This document surveys safety-related incidents on Russian nuclear submarines from 1958 to 2007. It finds that over this period there were over 160 recorded incidents, with the frequency peaking in the 1960s and declining since. The majority of incidents involved first-generation submarines and were of low safety significance, with under 30 involving the release of radioactivity. Statistical analysis shows the mean time between safety-related events on Russian nuclear submarines has generally increased over time as experience and submarine design has improved.
