This location release form is between Eclipse Studios members Suela Elezi, Laura Hodgson, and Hannah Mcdonagh and the property owner. It grants the filmmakers permission to use the property located at 33 Wannock Gardens for their horror film between specified dates and times. The filmmakers will be responsible for crew, equipment, temporary sets, and restoring the property. The property owner gives the filmmakers exclusive rights to footage and waives right to inspect recordings. The filmmakers agree to indemnify the owner for any losses from the project.

1. Location release form
This location release form has been made by Suela Elezi, Laura Hodgson and
Hannah Mcdonagh, who are the members of the Eclipse Studios. We will would like
you to grant us permission to use your site for our horror film which is for educational
reasons. We would like to use Fairlop Waters as a site that will need to use as a
setting for our horror film. For the amount of £___________GBP, the sufficiency of
which you acknowledge and agree to the statements and conditions stated in this
contract concerning to use your site.
Use of property:
We would like to use the public park which is located in Forest Rd, Ilford, from the
day of _______________ to the day ________________ during the hours of
_____________ to _______________. We will need to use the site for parking,
venue area and the public park as we will need to use these locations for our horror
film. Suela Elezi, Laura Hodgson and Hannah Mcdonagh, we will need to use this
site for photography, videotaping, and for making sound recordings. If on the day of
the recording of the horror film there is a delay, Suela Elezi, Laura Hodgson and
Hannah Mcdonagh, holds the right to reschedule to record on another day because
of any issues that may be brought up.
Rights to footage:
You have given us the right to record on the site and exclusive rights to Suela Elezi,
Laura Hodgson and Hannah Mcdonagh . You have also agreed not to inspect or
approve any of the recording we will make.
Crew and equipment:
Suela Elezi, Laura Hodgson and Hannah Mcdonagh, will need to bring in our crew,
actors and equipment. Suela Elezi, Laura Hodgson and Hannah Mcdonagh may
construct temporary sets and, after completing the finial photography and recording
for the horror film, will restore the site to its original condition as of the initial date of
occupancy by _______________________________ unless otherwise agreed to by
both parties in writing, with reasonable wear and tear excepted.

2. Assignments:
You will give Suela Elezi, Laura Hodgson and Hannah Mcdonagh, the right to assign
all terms stated in the contract.
Hold harmless:
Suela Elezi, Laura Hodgson and Nikon Ahamed have agreed to hold you harmless
liability and losses that may occur by our employee and equipment.
Authority:
You understand the terms described in the contract. As you are over the age of 18
years old, you have the right to grant us the permission to your site.
Insurance:
Suela Elezi, Laura Hodgson and Hannah Mcdonagh will represent that we are
covered by the Public Liability and Property Damage Insurance.
