This document summarizes a study examining the role of HLA variability in tuberculosis. The study analyzed the HLA-DRB1 and HLA-DQB1 genes in 93 tuberculosis patients and 101 healthy controls in Venezuela. The frequencies of DRB1*12 and DQB1*0602 were significantly higher in patients, while DRB1*03 and DQB1*0306 were higher in controls. Certain HLA-DQB1 allele combinations encoding or not encoding aspartate at position 57 in the beta chain were only found in patients. The DRB1*13-DQB1*06 haplotype was also more common in controls than patients. The results suggest HLA polymorphisms influence the occurrence of tuberculosis in these patients.

This study tested 619 patients in Germany for antibodies and RNA of the Borna Disease Virus (BDV) to examine the link between mental disorders and BDV infection. 416 psychiatric patients and 203 healthy controls were tested using reverse transcription polymerase chain reaction (RT-PCR) and serum testing. The results found that 40 of the psychiatric patients tested seropositive for BDV antibodies compared to only 3 of the controls. Of those psychiatric patients, 11 had schizophrenia and 7 of those 11 schizophrenic patients tested positive for BDV RNA strands. This provides evidence of a link between BDV infection and schizophrenia, as most BDV cases appeared in patients with schizophrenia.

This study retrospectively analyzed MEFV gene mutation testing results from 476 patients seen at UCLA between 2002-2012 to correlate genotypes with clinical phenotypes in different populations. They found:
1. The M694V mutation was significantly associated with Armenian ethnicity, seen in 35.3% of Armenian patients as a homozygous mutation and 77.3% of Armenian compound heterozygotes, compared to only 2.9% and 47.1% in non-Armenians respectively.
2. Armenian patients had a significant trend of increasing genetic contribution of the M694V mutation, while non-Armenians showed a decreasing trend.
3. The M694V mutation was also more prevalent in single mutation Armenian patients

Infective endocarditis is a life-threatening disease caused by bacterial infection of the endothelium and cardiac valves, either native or prosthetic. In the present work the role of the new microbiological techniques (techniques of detection and amplification of the subunit 16 ribosomal sRNA by means of the chain reaction of the polymerase in blood or tissue, fluorescent in situ hybridization, and matrix-assisted laser is reviewed desorption/ ionization time-of-flight mass spectrometry (MALDI-TOF MS) in the diagnosis of infective endocarditis.

This study evaluated the cost-effectiveness of screening all pregnant women (universal screening) or only those with a personal or family history of blood clots (selective screening) for a genetic mutation called Factor V Leiden (FVL) that is associated with complications in pregnancy. The researchers studied over 900 pregnant women. They found that with selective screening, the additional cost would be £3768 to prevent less than one complication, while universal screening's additional cost of £39,841 would prevent about three complications. Therefore, the study concluded that screening pregnant women for the FVL mutation is not cost-effective from the perspective of the UK healthcare system.

This study analyzed 211 Cryptococcus strains isolated from patients in Mexico City hospitals between 1989-1998. C. neoformans was the dominant species isolated (97.15%), followed by C. albidus and C. uniguttulatus. Most strains were isolated from cerebral spinal fluid (92.5%) and AIDS was the main predisposing factor (85%). The disease was found to predominantly affect males (87.3%) in their third (33.8%) and fourth (37.5%) decades of life. When compared to other Latin American countries, the isolation frequency of C. neoformans var. neoformans and var. gattii in Mexico was most similar to Brazil.

Intravenous Immunoglobulin and Plasmapheresis in Acute Humoral Rejection
This summary provides an overview of a study examining the use of intravenous immunoglobulin (IVIG) and plasmapheresis (PP) to treat acute humoral rejection (AHR) in kidney transplant recipients. The study analyzed 519 kidney transplant recipients between 1999-2003, identifying 23 patients (4.5%) with AHR. Most AHR patients were treated with IVIG and PP. Two-year graft survival for AHR patients treated with IVIG and PP (78%) was similar to those with acute cellular rejection (85%), and markedly better than historical rates without this treatment. Giant cell arteritis (GCA) is a common type of vasculitis that affects elderly individuals. The study assessed whether three single nucleotide polymorphisms (SNPs) in the BANK1 gene are associated with susceptibility to biopsy-proven GCA. Genotyping of 222 GCA patients and 534 controls found no significant differences in genotype frequencies between the two groups for the three BANK1 SNPs. There was a trend toward a decreased risk of GCA in individuals carrying a specific genotype. However, the results do not support a major role for these BANK1 gene variants in susceptibility to GCA.

