Solution In early 1999, The Joint Commission solicited input from a wide variety of stakeholders (e.g., clinical professionals, health care provider organizations, state hospital associations, health care consumers) and convened a Cardiovascular Conditions Clinical Advisory Panel about the potential focus areas for core measures for hospitals. In May 2001, the Joint Commission announced four initial core measurement areas for hospitals, which included acute myocardial infarction (AMI) and heart failure (HF). Simultaneously, The Joint Commission worked with the Centers for Medicare & Medicaid Services (CMS) on the AMI, and HF sets that were common to both organizations. CMS and the Joint Commission worked to align the measure specifications for use in the 7th Scope of Work and for Joint Commission accredited hospitals. Hospitals began collecting AMI measures for patient discharges beginning July 1, 2002. In November of 2003, CMS and The Joint Commission began to work to precisely and completely align these common measures so that they are identical. This resulted in the creation of one common set of measure specifications documentation known as the Specifications Manual for National Hospital Inpatient Quality Measures to be used by both organizations. The Manual contains common (i.e., identical) data dictionary, measure information forms, algorithms, etc. The goal is to minimize data collection efforts for these common measures and focus efforts on the use of data to improve the health care delivery process .

Crossing the Quality Chasm: The IOM Health Care Quality Initiative
In 1996, after releasing America's Health in Transition: Protecting and Improving Quality, the IOM launched a concerted, ongoing effort focused on assessing and improving the nation's quality of care.
The first phase of this Quality Initiative documented the serious and pervasive nature of the nation's overall quality problem, concluding that "the burden of harm conveyed by the collective impact of all of our health care quality problems is staggering" (Chassen et al., 1998).
IOM Definition of Quality
The degree to which health services for individuals and populations increase the likelihood of desired health outcomes and are consistent with current professional knowledge.
This phase built on an intensive review of the literature conducted by RAND to understand the scope of this issue (Schuster) and a framework was established that defined the nature of the problem as one of overuse, misuse and underuse of health care services (Chassen et al). More specifically, the report Ensuring Quality Cancer Care (1999) documented the wide gulf that exists between ideal cancer care and the reality many Americans with cancer experience.
During the second phase, spanning 1999-2001, the Committee on Quality of Health Care in America, laid out a vision for how the health care system and related policy environment must be radically transformed in order to close the chasm between what we know to be good quality care and what actually exists in practice. The reports released during this phase—To Err is Human: Building a Safer Health System(1999) and Crossing the Quality Chasm: A New Health System for the 21st Century(2001)—stress that reform around the margins is inadequate to address system ills.
The series of IOM quality reports have included a number of metrics that illustrate how wide the quality chasm is and how important it is to close this gulf, between what we know is good quality care and what the norm is in practice.
To Err is Human put the spotlight on how tens of thousands of Americans die each year from medical errors and effectively put the issue of patient safety and quality on the radar screen of public and private policymakers. The Quality Chasm report described broader quality issues and defines six aims—care should be safe, effective, patient-centered, timely, efficient and equitable—and 10 rules for care delivery redesign.

Abstract
Developing data-driven local solutions to address rising health care costs requires valid and reliable local data.
Traditionally, local public health agencies have relied on birth, death, and specific disease registry data to guide
health care planning, but these data sets provide neither health information across the lifespan nor information
on local health care utilization patterns and costs. Insurance claims data collected by local hospitals for ad-
ministrative purposes can be used to create valuable population health data sets. The Camden Coalition of
Healthcare Providers partnered with the 3 health systems providing emergency and inpatient care within
Camden, New Jersey, to create a local population all-payer hospital claims data set. The combined claims data
provide unique insights into the health status, health care utilization patterns, and hospital costs on the pop-
ulation level. The cross-systems data set allows for a better understanding of the impact of high utilizers on a
community-level health care system. This article presents an introduction to the methods used to develop
Camden’s hospital claims data set, as well as results showing the population health insights obtained from this

Surveillance is an integral part of public healthpractice. The ability to obtain accurate, timely information
about the patterns of disease incidence and mortality has
traditionally been a central goal of public health. With the
growing concerns over the inefficient uses of health care ser-
vices and rising health care costs, public health must add cost
and utilization to its surveillance practice. A key component
of any surveillance system is that data are gathered at the
geographic level at which the interventions will be delivered.
For example, interventions that aim to reduce preterm deliv-
ery must be delivered to those at risk for preterm delivery,
creating a need for preterm delivery surveillance data at the
city or neighborhood level in order to understand the size,
scale, and geographic concentration of the problem.
Birth outcome surveillance data, such as prevalence of
preterm births, have been available through birth certificate
registry data sets. These data sets are compiled from birth
certificate forms that states require hospitals to complete and
send to state health departments. State health departments
process the data, creating local population data sets for city
and county health departments. Death certificate data are

