Infection rates decreased significantly after implementation - the median rate fell from 2.7 to 0 infections per 1000 catheter days. The mean rate also decreased substantially and was sustained through 18 months of follow up. The large-scale study showed translating evidence into widespread practice can significantly reduce infections and save lives on a broad scale.

Physician-initiated daily verbal reminders to remove unnecessary urinary catheters decreased catheter duration more than nurse-initiated reminders. In a study comparing the two approaches, the mean catheter duration after physician reminders was 0.5 days compared to 1.7 days for nurse reminders. The physician reminders also significantly decreased catheter duration compared to a control group that received no reminders. Daily reminders from an infectious disease physician who was familiar with the hospital teams may have led to quicker catheter removal than from infection control nurses.
The role of bowel preparation in colorectal surgery
This study analyzed data from over 27,000 patients in the American College of Surgeons National Surgical Quality Improvement Program database who underwent elective colorectal resection from 2012 to 2015. The study found that while mechanical bowel preparation alone did not reduce postoperative infections, the combination of mechanical and antibiotic bowel preparation resulted in significantly fewer surgical site infections and Clostridium difficile infections compared to no preparation or antibiotic preparation alone. The authors conclude that combined mechanical and antibiotic bowel preparation should be used for elective colorectal resections when possible due to its effectiveness in reducing infectious complications.

Research-Based Literature Support for CAUTI
Preventing inappropriate use of a urinary catheter and the risk of patient harm associated with the use of indwelling catheters remains a significant challenge for healthcare facilities. The purpose of this paper is to provide research-based literature support from 10 (ten) different research-based references, on the prevention of CAUTI, evaluating interventions that support the use of nurse-driven protocols to reduce the incidence of CAUTI.
The following is the proposed PICO question: “How does the use nurse-driven protocol for evaluating the appropriateness and prevalence of catheter use or removal, compared with no protocol affect the CAUTI rates?”
The literature review from Fakih, Krein, Edson, Watson, Battles, and Saint (2014) aimed to engage healthcare workers to prevent CAUTI and prevent patient harm. A quantitative approach and a quasi-experimental design were used to assess the impact of guidelines for CAUTI prevention and reduce CAUTI rates in the state of Michigan through the use of a bladder bundle, promoting a daily evaluation of catheter indication (Fakih et al., 2014). The ability of the selected design to generate a cause and effect relationship collaborated in spreading educational insights and guidance to various stakeholders. A disadvantage of such design is the lack of control group, as internal validity is not easy to control once a probability of a stakeholder not following all the necessary steps of the protocol to generate true findings, for example nurses not consistent with their rounds to evaluate daily necessity of indwelling catheter, which are important in order to identify gaps and make appropriate and necessary modifications based on new evidence. Target population here are hospitalized patients from 163 units including ER, ICU, Surgical, and operating room from 71 acute care hospitals in the state of Michigan, with an equal probability of being selected resulting in random sampling and confidentiality (Fakih et al., 2014).
The aim of the study by Shehab (2017) is to analyze the impact of the protocol of care, from nurse’s knowledge, of patients that requires a urinary catheter. A quasi-experiment design was used to test assumptions of effective changes in nurse’s knowledge after caring out the protocol of care on patients with the urinary catheter by using a structured interview questionnaire and a nurse’s knowledge assessment tool (Shehab, 2017). An advantage of this study is to evaluate nurse’s readiness to follow protocols and to update their knowledge on how to provide the best EBP care for patients with a Foley catheter. ...

A hospital implemented a fast track colorectal surgery program to reduce patients' length of stay and improve recovery. The program utilized evidence-based practices like pre-operative education, early mobilization, and optimized pain relief. For 24 patients, the median length of stay decreased from 10 to 6 days with no adverse events. A patient satisfaction survey found high approval of the fast track approach. The program was expanded to involve more surgeons and showed potential to reduce hospital bed usage.