Genome-wide association studies (GWAS) of venous thrombosis have identified several new risk factors. The first GWAS identified two novel loci - GPVI and F11. Subsequent studies confirmed these and identified other loci including a region on chromosome 6p24.1. GWAS have also provided insights into other thrombosis-related traits such as shortened activated partial thromboplastin time. Overall, GWAS have expanded understanding of the genetic factors underlying venous thrombosis beyond the major genes previously known.

This document summarizes a study that identified a signature of 10 cytokines and chemokines that can correctly identify patients with chronic fatigue syndrome (CFS) associated with xenotropic murine leukemia virus-related virus (XMRV) infection. The study used Luminex multi-analyte profiling to measure cytokine and chemokine levels in the plasma of 118 CFS patients who tested positive for XMRV, compared to 138 healthy controls. Analysis identified a cytokine/chemokine signature that diagnosed XMRV-associated CFS with 93% specificity and 96% sensitivity. Yeasts such as Candida are common causes of bloodstream infections in ICU patients. Candida infections in the ICU have a high mortality rate of 15-25% and are the 4th most common cause of hospital-acquired bloodstream infections. Diagnosis can be challenging due to low sensitivity of blood cultures, but newer tests such as PCR, antigen detection assays, and MALDI-TOF mass spectrometry provide more rapid detection of Candida compared to standard culture methods. The presence of risk factors such as abdominal surgery, central venous catheters, antibiotics use, and prolonged ICU stay increase the risk of developing Candida bloodstream infections in critically ill patients.

Carbamazepine (CBZ) is a common cause of life-threatening cutaneous adverse drug reactions such as Stevens–Johnson syndrome (SJS) and toxic epidermal necrolysis (TEN). Previous studies have reported a strong association between the HLA genotype and CBZ-induced SJS/TEN.We investigated the association between the HLA genotype and CBZ-induced SJS/TEN in Javanese and Sundanese patients in Indonesia. Nine unrelated patients with CBZ-induced SJS/TEN and 236 healthy Javanese and Sundanese controls were genotyped for HLA-B and their allele frequencies were compared. The HLA-B*15:02 allele was found in 66.7% of the patients with CBZ-induced SJS/TEN, but only in 29.4% of tolerant control (p = 0.029; odds ratio [OR]: 6.5; 95% CI: 1.2–33.57) and 22.9% of healthy controls (p = 0.0021; OR: 6.78; 95% CI: 1.96– 23.38). These findings support the involvement of HLA-B*15:02 in CBZ-induced SJS/TEN reported in other Asian populations. Interestingly, we also observed the presence of the HLA-B*15:21 allele. HLA-B*15:02 and HLA-B*15:21 are members of the HLA-B75 serotype, for which a greater frequency was observed in CBZ-induced SJS/TEN (vs tolerant control [p = 0.0078; OR: 12; 95% CI: 1.90–75.72] and vs normal control [p = 0.0018; OR: 8.56; 95% CI: 1.83–40]). Our findings suggest that screening for the HLA-B75 serotype can predict the risk of CBZ-induced SJS/TEN more accurately than screening for a specific allele.More Related Content

Giant cell arteritis (GCA) is a common type of vasculitis that affects elderly individuals. The study assessed whether three single nucleotide polymorphisms (SNPs) in the BANK1 gene are associated with susceptibility to biopsy-proven GCA. Genotyping of 222 GCA patients and 534 controls found no significant differences in genotype frequencies between the two groups for the three BANK1 SNPs. There was a trend toward a decreased risk of GCA in individuals carrying a specific genotype. However, the results do not support a major role for these BANK1 gene variants in susceptibility to GCA.

Genome-wide association studies (GWAS) of venous thrombosis have identified several new risk factors. The first GWAS identified two novel loci - GPVI and F11. Subsequent studies confirmed these and identified other loci including a region on chromosome 6p24.1. GWAS have also provided insights into other thrombosis-related traits such as shortened activated partial thromboplastin time. Overall, GWAS have expanded understanding of the genetic factors underlying venous thrombosis beyond the major genes previously known.

This document summarizes a study that identified a signature of 10 cytokines and chemokines that can correctly identify patients with chronic fatigue syndrome (CFS) associated with xenotropic murine leukemia virus-related virus (XMRV) infection. The study used Luminex multi-analyte profiling to measure cytokine and chemokine levels in the plasma of 118 CFS patients who tested positive for XMRV, compared to 138 healthy controls. Analysis identified a cytokine/chemokine signature that diagnosed XMRV-associated CFS with 93% specificity and 96% sensitivity. These patients were more likely to be female, black, have received a deceased donor kidney, have a positive panel reactive antibody, and have received induction therapy. Most AHR patients were treated with plasmapheresis and intravenous immunoglobulin, while ACR patients received steroids or antilymphocyte therapy. Yeasts such as Candida are common causes of bloodstream infections in ICU patients. Candida infections in the ICU have a high mortality rate of 15-25% and are the 4th most common cause of hospital-acquired bloodstream infections. Diagnosis can be challenging due to low sensitivity of blood cultures, but newer tests such as PCR, antigen detection assays, and MALDI-TOF mass spectrometry provide more rapid detection of Candida compared to standard culture methods. The presence of risk factors such as abdominal surgery, central venous catheters, antibiotics use, and prolonged ICU stay increase the risk of developing Candida bloodstream infections in critically ill patients.