Solution
. The name of the function changed from medical records management to health information
management as enhancements in technology expanded responsibilities from managing paper
records to managing the full scope of the process of collecting and sharing electronically-
captured information among disparate entities.
The history of health information management begins with the simplest form of recording a
patient’s symptoms, complaints and treatment for the use of one provider, to a comprehensive
aggregation, integration and harmonization of data to support collaboration among providers,
researchers and administrators.
The First Medical Records
The earliest forms of medical records were narratives written by ancient Greeks to document
successful cures, share observations about symptoms and outcomes, and teach others who
provided medical advice through these case studies. While written reports describing patients’
complaints and diagnoses predate the records of Simon Forman and Richard Napier – astrologers
who documented clients’ medical questions and treatment – their records from 1596 to 1634
form the earliest complete collection of medical records in existence.
The 1920s
As healthcare advanced, physicians realized that the best way to continue improving diagnosing
and treating illnesses was to carefully document observations and actions while treating patients
– and share this information as a way to teach other health professionals.
As early as 1600, physicians offered advice on how to present information in a medical record,
but it wasn’t until 1928 that the American College of Surgeons (ACOS) took steps to standardize
the growing number of medical records by establishing the American Association of Record
Librarians (AARL) – known today as the American Health Information Management
Association (AHIMA). “Record librarians” was the term used because early medical records
were documented on paper.
Standardization of medical records and growth of complete record-keeping continued from the
1920s through the 1960s, but records were paper-based.
The 1960s
The development of computers presented the opportunity to maintain records electronically, but
the expense of purchasing and maintaining a mainframe, and the expense associated with storage
of data, meant that only the largest organizations could use technology to handle medical
records.
The field of health informatics, as it is known today, emerged when computer technology
became sophisticated enough to manage large amounts of data. One of the earliest efforts took
place under the jurisdiction of the American Society for Testing and Materials (ASTM). These

The Alzheimer's Disease Research Network and the Uniform Data Set
Alzheimer's disease (AD) is recognized as a public health crisis worldwide (IADRP, 2013). AD is a complex neurodegenerative disease and the leading cause of dementia among the elderly people (Evans et al., 1989). Currently, there are approximately five million AD cases in the United States and about 35 million cases worldwide (Alzheimer's Disease International, 2009.
The focus of this case study is on the Uniform Data Set (UDS), a longitudinal database on Alzheimer’s patients, and the 29 Alzheimer’s Disease Centers (ADC) that submit their data to the UDS and actively collaborate in the ongoing maintenance, development and research utilization of the database. The ADCs are based in major medical institutions across the United States. They have a multi-decade track record of collaborative research and a networked and virtual approach to the scientific study of AD. The central coordinating mechanism for the ADCs and the UDS is the National Alzheimer’s Coordinating Center (NACC), which is located at the University of Washington. The NACC coordinates data collection and supports collaborative research among the ADCs.

1. WHY A NEW HEALTH STRATEGY?
Health is central in people's lives and needs to be supported by effective policies and actions
in Member States, at EC1 level and at global level.
Member States have the main responsibility for health policy and provision of healthcare to
European citizens. The EC's role is not to mirror or duplicate their work. However, there are
areas where Member States cannot act alone effectively and where cooperative action at
Community level is indispensable. These include major health threats and issues with a cross-
border or international impact, such as pandemics and bioterrorism, as well as those relating
to free movement of goods, services and people.
To carry out this role, cross-sectoral work is required. Article 152 of the EC Treaty says that a
"high level of human health protection shall be ensured in the definition and implementation
of all Community policies and activities". This Strategy reinforces the importance of health in
policies such as the Lisbon Strategy for Growth and Jobs, emphasising the links between
health and economic prosperity, and the Citizens' Agenda, recognising people's right to be
empowered in relation to their health and healthcare. Actions in the Strategy represent work
on health across all sectors. Health is found in Treaty articles on the Internal Market,
Environment, Consumer Protection, Social Affairs including the Safety and Health of
Workers, Development Policy, and Research, amongst many others2.
The EC's important role in health policy has been reaffirmed in the Reform Treaty which was
agreed by EU Heads of State and Government in Lisbon on 19 October 2007, and which
proposes to reinforce the political importance of health. A new overall aim on supporting
citizens' wellbeing is expected, as well as an encouragement of cooperation amongst Member
States on health and health services. Work on health at Community level adds value to
Member States' actions, particularly in the area of prevention of illness, including work on
food safety and nutrition, the safety of medical products, tackling smoking, legislation on
blood, tissues and cells, and organs, water and air quality, and the launch of a number of
health-related agencies. However, there are several growing challenges to the health of the
population which require a new strategic approach.

