1. Nook Permissions and Acceptable Use Form
Hill Country MS Library is providing a Nook for students to check out and read electronic
books. It has helpful educational features--unfamiliar words can be instantly defined using
the helpful dictionary and fonts can easily be enlarged. Because each Nook is valued at or
above $139, we require a parental signature before a Nook can be assigned to a student.
Parent Responsibilities and Permission
I am authorizing my student to check out a Nook from the library for a two-week check out
period. I understand that it is to be used as a tool for reading and that my child will comply
with the Eanes ISD Acceptable Use Policy. I will help ensure the timely return of the Nook; I
also understand that I am financially responsible for any damage to the Nook as well as any
charges resulting from content downloaded to the Nook without permission from the library
staff.

Student Responsibilities and Permission
I agree to take care of the Hill Country MS Library Nook while it is in my possession and will
use the Nook in the appropriate manner. I will not give the Nook to another student for his/her
use. I will only download any content to the Nook with written permission from the library
staff. I will not delete any book or change any settings. I will return the Nook, it’s AC & USB
adaptor, and its case in good condition.