Carbamazepine (CBZ) is a common cause of life-threatening cutaneous adverse drug reactions such as Stevens–Johnson syndrome (SJS) and toxic epidermal necrolysis (TEN). Previous studies have reported a strong association between the HLA genotype and CBZ-induced SJS/TEN.We investigated the association between the HLA genotype and CBZ-induced SJS/TEN in Javanese and Sundanese patients in Indonesia. Nine unrelated patients with CBZ-induced SJS/TEN and 236 healthy Javanese and Sundanese controls were genotyped for HLA-B and their allele frequencies were compared. The HLA-B*15:02 allele was found in 66.7% of the patients with CBZ-induced SJS/TEN, but only in 29.4% of tolerant control (p = 0.029; odds ratio [OR]: 6.5; 95% CI: 1.2–33.57) and 22.9% of healthy controls (p = 0.0021; OR: 6.78; 95% CI: 1.96– 23.38). These findings support the involvement of HLA-B*15:02 in CBZ-induced SJS/TEN reported in other Asian populations. Interestingly, we also observed the presence of the HLA-B*15:21 allele. HLA-B*15:02 and HLA-B*15:21 are members of the HLA-B75 serotype, for which a greater frequency was observed in CBZ-induced SJS/TEN (vs tolerant control [p = 0.0078; OR: 12; 95% CI: 1.90–75.72] and vs normal control [p = 0.0018; OR: 8.56; 95% CI: 1.83–40]). This study analyzed 415 patients with brucellosis in Tehran, Iran between 1998-2005. Most patients were female, aged 20-40 years, and had symptoms for less than 2 months. The most common symptoms were sweating and fever, while the most frequent signs were fever, arthritis, and splenomegaly. The majority of patients consumed non-pasteurized dairy products. Rifampin plus cotrimoxazole was the most common treatment, but resulted in the highest relapse rate, while doxycycline and cotrimoxazole led to the fewest relapses. Relapse remained a complication even following appropriate treatment.

This study examined the association between HLA-B*1502 and carbamazepine-induced Stevens-Johnson syndrome (SJS) in North Indians. The study found that HLA-B*1502 was present in 2 of 9 (22.2%) patients with carbamazepine-induced SJS but in none of 37 carbamazepine-tolerant controls, suggesting HLA-B*1502 is a risk factor. HLA-B*1502 was not associated with phenytoin-induced SJS. The results provide evidence that HLA-B*1502 testing should be performed before initiating carbamazepine treatment in North Indian patients.

This study evaluated the diagnostic accuracy of a Trypanosoma cruzi Loop-mediated isothermal amplification (Tc LAMP) kit for detection of T. cruzi infection in congenital, acute, and reactivated Chagas disease. Clinical samples from 46 patients were tested including those with confirmed congenital Chagas disease, recipients of organs from infected donors, cases of oral transmission, and coinfected HIV/T. cruzi patients. This cross-sectional pilot study evaluated immunology patterns in patients with acute and chronic phases of Bartonella bacilliformis infection in Peru. Patients between 5-60 years old from endemic areas were included. For those in the acute phase, elevated levels of the anti-inflammatory cytokine IL-10 and abnormal CD4+ and CD8+ T-lymphocyte counts correlated with an unfavorable immune state that may allow bacterial persistence. During the chronic phase, elevated IFN-γ and IL-4 levels correlated with previous findings of endothelial invasion in animal models. This study analyzed Trypanosoma cruzi seroprevalence in 520 pregnant women and their newborns in Guanajuato, Mexico. Anti-T. cruzi antibodies were detected in 20 mothers (4%) by ELISA and IHA tests. Four newborn cases tested positive by PCR. Risk factors for T. cruzi infection that were significant (P < 0.050) included building materials of dwellings, presence of pets, and dwellings in rural areas. Hospital,Tripura. This hospital based cross sectional study was done on 60 confirmed cases of falciparum
malaria (either by peripheral smear or rapid diagnostic test) admitted in Kulai District Hospital. A case sheet
proforma was prepared and data (demographic profile,clinical feature, investigation, treatment, and
complication) from all indoor patients was collected and analyzed.
Result: Out of 60 patients, 40(66.6%) were males and 20 (33.4%) were females. Most of the patients were
between the age group 21-40 years with the highest prevalence between the age group of 21-30. Fever was the
most common symptom. Anemia was present in 42(70%) patients, out of which 6(10%) patients had severe
anemia. Thrombocytopenia was present in 36(60%) patients.Abnormal liver function tests were observed in
26(43.3%) subjects while abnormal kidney function tests were observed in16(26.6%) patients. All the 60
patients received Artemisinin based antimalarial drugs.
Conclusion: Early detection, prompt management, and adequate supportive therapy may reduce mortality due
to falciparum cerebral malaria.

