This document compares reasons for donor deferral between the Mansoura Regional Blood Transfusion Centre and the blood bank at Mansoura International Hospital over one year. At the regional centre, the most common reasons for deferral were low hemoglobin levels (31.4%), dental issues (23.85%), and age below 18 (16.3%). At the hospital blood bank, the most frequent reasons were age over 45 (49.2%), chronic diseases (21.1%), and low hemoglobin (16.8%). Background:
Blood donation decision making has been investigated worldwide for
decades to understand the process better to increase donation efficiency, safety,
retention, collection numbers and diversity of the donor pool.
Shortage of safe blood donors is frequent and it is important to understand
the causes of deferral of potential donors, who reside mainly in urban areas, to
improve recruitment campaigns aiming at the quality/availability of donors.
The donors are deferred for several reasons related to the safety of the
donor and the potential threat to the recipient. Analysis of rejection patterns may
help medical personnel in improving donor and recipient safety and also in
maintaining a healthy donor pool in the long run.
Aim of work:
To determine the reasons of donor deferral in Mansoura Regional Blood
Transfusion Centre and to compare the reasons of donor deferral between
Mansoura Regional Blood Transfusion Centre and blood bank of Mansoura
International Hospital.
Methods:
This study was carried out in Mansoura Regional Blood Transfusion Centre
(MRBTC) in cooperation with blood bank of Mansoura International Hospital from
1st of January 2012 to 31st of December 2012. It includes total number of
2. (49989) blood donors. They were divided into 2 groups; Group 1: it includes a
total number of (38663) blood donors attended to Mansoura Regional Blood
Transfusion Centre; Group 2: it includes a total number of (11326) blood donors
attended to blood bank of Mansoura International Hospital.
All donors were submitted to full clinical assessment including history
taking and clinical examination. Special emphasis was made on the following
points: hemoglobin level, blood pressure, pulse, body temperature, chest and
cardiac examination.
Results:
Of 38663 blood donors, 3215 (8.3%) were deferred for various reasons in
Mansoura Regional Blood Transfusion Centre ,while there were 609(5.38%)
blood donors were deferred from total 11326 blood donor in Mansoura
International Hospital at the same duration.
In Mansoura Regional Blood Transfusion Centre, low hemoglobin level
among donors was the commonest cause of deferral (31.4%) followed by dental
causes (23.85%), while the third cause of deferral was age (below 18 years)
(16.3%), otherwise in Mansoura International Hospital, the age (above 45 year)
of donors was the commonest cause of deferral (49.2%) followed by chronic
diseases (21.1%), while the third cause of deferral was low hemoglobin level
(16.8%). In our study 10.4% of donors in MRBTC were deferred for permanent
reasons (jaundice, chronic diseases and etc.).
Summary / Conclusion:
Our deferral study in blood donor candidates sheds light on regional
diversity, highlighting how social inequalities and health status of the general
population may affect the blood supply.
Temporary donor deferrals need to be actively and aggressively managed
so as not to lead to a diminished supply of future donors.
3. The difference of reasons of deferral between regional blood bank and
hospital blood bank is because of the regional blood bank implements the
national donor selection criteria and follows the national policy and standards.
