     age in place. To be rooted is perhaps the most important and least recognized need of the human
     condition. We are committed to keeping heart and home under one roof for as long as safely possible.

                                                           Last year, 700 individuals and businesses supported us with

                                                           hours of their time per month, a $1,100,000 in-kind, community

                            I invite you to meet 80-year-old Esther. She is able to age in place because
                            of the basics of life – food, friendship and increased mobility – that LIFE
                            ElderCare, a Tri-City nonprofit, provides. This was not the case two years ago.
                            In 2010, Esther took a bad fall that limited her mobility and forced her to make
                            some compromising choices. Because it was hard for her to prepare meals, she
                            developed severe malnutrition and was readmitted to the hospital.

                            After she was discharged, she started getting meals from Meals On Wheels, rides
                            to her doctor appointments from VIP Rides, weekly exercise assistance through
                            our Fall Prevention program and a new friend through Friendly Visitors. With
                            no family in the area, Esther now counts on LIFE ElderCare to help her stay in
                            her own home and live independently.


Every day, through no fault of their own, older adults are facing heartbreaking choices.

Many are forced to decide between medication and food, or not going to the doctor because they
have no transportation. And those like Esther, who have no family in the area, sometimes go for
weeks without seeing a friendly face.

In Fremont alone - the percentage of people aged 60 and older has grown by 30%.

Since 1975, we have provided a safety net for the mothers, fathers and grandparents in the
community. In the Tri-City area, unlike many other parts of the country, this vulnerable
population has access to a trusted, community-based organization that treats them like family.

But we can’t do it without your help.