This study analyzed the clinical and laboratory profiles of 150 adult patients diagnosed with dengue fever at a hospital in India over 5 months. Most patients were male between the ages of 21-40. The most common symptoms were fever, headache, myalgia, and abdominal pain. Bleeding manifestations occurred in 19% of patients, most commonly melena. Laboratory findings included thrombocytopenia in all patients and elevated hematocrit in 23% of patients. 21% of cases had severe dengue hemorrhagic fever/dengue shock syndrome. The study aims to better understand the characteristics of dengue patients to aid in diagnosis and management.

This study aimed to determine the frequency of mosaicism in neurofibromatosis type 2 (NF2) patients with bilateral vestibular schwannomas. The researchers analyzed 233 NF2 patients for mutations in the NF2 gene via exon scanning of blood samples. Mutations were detected in 52% of samples. In 10 cases, the ratio of mutant to normal alleles suggested mosaicism. Tumor specimens from 35 patients without blood mutations were also analyzed, revealing mutations in 21 tumors. Identical mutations in multiple tumors from 9 patients supported constitutional mosaicism. Extrapolating these results, the estimated rate of mosaicism was approximately 25% in this cohort. It found 55 cases of West Nile infection, including 12 cases of West Nile fever and 43 cases of West Nile neuroinvasive disease. Plasma levels of circulating nucleic acids (CNAs) were significantly higher in patients with Plasmodium vivax malaria compared to healthy donors. CNAs levels strongly correlated with clinical markers of malaria morbidity, including fever and thrombocytopenia. Higher CNAs levels were also associated with a more severe clinical presentation based on a scoring system. These findings suggest that CNAs have potential as sensitive biomarkers for assessing severity of P. vivax malaria infections.

Este documento describe un estudio que evaluó la expresión de la proteína gp-170 en biopsias de tejido mamario normal y tumoral mediante inmunohistoquímica. Los resultados mostraron que el anticuerpo monoclonal C494 detectó gp-170 en el 57.14% de las muestras tumorales, mientras que C219 solo detectó en el 7.14%, indicando que C494 es más sensible. La expresión de gp-170 se asoció con grados más altos de malignidad y cambios post-terapia. Las muestras normales fueron negativas.

Z. Layrisse + Author Affiliations 1. IVIC, Caracas, Bolivarian Republic of Venezuela Abstract Tuberculosis (TB), is still a problem of public health in Venezuela. By still unknown reasons only 30% of the individuals exposed are infected, 5% of them develop the illness within the first year of the infection and 95% remain in latency. The susceptibility to TB is multifactorial and genetic factors of the host play an important role. The aim of this study was to investigate the variability of HLA-DRB1 and HLA-DQB1 genes in patients with clinical diagnosis of pulmonary tuberculosis and healthy individuals exposed to the bacillus. We have examined these polymorphisms in a case-control study of 194 Venezuelan ethnically-mixed adults, including 93 patients with smear positive pulmonary TB (mean of age 37,9 years) and 101 unrelated healthy controls (mean age 41,4 years), selected of the Algodonal Hospital in Caracas. Definition of HLA variants was done using Dynal RELI SSO HLA Typing Kits. Allelic and genotypic frequencies were determined by direct count and the association was established as odd ratios (OR). DRB1*12 and DQB1*0602 frequencies were significantly increased in patients, while HLA- DRB1*03 and HLA-DQB1*0306 were increased in controls. The analysis of HLA-DQB1 alleles that encode or not aspartate at position 57 at the β chain (ASP-β57) showed the presence of the phenotypes: DQB1*0301-0302 (ASP-β57/No ASP-β57), DQB1*0301-0307 (ASP-β57/No ASP-β57) and DQB1*0202-0501(No ASP-β57/ No ASP-β57) only in patients. The DRB1*13-DQB1*06 haplotype was observed also increased in the controls vs. patients (OR: 0, 26; p: 0.031). The results suggest an influence of HLA polymorphism in the occurrence of TB in our patients.
