License - Should not be empty

City - Should not be empty
State - Should not be empty
ZIP Code - Should not be empty, Should be of 5 digit numeric value
Mobile Plan Details
Select Mobile Model - Should not be empty
Select Mobile Plan - Should not be empty
Select Mobile Feature - Should not be empty
Contractual Agreement - Should check the box
Credit Card Details
Credit Card Number - Should not be empty, Should be of 16 digit numeric value
CVC - Should not be empty, Should be of 3 digit numeric value

2) After new SIM is Activate, You can register to H2K Mobile portal by clicking on “Sign Up”
