Easy-to-Use Home
Telehealth Monitor
A tablet-based telehealth monitor, provided by VA
at no cost, connects you at home with your local
VA Medical Center or Clinic.
Information from health devices such as blood
pressure and blood glucose monitors is electronically
transmitted to your Care Coordinator. This information
is then reviewed by your physician and VA care
team, who may recommend changes in your
treatment or lifestyle such as diet. They can also
advise you of tips, appointments for visits and more.
VA Home Telehealth securely stores your
information, so it can be accessed at any time by
your Care Coordinator and care team. This enables
them to provide you with the right treatment
inthe right place at the right time—and for you to
continue to live in the comfort of your own home
and community.
