• Patients with major organ failure or whose life cycle is estimated to be
within 3 months;
• Tumors are not clearly visible in the matched localization image system.

• It is the 1st

Abstract
High-intensity focused ultrasound (HIFU) is a noninvasive treatment that induces complete
coagulative necrosis of a tumour at depth through the intact skin. This study was to explore the
possibility of using HIFU for the treatment of patients with localised breast cancer in a controlled
clinical trial. A total of 48 women with biopsy-proven breast cancer (T1–2
, N0–2
, M0
) were randomised
to the control group in which modified radical mastectomy was performed, and the HIFU group in
which an extracorporeal HIFU ablation of breast cancer was followed by modified radical mastectomy.
Short-term follow-up, pathologic and immunohistochemical stains were performed to assess the
therapeutic effects on tumour and complications of HIFU. The results showed that no severe side
effect was observed in the HIFU-treated patients. Pathologic findings revealed that HIFU-treated
tumour cells underwent complete coagulative necrosis, and tumour vascular vessels were severely
damaged. Immunohistochemical staining showed that no expression of PCNA, MMP-9, and CD44v6
was detected within the treated tumour cells in the HIFU group, indicating that the treated tumour
cells lost the abilities of proliferation, invasion, and metastasis.
It is concluded that, as a noninvasive therapy, HIFU could be effective, safe, and feasible in the
extracorporeal treatment of localised breast cancer.

Dr. Alessandro Napoli, M.D., Ph.D., assistant professor of radiology at Sapienza
University in Rome, presented the results of his study involving 12 patients with
invasive ductal breast cancer. All patients were treated with FUS, and post-
treatment, histological evaluation confirmed the absence of residual disease in
the treatment area in 10 patients. Though this study is part of early stage research
Dr. Napoli is hopeful that focused ultrasound will one day be a treatment option
for women with Breast cancer