The New Focus on Quality and Outcomes
Introduction
In 1999, the Institute of Medicine (IOM) published a groundbreaking analysis of the impact of medical errors on the health care delivery system and the patients it serves. The analysis, published as "To Err is Human: Building a Safer Healthcare System," concluded that medical errors resulted in up to 98,000 patient deaths in American hospitals every year. This report hit the national press and participants in the health care system and the political system with the force of a large bomb. Since that time, hospitals and other health care entities have refocused their attention on quality, errors, and patient safety in an unprecedented way, urged on by public outcry and by federal and state efforts to compel improvements in the health care system. Such entities as the Institute for Healthcare Improvement (www.ihi.org) the National Quality Forum (www.qualityforum.org), and the Institute of Medicine (www.iom.edu) have all emerged as champions of quality and safety initiatives, offering training, resources, access to best practices, and data collection strategies to move the cause of quality and safety for patients forward.
History
The IOM report had a huge impact on the discussion of quality and safety in the health care field. Aspects of quality care have always been present in hospitals, typically focused around the quality assurance or quality management departments. They historically collected data on department indicators and monitored them as part of accreditation. However, departmental data was typically focused on operational performance in the departments in question, and not a great deal was collected on issues of medical errors and near-misses. The litigious legal climate caused most hospitals to fear collecting and sharing data that could potentially be used against them in a legal action. However, the IOM report caused a national demand to know what health care institutions were doing to protect their patients from injury caused by errors. A climate of increased transparency has begun to emerge, although it is still a very long way from the concept of full openness on standardized reporting of indicators. The Centers for Medicare and Medicaid Services (CMS) weighed in with publication of their never-events, as explored further below.
Finally there has been an increased push for public reporting of data on individual hospital performance on selected indicators. While some progress has been made, there is a large range of indicators that is not yet publically reported, and medical errors are not publically reported at all at this point, although those with great potential to cause harm must be reported to their relevant state licensing agency.

thrilled when President George
W. Bush said in his 2004 State of
the Union address “…an
Electronic Health Record for
every American by the year
2014…by computerizing health
records, we can avoid dangerous
medical mistakes, reduce costs,
and improve care” (Bush, 2004). This was the first
time a president formally recognized the value of HIT
and set a deadline to do something about it! President
Bush went on to establish the Office of the National
Coordinator for HIT (ONC), and Dr. David Brailer was
appointed as the first coordinator by Tommy
Thompson, then Secretary of the Department of
Health and Human Services (HHS).
The support continued. In 2005, funding from
HHS was earmarked to establish organizations for
standards harmonization (HIT Standards Panel) and
for certification of electronic health record (EHR) sys-
tems (Certification Commission for HIT). In 2006, the
Agency for Healthcare Research and Quality (AHRQ)
launched its National Resource Center for HIT.
Government attention persisted in 2007 with the
funding of National Health Information Network pro-
totypes. Momentum was building and there was
much attention on HIT from the federal government.
Fast forward to 2009. President-Elect Barack
Obama says he wants the federal government to
invest in EHRs so all medical records are digitized
within 5 years and vows to continue to push for the
2014 deadline established by Bush. “This will cut
waste, eliminate red tape, and reduce the need to
repeat expensive medical tests,” he said, adding that

sets the stage for today’s focus on the use of HIT, and
the proliferation of EHR implementation projects in

Legislative Background
On March 23, 2010, President Obama signed into

Abstract
Developing data-driven local solutions to address rising health care costs requires valid and reliable local data.
Traditionally, local public health agencies have relied on birth, death, and specific disease registry data to guide
health care planning, but these data sets provide neither health information across the lifespan nor information
on local health care utilization patterns and costs. Insurance claims data collected by local hospitals for ad-
ministrative purposes can be used to create valuable population health data sets. The Camden Coalition of
Healthcare Providers partnered with the 3 health systems providing emergency and inpatient care within
Camden, New Jersey, to create a local population all-payer hospital claims data set. The combined claims data
provide unique insights into the health status, health care utilization patterns, and hospital costs on the pop-
ulation level. The cross-systems data set allows for a better understanding of the impact of high utilizers on a
community-level health care system. This article presents an introduction to the methods used to develop
Camden’s hospital claims data set, as well as results showing the population health insights obtained from this