Introduction
Clinicians often witness impressive treatment
results in practice and may wish to pursue
research to formally explore their anecdotal
experiences. The potential to further new
knowledge both within the profession and to the
greater healthcare system is compelling. An
obvious next step for a practitioner considering
research is to connect with experienced
researchers to convey their idea for a study, who
may in turn ask, “What is your research
question?” With limited understanding of how to
respond, this interaction may result in the first
and last experience these clinicians will have with
the research community.
It has been estimated that between 1% and 7% of
the chiropractic profession in Canada is engaged
in research. Arguably, this low engagement
could be the result of practitioners’ perceived
importance of research and levels of research
literacy and capacity. However, increasing
demands for evidence-based approaches across
the health system puts pressure on all clinicians to
base their decisions on the best available
scientific evidence. Lack of clinician
representation in research has the probable effect
of limiting growth and new developments for the
profession. Furthermore, lack of clinician
involvement in research complicates the transfer
of study findings into practical settings.
The Canadian Institutes of Health Research
describes integrated knowledge translation as a
process that involves collaboration between
researchers and knowledge users at all stages of a
research project. This necessitates involvement
of clinicians to help in forming a research
question, interpreting the results, and moving
research findings into practice. This shared effort
between clinicians and researchers increases the
likelihood that research initiatives will be relevant
to practice. Conversely, it has been reported that
there is a growing communication gap between
clinicians and academics in chiropractic.

questions to ask, but many may lack the ability to
map out their research strategy, specifically in
communicating their question in a manner
required to develop a research protocol.

Introduction
After exploring the available case studies on the assigned websites Agency for Healthcare Research and Quality (AHRQ) Impact Case Studies site, I selected the case study titled Chicago Hospital Reduces Catheter-Related Infections to Zero with AHRQ’s CUSP Tools. (Agency for Healthcare Research and Quality, 2018). This seemed to be a great choice because, as a former ICU Registered Nurse, infection control was always at the top of our hospital’s quality initiatives.
Summary of Case Study
This case study was focused on reducing catheter-related infections in patients at Saint Anthony Hospital, which is a 150-bed hospital located in Chicago, IL. In order to achieve the goal of reducing catheter-related infections, the hospital adopted tools and strategies AHRQ’s Comprehensive Unit-Based Safety Programs (CUSP). The concept of team “huddles” was to have daily dialogue reporting of patients with special attention to those having catheters (urinary or venous). This awareness allowed for appropriate decision making regarding the need for the catheters to remain. The adoption of this method resulted in a 100% reduction rate of catheter-related infection between 2014 and 2017. If any barriers of catheter removal presented, such as provider preferences, they were addressed by hospital leadership. According to the case study, the medical director of infection control attributes the success of infection reduction to the use of huddles. They saw a reduction in readmissions, extended stays and a cost-savings of about $500,000 over a 2-year period.
Conclusion
Overall, the implementation of the huddles was a great achievement for the hospital. They were recognized by the Illinois Health and Hospital Association (IHA) and awarded the Innovation Challenge: Partners in Progress Award. This speaks volumes to the hospital's commitment to quality. Although I am no longer in direct patient care, I would say using the AHRQ CUSP tool and making it an all-inclusive practice was an excellent strategy by the hospital. Having all staff be accountable for seeing and reporting is vital to forward progress. The trickle-down effect is positive in that respect. Fewer catheters result in fewer infections which results in fewer readmissions resulting in cost-savings. That is a win-win for patients, providers and the hospital.

This study evaluated the outcome of immediately loading 15 dental implants in 4 patients over a mean follow-up period of 4.8 months. The mean marginal bone loss from implant surgery to immediate loading was 0.03 ± 0.07 mm, and 0.16 ± 0.17 mm after 3 months of continued loading. No implants failed, resulting in a 100% survival rate. The results suggest that immediate loading of dental implants can achieve high success rates of up to 6 months. However, long-term clinical studies with larger sample sizes are still needed.

Expo 15 returned to Manchester and was hosted once again by NHS England. Around 5000 people a day from health and care, the voluntary sector, local government, and industry joined together at Manchester Central Convention Centre for two packed days of speakers, workshops, exhibitions and professional development.
This year, Expo was more relevant and engaging than ever before, happening within the first 100 days of the new Government, and almost 12 months after the publication of the NHS Five Year Forward View. It was also a great opportunity to check on and learn from the progress of Greater Manchester as the area prepares to take over a £6 billion devolved health and social care budget, pledging to integrate hospital, community, primary and social care and vastly improve health and well-being.

