Service Quality and Patient-Centered Care L eaders of the U.S. Depart - ment of Health & Human Services (2011) urge providers to improve the overall quality of health care by making it more patient centered. Patient-centered care (or person-centered care) refers to the therapeutic relationship between health care providers and recipients of health care services, with emphasis on meeting the needs of individual patients. Al - though the term has been used widely in recent years, it remains a poorly defined and conceptualized phenomenon (Hobbs, 2009). Patient-centered care is believed to be holistic nursing care. It pro- vides a mechanism for nurses to engage patients as active partici- pants in every aspect of their health (Scott, 2010). Patient shadowing and care flow mapping were used to create a sense of empathy and urgency among clinicians by clarify- ing the patient and family experience. These two approaches, which were meant to promote patient-centered care, can improve patient satisfaction scores without increasing costs (DiGioia, Lorenz, Greenhouse, Bertoty, & Rocks, 2010). A better under standing of attributes of patient-centered care and areas for improvement is needed in order to develop nursing policies that in - crease the use of this model in health care settings. The purpose of this discussion is to clarify the concept of patient-centered care for consistency with the common understanding about pa - tient satisfaction and the quality of care delivered from nurses to patients. Attributes from a customer service model, the Gap Model of Service Quality, are used in a focus on the perspective of the patient as the driver and evaluator of service quality. Relevant literature and the Gap Model of Service Quality (Parasuraman, Zeithaml, & Leonard, 1985) are reviewed. Four gaps in patient-centered care are identified, with discussion of nursing implica- tions. Background and Brief Literature Review Patient-Centered Care The Institute of Medicine (IOM, 2001a) and Epstein and Street (2011) identified patient-centeredness as one of the areas for improvement in health care quality. Notes: Assignment 1: Dealing with Diversity in America from Reconstruction through the 1920s Due Week 3 and worth 120 points After the Civil War, the United States had to recover from war, handle western expansion, and grapple with very new economic forms. However, its greatest issues would revolve around the legacies of slavery and increasing diversity in the decades after the Civil War. In the South, former slaves now had freedom and new opportunities but, despite the Reconstruction period, faced old prejudices and rapidly forming new barriers. Immigrants from Europe and Asia came in large numbers but then faced political and social restrictions. Women continued to seek rights. Yet, on the whole, America became increasingly diverse by the 1920s. Consider developments, policies, and laws in that period from 1865 to the 1920s. Examine the statement below and drawing from provided sources, present a paper with specific examples and arguments to demonstrate the validity of your position. Statement—in which you can take a pro or con position: Political policies and movements in the period from 1865 to the 1920s generally promoted diversity and “the melting pot” despite the strong prejudices of a few. (or you can take the position that they did not). Use specific examples of policies or movements from different decades to support your position. After giving general consideration to your readings so far and any general research, select one of the positions above as your position—your thesis. NOTES TO CONSOLIDATED FINANCIAL STATEMENTS — (Continued) As of February 1, 2020 and February 2, 2019, there were no revolving credit loans outstanding under the credit agreements, and there were no borrowings under the agreements during 2019 and 2018. In addition, there were no standby letters of credit outstanding at February 1, 2020 and February 2, 2019. Revolving loans under the credit agreement bear interest based on various published rates. The Company's credit agreement, which is an obligation of a 100%-owned subsidiary of Macy’s, Inc. (“Parent”), is not secured. However, Parent has fully and unconditionally guaranteed this obligation. The credit agreement requires the Company to maintain a specified interest coverage ratio for the latest four quarters of no less than 3.25 and a specified leverage ratio as of and for the latest four quarters of no more than 3.75. The Company’s interest coverage ratio for 2019 was 12.68 and its leverage ratio at February 1, 2020 was 1.78, in each case as calculated in accordance with the credit agreement. The interest coverage ratio is defined as EBITDA divided by net interest expense and the leverage ratio is defined as debt divided by EBITDA. For purposes of these calculations EBITDA is calculated as net income plus interest expense, taxes, depreciation, amortization, non-cash impairment of goodwill, intangibles and real estate, settlement charges, non-recurring cash charges not to exceed in the aggregate $200 million and extraordinary losses less interest income and non-recurring or extraordinary gains. Net interest is adjusted to exclude the premium on early retirement of debt. A breach of a restrictive covenant in the Company’s credit agreement or the inability of the Company to maintain the financial ratios described above could result in an event of default under the credit agreement. In addition, an event of default would occur under the credit agreement if any indebtedness of the Company in excess of an aggregate principal amount of $150 million becomes due prior to its stated maturity or the holders of such indebtedness become able to cause it to become due prior to its stated maturity. Upon the occurrence of an event of default, the lenders could, subject to the terms and conditions of the credit agreement, elect to declare the outstanding principal, together with accrued interest, to be immediately due and payable. Moreover, most of the Company’s senior notes and debentures contain cross-default provisions based on the non-payment at maturity, or other default after an applicable grace period, of any other debt, the unpaid principal amount of which is not less than $100 million that could be triggered by an event of default under the credit agreement. In such an event, the Company’s senior notes and debentures that contain cross-default provisions would also be subject to acceleration.
