Defining Mental Health Within a Transcultural Nursing Perspective
The World Health Organization (WHO) (2007) proposed: “There is no health without mental health” and the influential organization incorporated mental well-being in their definition of health. According to WHO, “Health is a state of complete physical, mental and social well-being and not merely the absence of disease or infirmity” (p. 1). WHO further specified that mental health is “a state of well-being in which the individual realizes his or her own abilities, can cope with the normal stresses of life, can work productively and fruitfully, and is able to make a contribution to his or her community” (p. 1), and that this understanding of mental health can be interpreted “across cultures” (p. 1). For example, in the Rural Healthy People 2010 Report, survey results of state and local rural leaders indicated that mental health and mental disorders are the fourth most often identified rural health priority (Gamm & Hutchison, 2003). Taking into consideration the WHO definition of mental health, a definition of mental illness would then include one or more of the following: a lack of a sense of well-being in which the individual does not realize his or her own disabilities, is not able to cope with the normal stresses of life, is not able to work productively and fruitfully, and is not able to make a contribution to his or her community. It is important to remember there is a continuum of mental health on one end and mental illness on the extreme other end. An individual can fall on one end of the continuum or the other, or anywhere in between. Individuals’ and communities’ cultural beliefs and values about mental health and mental illness can influence one’s placement on the continuum, as well. It is a daunting task to know all there is to know about each cultural group that mental health nurses care for in their daily practice. Leininger (1991; Leininger & McFarland, 2002) in Culture Care Diversity and Universality: A Theory of Nursing, theorized the importance of identifying what is common and universal among cultures, while at the same time understanding there is individual diversity within cultures. Diversity for transcultural mental health nurses would encompass not only culture and ethnicity, but also gender, sexual orientation, socioeconomic status, age, physical abilities or disabilities, religious beliefs, and political beliefs or other ideologies. Figure 10-1 shows a transcultural nurse working on promoting health and well-being with a patient from a culture different from her own. In this chapter on Transcultural Perspectives in Mental Health Nursing, patterns of values, beliefs, and practices for mental health care are presented and can be used as one “tool” in caring for patients, families, and communities from diverse cultural groups. Loneliness has long been linked to poor physical health. In 2017, the American Psychological Association (APA) published a study that provided data supporting this claim. Its researchers had 213 participants take two tests to gauge their loneliness scores. The participants then received nasal drops containing rhinovirus 39 (common cold virus) and were put on quarantine for five days.

The technological advances of the past century tended tochange the focus of medicine from a caring, service-oriented model to a technological, cure-oriented model.
Technology has led to phenomenal advances in medicine and
has given us the ability to prolong life. However, in the past few
decades physicians have attempted to balance their care by re-
claiming medicine’s more spiritual roots, recognizing that until
modern times spirituality was often linked with health care.
Spiritual or compassionate care involves serving the whole per-
son—the physical, emotional, social, and spiritual. Such service
is inherently a spiritual activity. Rachel Naomi Remen, MD, who
has developed Commonweal retreats for people with cancer, de-
scribed it well:
Helping, fixing, and serving represent three different ways of see-
ing life. When you help, you see life as weak. When you fix, you
see life as broken. When you serve, you see life as whole. Fixing
and helping may be the work of the ego, and service the work of
the soul (1).
Serving patients may involve spending time with them, hold-
ing their hands, and talking about what is important to them.
Patients value these experiences with their physicians. In this
article, I discuss elements of compassionate care, review some
research on the role of spirituality in health care, highlight ad-
vantages of understanding patients’ spirituality, explain ways to
practice spiritual care, and summarize some national efforts to
incorporate spirituality into medicine.
COMPASSIONATE CARE: HELPING PATIENTS FIND MEANING IN
THEIR SUFFERING AND ADDRESSING THEIR SPIRITUALITY
The word compassion means “to suffer with.” Compassionate
care calls physicians to walk with people in the midst of their
pain, to be partners with patients rather than experts dictating
information to them.
Victor Frankl, a psychiatrist who wrote of his experiences in
a Nazi concentration camp, wrote: “Man is not destroyed by suf-
fering; he is destroyed by suffering without meaning” (2). One
of the challenges physicians face is to help people find meaning
and acceptance in the midst of suffering and chronic illness.
Medical ethicists have reminded us that religion and spiritual-
ity form the basis of meaning and purpose for many people (3).
At the same time, while patients struggle with the physical as-
pects of their disease, they have other pain as well: pain related
to mental and spiritual suffering, to an inability to engage the

Anxiety and depression have been prevalent issues in the United States and other parts of the world since the beginning of time. Anxiety and depression, which manifest in a variety of ways, can have a negative impact on people of all ages. Despite the fact that the first person to develop an addiction has never been identified, it is a widely held belief (Freeman, 2020). It has exacted a significant toll on the younger generation in recent years, which is especially concerning given the ongoing global epidemic. Many people do not prioritize their mental health and do not believe it should be one of their primary concerns. When, in fact, one's mental health is absolutely necessary in order to complete mundane tasks like going to class or working a job. As a result of increased focus on the topic from a wide range of fields, the state of people's mental health has been elevated to the status of a critical problem in today's society. Despite mounting evidence to the contrary, a sizable minority of people continue to believe that medication is the only way to treat mental health issues, despite the fact that the vast majority of people understand the importance of mental health (Ginsburg, 2020). This essay will coherently argue the alternative solutions to anxiety and depression. In that case, getting enough sleep and maintain a stable diet are the solutions other than medication that are discussed in below paragraphs.
Despite this, a significant number of people either do not give mental health the attention it deserves or do not believe it is all that important. Some people are also responsible for contributing to the stigma associated with mental illness. A little more than half of those polled in the General Social Survey in 2020 said they would not want a member of their family or a close friend to marry, work next to, or spend the night out with someone who had a mental illness (Knowles, 2020). People who have mental health issues may be hesitant to seek help because they are afraid of the stigma that may be attached to them if they do, which is a significant issue. The findings of this 2020 survey show that many American households continue to struggle with the concept of accepting mental health issues in their loved ones.
According to a 2020 statement from Zhang, "many people believe that people with mental illnesses are aggressive and dangerous." In fact, they are more likely to be assaulted or to injure themselves than they are to cause harm to others. People suffering from mental illnesses such as anxiety and depression are less likely to seek alternative treatments and are more likely to rely on excessive medication if these stereotypes are perpetuated. I do not believe that these points of view are valid, and I believe that they are harmful to people seeking treatment. We can be thankful that there has been .

Losing a sense of connection and community changes a person’s perception of the world. Someone
experiencing chronic loneliness feels threatened and mistrustful of others, which activates a biological defense
mechanism, according to Steve Cole, Ph.D., director of the Social Genomics Core Laboratory at the University
of California, Los Angeles. His NIA-funded research focuses on understanding the physiological pathways of
loneliness (the different ways that loneliness affects how your mind and body function) and developing social
and psychological interventions to combat it.
Dr. Cole said, “For example, loneliness may alter the tendency of cells in the immune system to promote
inflammation, which is necessary to help our bodies heal from injury. But inflammation that lasts too long
increases the risk of chronic diseases.”

purpose in life is linked to healthier immune cells. Helping others through caregiving or volunteering also helps
people feel less lonely.”