Surveillance is an integral part of public healthpractice. The ability to obtain accurate, timely information
about the patterns of disease incidence and mortality has
traditionally been a central goal of public health. With the
growing concerns over the inefficient uses of health care ser-
vices and rising health care costs, public health must add cost
and utilization to its surveillance practice. A key component
of any surveillance system is that data are gathered at the
geographic level at which the interventions will be delivered.
For example, interventions that aim to reduce preterm deliv-
ery must be delivered to those at risk for preterm delivery,
creating a need for preterm delivery surveillance data at the
city or neighborhood level in order to understand the size,
scale, and geographic concentration of the problem.
Birth outcome surveillance data, such as prevalence of
preterm births, have been available through birth certificate
registry data sets. These data sets are compiled from birth
certificate forms that states require hospitals to complete and
send to state health departments. State health departments
process the data, creating local population data sets for city
and county health departments. Death certificate data are

. The name of the function changed from medical records management to health information
management as enhancements in technology expanded responsibilities from managing paper
records to managing the full scope of the process of collecting and sharing electronically-
captured information among disparate entities.
The history of health information management begins with the simplest form of recording a
patient’s symptoms, complaints and treatment for the use of one provider, to a comprehensive
aggregation, integration and harmonization of data to support collaboration among providers,
researchers and administrators.
The First Medical Records
The earliest forms of medical records were narratives written by ancient Greeks to document
successful cures, share observations about symptoms and outcomes, and teach others who
provided medical advice through these case studies. While written reports describing patients’
complaints and diagnoses predate the records of Simon Forman and Richard Napier – astrologers
who documented clients’ medical questions and treatment – their records from 1596 to 1634
form the earliest complete collection of medical records in existence.
The 1920s
As healthcare advanced, physicians realized that the best way to continue improving diagnosing
and treating illnesses was to carefully document observations and actions while treating patients
– and share this information as a way to teach other health professionals.
As early as 1600, physicians offered advice on how to present information in a medical record,
but it wasn’t until 1928 that the American College of Surgeons (ACOS) took steps to standardize
the growing number of medical records by establishing the American Association of Record
Librarians (AARL) – known today as the American Health Information Management
Association (AHIMA). “Record librarians” was the term used because early medical records
were documented on paper.
Standardization of medical records and growth of complete record-keeping continued from the
1920s through the 1960s, but records were paper-based.
The 1960s
The development of computers presented the opportunity to maintain records electronically, but
the expense of purchasing and maintaining a mainframe, and the expense associated with storage
of data, meant that only the largest organizations could use technology to handle medical
records.
The field of health informatics, as it is known today, emerged when computer technology
became sophisticated enough to manage large amounts of data. One of the earliest efforts took
place under the jurisdiction of the American Society for Testing and Materials (ASTM). These

The Alzheimer's Disease Research Network and the Uniform Data Set
Alzheimer's disease (AD) is recognized as a public health crisis worldwide (IADRP, 2013). AD is a complex neurodegenerative disease and the leading cause of dementia among the elderly people (Evans et al., 1989). Currently, there are approximately five million AD cases in the United States and about 35 million cases worldwide (Alzheimer's Disease International, 2009.
The focus of this case study is on the Uniform Data Set (UDS), a longitudinal database on Alzheimer’s patients, and the 29 Alzheimer’s Disease Centers (ADC) that submit their data to the UDS and actively collaborate in the ongoing maintenance, development and research utilization of the database. The ADCs are based in major medical institutions across the United States. They have a multi-decade track record of collaborative research and a networked and virtual approach to the scientific study of AD. The central coordinating mechanism for the ADCs and the UDS is the National Alzheimer’s Coordinating Center (NACC), which is located at the University of Washington. The NACC coordinates data collection and supports collaborative research among the ADCs.

