A 16-year-old male presented with pain and instability in his right knee for two months with a history of recurrent patellar subluxation in both knees. Examination revealed positive anterior drawer, Lanchman, patellar apprehension, and quadrant tests. Imaging including radiographs, CT scan, and MRI showed an interstitial ACL tear, trochlear dysplasia, high riding patella, and increased tibial tuberosity-trochlear groove distance. The patient subsequently underwent MPFL reconstruction surgery.