Physician-initiated daily verbal reminders to remove unnecessary urinary catheters decreased catheter duration more than nurse-initiated reminders. In a study comparing the two approaches, the mean catheter duration after physician reminders was 0.5 days compared to 1.7 days for nurse reminders. The physician reminders also significantly decreased catheter duration compared to a control group that received no reminders. Daily reminders from an infectious disease physician who was familiar with the hospital teams may have led to quicker catheter removal than from infection control nurses.
The role of bowel preparation in colorectal surgery
This study analyzed data from over 27,000 patients in the American College of Surgeons National Surgical Quality Improvement Program database who underwent elective colorectal resection from 2012 to 2015. The study found that while mechanical bowel preparation alone did not reduce postoperative infections, the combination of mechanical and antibiotic bowel preparation resulted in significantly fewer surgical site infections and Clostridium difficile infections compared to no preparation or antibiotic preparation alone. The authors conclude that combined mechanical and antibiotic bowel preparation should be used for elective colorectal resections when possible due to its effectiveness in reducing infectious complications.

Research-Based Literature Support for CAUTI
Preventing inappropriate use of a urinary catheter and the risk of patient harm associated with the use of indwelling catheters remains a significant challenge for healthcare facilities. The purpose of this paper is to provide research-based literature support from 10 (ten) different research-based references, on the prevention of CAUTI, evaluating interventions that support the use of nurse-driven protocols to reduce the incidence of CAUTI.
The following is the proposed PICO question: “How does the use nurse-driven protocol for evaluating the appropriateness and prevalence of catheter use or removal, compared with no protocol affect the CAUTI rates?”
The literature review from Fakih, Krein, Edson, Watson, Battles, and Saint (2014) aimed to engage healthcare workers to prevent CAUTI and prevent patient harm. A quantitative approach and a quasi-experimental design were used to assess the impact of guidelines for CAUTI prevention and reduce CAUTI rates in the state of Michigan through the use of a bladder bundle, promoting a daily evaluation of catheter indication (Fakih et al., 2014). The ability of the selected design to generate a cause and effect relationship collaborated in spreading educational insights and guidance to various stakeholders. A disadvantage of such design is the lack of control group, as internal validity is not easy to control once a probability of a stakeholder not following all the necessary steps of the protocol to generate true findings, for example nurses not consistent with their rounds to evaluate daily necessity of indwelling catheter, which are important in order to identify gaps and make appropriate and necessary modifications based on new evidence. Target population here are hospitalized patients from 163 units including ER, ICU, Surgical, and operating room from 71 acute care hospitals in the state of Michigan, with an equal probability of being selected resulting in random sampling and confidentiality (Fakih et al., 2014).
The aim of the study by Shehab (2017) is to analyze the impact of the protocol of care, from nurse’s knowledge, of patients that requires a urinary catheter. A quasi-experiment design was used to test assumptions of effective changes in nurse’s knowledge after caring out the protocol of care on patients with the urinary catheter by using a structured interview questionnaire and a nurse’s knowledge assessment tool (Shehab, 2017). An advantage of this study is to evaluate nurse’s readiness to follow protocols and to update their knowledge on how to provide the best EBP care for patients with a Foley catheter. ...

A hospital implemented a fast track colorectal surgery program to reduce patients' length of stay and improve recovery. The program utilized evidence-based practices like pre-operative education, early mobilization, and optimized pain relief. For 24 patients, the median length of stay decreased from 10 to 6 days with no adverse events. A patient satisfaction survey found high approval of the fast track approach. The program was expanded to involve more surgeons and showed potential to reduce hospital bed usage.

Clinicians often witness impressive treatment
results in practice and may wish to pursue
research to formally explore their anecdotal
experiences. The potential to further new
knowledge both within the profession and to the
greater healthcare system is compelling. An
obvious next step for a practitioner considering
research is to connect with experienced
researchers to convey their idea for a study, who
may in turn ask, “What is your research
question?” With limited understanding of how to
respond, this interaction may result in the first
and last experience these clinicians will have with
the research community.
It has been estimated that between 1% and 7% of
the chiropractic profession in Canada is engaged
in research. Arguably, this low engagement
could be the result of practitioners’ perceived
importance of research and levels of research
literacy and capacity. However, increasing
demands for evidence-based approaches across
the health system puts pressure on all clinicians to
base their decisions on the best available
scientific evidence. Lack of clinician
representation in research has the probable effect
of limiting growth and new developments for the
profession. Furthermore, lack of clinician
involvement in research complicates the transfer
of study findings into practical settings.
The Canadian Institutes of Health Research
describes integrated knowledge translation as a
process that involves collaboration between
researchers and knowledge users at all stages of a
research project. This necessitates involvement
of clinicians to help in forming a research
question, interpreting the results, and moving
research findings into practice. This shared effort
between clinicians and researchers increases the
likelihood that research initiatives will be relevant
to practice. Conversely, it has been reported that
there is a growing communication gap between
clinicians and academics in chiropractic.
Clinicians have important practice-related
questions to ask, but many may lack the ability to
map out their research strategy, specifically in
communicating their question in a manner
required to develop a research protocol.