1. WHY A NEW HEALTH STRATEGY?
Health is central in people's lives and needs to be supported by effective policies and actions
in Member States, at EC1 level and at global level.
Member States have the main responsibility for health policy and provision of healthcare to
European citizens. The EC's role is not to mirror or duplicate their work. However, there are
areas where Member States cannot act alone effectively and where cooperative action at
Community level is indispensable. These include major health threats and issues with a cross-
border or international impact, such as pandemics and bioterrorism, as well as those relating
to free movement of goods, services and people.
To carry out this role, cross-sectoral work is required. Article 152 of the EC Treaty says that a
"high level of human health protection shall be ensured in the definition and implementation
of all Community policies and activities". This Strategy reinforces the importance of health in
policies such as the Lisbon Strategy for Growth and Jobs, emphasising the links between
health and economic prosperity, and the Citizens' Agenda, recognising people's right to be
empowered in relation to their health and healthcare. Actions in the Strategy represent work
on health across all sectors. Health is found in Treaty articles on the Internal Market,
Environment, Consumer Protection, Social Affairs including the Safety and Health of
Workers, Development Policy, and Research, amongst many others2.
The EC's important role in health policy has been reaffirmed in the Reform Treaty which was
agreed by EU Heads of State and Government in Lisbon on 19 October 2007, and which
proposes to reinforce the political importance of health. A new overall aim on supporting
citizens' wellbeing is expected, as well as an encouragement of cooperation amongst Member
States on health and health services. Work on health at Community level adds value to
Member States' actions, particularly in the area of prevention of illness, including work on
food safety and nutrition, the safety of medical products, tackling smoking, legislation on
blood, tissues and cells, and organs, water and air quality, and the launch of a number of
health-related agencies. However, there are several growing challenges to the health of the
population which require a new strategic approach.

Introduction
In 1999, the Institute of Medicine (IOM) published a groundbreaking analysis of the impact of medical errors on the health care delivery system and the patients it serves. The analysis, published as "To Err is Human: Building a Safer Healthcare System," concluded that medical errors resulted in up to 98,000 patient deaths in American hospitals every year. This report hit the national press and participants in the health care system and the political system with the force of a large bomb. Since that time, hospitals and other health care entities have refocused their attention on quality, errors, and patient safety in an unprecedented way, urged on by public outcry and by federal and state efforts to compel improvements in the health care system. Such entities as the Institute for Healthcare Improvement (www.ihi.org) the National Quality Forum (www.qualityforum.org), and the Institute of Medicine (www.iom.edu) have all emerged as champions of quality and safety initiatives, offering training, resources, access to best practices, and data collection strategies to move the cause of quality and safety for patients forward.
History
The IOM report had a huge impact on the discussion of quality and safety in the health care field. Aspects of quality care have always been present in hospitals, typically focused around the quality assurance or quality management departments. They historically collected data on department indicators and monitored them as part of accreditation. However, departmental data was typically focused on operational performance in the departments in question, and not a great deal was collected on issues of medical errors and near-misses. The litigious legal climate caused most hospitals to fear collecting and sharing data that could potentially be used against them in a legal action. However, the IOM report caused a national demand to know what health care institutions were doing to protect their patients from injury caused by errors. A climate of increased transparency has begun to emerge, although it is still a very long way from the concept of full openness on standardized reporting of indicators. The Centers for Medicare and Medicaid Services (CMS) weighed in with publication of their never-events, as explored further below.
Finally there has been an increased push for public reporting of data on individual hospital performance on selected indicators. While some progress has been made, there is a large range of indicators that is not yet publically reported, and medical errors are not publically reported at all at this point, although those with great potential to cause harm must be reported to their relevant state licensing agency.

nology (HIT) trends were
thrilled when President George
W. Bush said in his 2004 State of
the Union address “…an
Electronic Health Record for
every American by the year
2014…by computerizing health
records, we can avoid dangerous
medical mistakes, reduce costs,
and improve care” (Bush, 2004). This was the first
time a president formally recognized the value of HIT
and set a deadline to do something about it! President
Bush went on to establish the Office of the National
Coordinator for HIT (ONC), and Dr. David Brailer was
appointed as the first coordinator by Tommy
Thompson, then Secretary of the Department of
Health and Human Services (HHS).
The support continued. In 2005, funding from
HHS was earmarked to establish organizations for
standards harmonization (HIT Standards Panel) and
for certification of electronic health record (EHR) sys-
tems (Certification Commission for HIT). In 2006, the
Agency for Healthcare Research and Quality (AHRQ)
launched its National Resource Center for HIT.
Government attention persisted in 2007 with the
funding of National Health Information Network pro-
totypes. Momentum was building and there was
much attention on HIT from the federal government.
Fast forward to 2009. President-Elect Barack
Obama says he wants the federal government to
invest in EHRs so all medical records are digitized
within 5 years and vows to continue to push for the
2014 deadline established by Bush. “This will cut
waste, eliminate red tape, and reduce the need to
repeat expensive medical tests,” he said, adding that

