2/Assignment 2Emil Mowry is a 62 y.o., single, Latino rancher who did not complete high
school. He remembers rolling his first cigarette when he was 11 years old. He does not
remember ever wanting to quit smoking. For the last five years he has had increasing
difficulty breathing. Emil was diagnosed by his Family Nurse Practitioner as having
emphysema. He was told that he has Chronic Obstructive Pulmonary Disease (COPD). He
did not ask his Nurse Practitioner what this means, but he plans to look it up on the
Internet.The purpose of this assignment is to create an evidence-based patient-centered
educational brochure on the pathophysiology and clinical manifestations of, and common
treatment options for, COPD that could be used by Nurse Practitioners or Nurse Educators
with their patients newly-diagnosed with this disorder. The proposed brochure should
include graphics and citations from your textbook and the external, scholarly-practice
literature.You may present the content in the electronic format of your choice: single sheet,
folded, tri-fold. In other words, you may use MS Word Document, MS Publisher, or any other
software to develop your educational brochure. Since your audience are patients, please
keep in mind the literacy and numeracy level (some literature suggests no more than
8th grade level literacy and numeracy). Also, please refer to the Assignment Rubric below
for grading criteria and to help you complete this assignment.In regards to APA format,

Thomas, a 25 y.o. African-American previously well woman, presents to her primary care
Nurse Practitioner’s office with complaints of episodic shortness of breath and chest
tightness. She has had the symptoms on and off for about 2 years but states that they have
worsened lately, occurring two or three times a month. She notes that the symptoms are
worse during the spring months. She has no exercise-induced or nocturnal symptoms. The
family history is notable for a father with asthma. She is single and works as a secretary in a
high-tech firm. She lives with a roommate, who moved in approximately 2 months ago. The
roommate has a cat. The patient smokes occasionally when out with friends, drinks socially,
and has no history of drug use. Examination is notable for mild end-expiratory wheezing.
Pulmonary function tests are ordered to confirm the diagnosis. N512 Respiratory Disease

likely diagnosis is. Why do you this “likely” diagnosis?Identify the pathogenetic
mechanisms that are responsible for this patient’s symptoms of wheezing, shortness of

Criteria.Sample Discussion ApproachDarla Thomas has a likely diagnosis of allergic asthma.
She was a previously well woman, but for the past two years, she has had intermittent
episodes of chest tightness and shortness of breath. In evaluating her condition, the issue of
her symptoms lasting for over six months makes her condition chronic. The recent changes
in her life may attribute to her worsening condition. Allergic asthma produces symptoms
due to cellular reactions by the lungs. Airway mucosa is thickened edematous and
infiltrated with inflammatory cells, principally lymphocytes, eosinophils, and mast cells
(Hamner 2016). Darla has symptom inducing environmental factors based on her account
of her symptoms worsening with the introduction of a cat, and seasonal discomfort during
springtime. Darla’s father has asthma, which is a genetic risk factor that contributes to her
asthma diagnosis. Asthma is a restriction of airflow, making air hard to escape the lungs; a
pulmonary function test will produce a definitive diagnosis. However, if Darla is not having
symptoms, her pulmonary function test may not indicate disease. I suspect her
pulmonary function test will be healthy because there are no triggers when taking the test.
The plan of care for Darla will be to reduce her triggers, quitting smoking, and getting her an
inhaler to use as needed for symptoms. The most important part of her treatment is to get
her a bronchodilator; this can be life-saving if she has an episode where she can not breathe.
Secondly will be to rid Darla of the cat and take allergy medication to reduce the issues that
trigger her asthma. Third, for Darla, quitting smoking, although it is occasionally, the
importance of stopping is imperative to her lung health and the future of her overall
health. Darla is a young woman working at a tech company. Cell phone reminders to
take her allergy pills and carry her inhaler may be useful. There are apps to assist in smoke
cessation, which are in line with Darla’s tech experience. Darla will need to schedule follow
up appointments to monitor her progress. Improvement will be evident in her tracking
asthma symptoms and a decreasing need to use her