Introduction
After exploring the available case studies on the assigned websites Agency for Healthcare Research and Quality (AHRQ) Impact Case Studies site, I selected the case study titled Chicago Hospital Reduces Catheter-Related Infections to Zero with AHRQ’s CUSP Tools. (Agency for Healthcare Research and Quality, 2018). This seemed to be a great choice because, as a former ICU Registered Nurse, infection control was always at the top of our hospital’s quality initiatives.
Summary of Case Study
This case study was focused on reducing catheter-related infections in patients at Saint Anthony Hospital, which is a 150-bed hospital located in Chicago, IL. In order to achieve the goal of reducing catheter-related infections, the hospital adopted tools and strategies AHRQ’s Comprehensive Unit-Based Safety Programs (CUSP). The concept of team “huddles” was to have daily dialogue reporting of patients with special attention to those having catheters (urinary or venous). This awareness allowed for appropriate decision making regarding the need for the catheters to remain. The adoption of this method resulted in a 100% reduction rate of catheter-related infection between 2014 and 2017. If any barriers of catheter removal presented, such as provider preferences, they were addressed by hospital leadership. According to the case study, the medical director of infection control attributes the success of infection reduction to the use of huddles. They saw a reduction in readmissions, extended stays and a cost-savings of about $500,000 over a 2-year period.
Conclusion
Overall, the implementation of the huddles was a great achievement for the hospital. They were recognized by the Illinois Health and Hospital Association (IHA) and awarded the Innovation Challenge: Partners in Progress Award. This speaks volumes to the hospital's commitment to quality. Although I am no longer in direct patient care, I would say using the AHRQ CUSP tool and making it an all-inclusive practice was an excellent strategy by the hospital. Having all staff be accountable for seeing and reporting is vital to forward progress. The trickle-down effect is positive in that respect. Fewer catheters result in fewer infections which results in fewer readmissions resulting in cost-savings. That is a win-win for patients, providers and the hospital.

This study evaluated the outcome of immediately loading 15 dental implants in 4 patients over a mean follow-up period of 4.8 months. The mean marginal bone loss from implant surgery to immediate loading was 0.03 ± 0.07 mm, and 0.16 ± 0.17 mm after 3 months of continued loading. No implants failed, resulting in a 100% survival rate. The results suggest that immediate loading of dental implants can achieve high success rates of up to 6 months. However, long-term clinical studies with larger sample sizes are still needed.

Previous studies have shown that receiving antibiotics within 3 hours is important in decreasing the risk of infection However, more recent work has shown that administering antibiotics by 66 minutes is an even better guideline to avoid an infection. Current American College of Surgeons (ACS) Trauma Quality Improvement Program (TQIP) Best Practice Guidelines recommend receiving antibiotics within 60 minutes of arrival to the emergency department. - 4. This study aimed to assess whether this CQI, through the use of a measured process metric, feedback reporting, and regular collaborative meetings, could improve the timeliness of antibiotic administration to patients with open long-bone fractures. JOURNAL PURPOSE
- 5. A performance baseline for each trauma center and the overall CQI was established using data from 2017. This project was designed and carried out based on the Standards for Quality Improvement Reporting Excellence (SQUIRE 2.0) framework. METHODE

In conclusion, this study demonstrates that the time to receiving antibiotics for patients with an open lower-extremity fracture can be reduced by members of a statewide trauma CQI in a population health driven manner. Through a performance improvement initiative involving a process metric, participating centers achieved the goal of 85% patients in the CQI received antibiotics in less than 120 minutes after trauma center arrival.