The US House of Representatives is deeply concerned by ongoing and pervasive acts of antisemitic
harassment and intimidation at the Massachusetts Institute of Technology (MIT). Failing to act decisively to ensure a safe learning environment for all students would be a grave dereliction of your responsibilities as President of MIT and Chair of the MIT Corporation.
This Congress will not stand idly by and allow an environment hostile to Jewish students to persist. The House believes that your institution is in violation of Title VI of the Civil Rights Act, and the inability or
unwillingness to rectify this violation through action requires accountability.
Postsecondary education is a unique opportunity for students to learn and have their ideas and beliefs challenged. However, universities receiving hundreds of millions of federal funds annually have denied
students that opportunity and have been hijacked to become venues for the promotion of terrorism, antisemitic harassment and intimidation, unlawful encampments, and in some cases, assaults and riots.
The House of Representatives will not countenance the use of federal funds to indoctrinate students into hateful, antisemitic, anti-American supporters of terrorism. Investigations into campus antisemitism by the Committee on Education and the Workforce and the Committee on Ways and Means have been expanded into a Congress-wide probe across all relevant jurisdictions to address this national crisis. The undersigned Committees will conduct oversight into the use of federal funds at MIT and its learning environment under authorities granted to each Committee.
• The Committee on Education and the Workforce has been investigating your institution since December 7, 2023. The Committee has broad jurisdiction over postsecondary education, including its compliance with Title VI of the Civil Rights Act, campus safety concerns over disruptions to the learning environment, and the awarding of federal student aid under the Higher Education Act.
• The Committee on Oversight and Accountability is investigating the sources of funding and other support flowing to groups espousing pro-Hamas propaganda and engaged in antisemitic harassment and intimidation of students. The Committee on Oversight and Accountability is the principal oversight committee of the US House of Representatives and has broad authority to investigate “any matter” at “any time” under House Rule X.
• The Committee on Ways and Means has been investigating several universities since November 15, 2023, when the Committee held a hearing entitled From Ivory Towers to Dark Corners: Investigating the Nexus Between Antisemitism, Tax-Exempt Universities, and Terror Financing. A Strategic Approach: GenAI in Education
Artificial Intelligence (AI) technologies such as Generative AI, Image Generators and Large Language Models have had a dramatic impact on teaching, learning and assessment over the past 18 months. The most immediate threat AI posed was to Academic Integrity with Higher Education Institutes (HEIs) focusing their efforts on combating the use of GenAI in assessment. Guidelines were developed for staff and students, policies put in place too. Innovative educators have forged paths in the use of Generative AI for teaching, learning and assessments leading to pockets of transformation springing up across HEIs, often with little or no top-down guidance, support or direction.
This Gasta posits a strategic approach to integrating AI into HEIs to prepare staff, students and the curriculum for an evolving world and workplace. We will highlight the advantages of working with these technologies beyond the realm of teaching, learning and assessment by considering prompt engineering skills, industry impact, curriculum changes, and the need for staff upskilling. In contrast, not engaging strategically with Generative AI poses risks, including falling behind peers, missed opportunities and failing to ensure our graduates remain employable. The rapid evolution of AI technologies necessitates a proactive and strategic approach if we are to remain relevant.

Solution In early 1999, The Joint Commission solicited input from a wide variety of stakeholders (e.g., clinical professionals, health care provider organizations, state hospital associations, health care consumers) and convened a Cardiovascular Conditions Clinical Advisory Panel about the potential focus areas for core measures for hospitals. In May 2001, the Joint Commission announced four initial core measurement areas for hospitals, which included acute myocardial infarction (AMI) and heart failure (HF). Simultaneously, The Joint Commission worked with the Centers for Medicare & Medicaid Services (CMS) on the AMI, and HF sets that were common to both organizations. CMS and the Joint Commission worked to align the measure specifications for use in the 7th Scope of Work and for Joint Commission accredited hospitals. Hospitals began collecting AMI measures for patient discharges beginning July 1, 2002. In November of 2003, CMS and The Joint Commission began to work to precisely and completely align these common measures so that they are identical. This resulted in the creation of one common set of measure specifications documentation known as the Specifications Manual for National Hospital Inpatient Quality Measures to be used by both organizations. The Manual contains common (i.e., identical) data dictionary, measure information forms, algorithms, etc. The goal is to minimize data collection efforts for these common measures and focus efforts on the use of data to improve the health care delivery process