very important intervention for any patients with asthma. Smoke from tobacco, even
through second-hand smoke, is a significant trigger for asthma exacerbations (Perret et al.,
2016). I found it interesting that Perret et al. (2016) indicated that the prevalence of
smoking in people diagnosed with is similar to or even higher than the general population.
They indicated that patients with asthma were less likely to stop smoking compared to the
general public. Additionally, adolescents with asthma were more likely to smoke than those
who don’t have the condition in the US (Perret et al., 2016). Smoking cessation is very
important for populations with asthma as their quality of life is severely declined.
Continuous smokers are more likely to die a decade earlier than than nonsmokers (Perret et
al., 2016). There is a 90% reduction in risk of death if one quits smoking before the age of
40 (Perret et al., 2016). Thus, smoking behaviors should be analyzed in patients with
asthma and smoking cessation should be included in treatment counseling for any patient

Reynaldo,Smoking plagued many people in the United States and around the world.
Convincing people to change addictive behavior is complicated, especially when it is legal
and not immediately affected physically. Available research suggests that physicians’
healthy practices influence patients’ attitudes towards preventive counseling and their
motivation to make healthy lifestyle choices(Tovalacci 2018). Behavior modification is
necessary to empower patients to take control of their heath-care. Life comes with
challenges people need tools to meet those challenges with encouragement and easy to
follow tasks. Being a nurse practitioner is a big responsibility; I will use evidence-based
practice to improve my patient’s health. I know that due to human nature, there will be wins
and losses, but the need to be an advocate for my patients will keep me encouraged to do

She lives with a roommate, who moved in approximately 2 months ago. The roommate has a cat. The patient smokes occasionally when out with friends, drinks socially, and has no history of drug use. She was a previously well woman, but for the past two years, she has had intermittent episodes of chest tightness and shortness of breath. In evaluating her condition, the issue of her symptoms lasting for over six months makes her condition chronic. The recent changes in her life may attribute to her worsening condition. Allergic asthma produces symptoms due to cellular reactions by the lungs. Airway mucosa is thickened edematous and infiltrated with inflammatory cells, principally lymphocytes, eosinophils, and mast cells (Hamner 2016). Darla has symptom inducing environmental factors based on her account of her symptoms worsening with the introduction of a cat, and seasonal discomfort during springtime. Darla’s father has asthma, which is a genetic risk factor that contributes to her asthma diagnosis. Asthma is a restriction of airflow, making air hard to escape the lungs; a pulmonary function test will produce a definitive diagnosis. However, if Darla is not having symptoms, her pulmonary function test may not indicate disease. I suspect her pulmonary function test will be healthy because there are no triggers when taking the test. The plan of care for Darla will be to reduce her triggers, quitting smoking, and getting her an inhaler to use as needed for symptoms. The most important part of her treatment is to get her a bronchodilator; this can be life-saving if she has an episode where she can not breathe. Secondly will be to rid Darla of the cat and take allergy medication to reduce the issues that trigger her asthma. Third, for Darla, quitting smoking, although it is occasionally, the importance of stopping is imperative to her lung health and the future of her overall health. Darla is a young woman working at a tech company. Cell phone reminders to take her allergy pills and carry her inhaler may be useful. There are apps to assist in smoke cessation, which are in line with Darla’s tech experience. Darla will need to schedule follow up appointments to monitor her progress. Smoke from tobacco, even through second-hand smoke, is a significant trigger for asthma exacerbations (Perret et al., 2016). I found it interesting that Perret et al. (2016) indicated that the prevalence of smoking in people diagnosed with is similar to or even higher than the general population. They indicated that patients with asthma were less likely to stop smoking compared to the general public. Additionally, adolescents with asthma were more likely to smoke than those
  • 4. who don’t have the condition in the US (Perret et al., 2016). Smoking cessation is very important for populations with asthma as their quality of life is severely declined. Continuous smokers are more likely to die a decade earlier than than nonsmokers (Perret et al., 2016). There is a 90% reduction in risk of death if one quits smoking before the age of 40 (Perret et al., 2016).
