Reflecting on the NHS Five Year Forward View and the Call to Action on General Practice, I conclude there is much to be done still. I use our recent experience of the Prime Minister's Challenge Fund to suggest ways in which CCGs can accelerate primary care transformation.

For example, an electric bike company sold more bikes to Go Green members and a printing company saved £48,000 by installing new power management software from tips at Go Green events. The growth of the low carbon sector supports thousands of jobs and is important as the UK works towards its goal of an 80% reduction in carbon emissions by 2050.

Infrastructure forms the framework within which modern societies operate both at the physical and social level. It
includes (amongst others) digital, green and social infrastructures, emergency services and food networks as well as water, energy, waste and transport. In 2012 Liveable Cities, a 5-year research programme, combining the Universities of Birmingham, Lancaster, Southampton and UCL, was established to identify and test radical engineering interventions that will lead to liveable cities of the future (low carbon, resource secure cities in which societal wellbeing is prioritised). This paper presents a discussion of what comprises a truly liveable city and how it might be achieved. It presents the City Analysis Framework, a technique for the analysis of city operations that establishes a hierarchy of needs relevant to successfully achieving a liveable city. The results reflect the changing perceptions and importance of infrastructure
and how proposed changes can radically alter people’s lifestyle and wellbeing.

Editor's Notes
Person centred care – it’s about wrapping care around the person
We have to change the conversation from “What’s the matter with you” – to “What matters to you?”
Patients having access to, and understanding their medical records gives them more control and responsibility around managing their long term conditions
Let’s not think about the patient moving around the system, but shift pathways to move around the patient. Treat the patient as a whole – not by specific conditions.
People having support to manage their conditions early on helps them to be more involved in their care
The only person who knows my condition and my body best… is me!
Quality of care isn’t about ‘other people’. It’s about every single one of us – we are all potential patients
One of the biggest stumbling blocks is attitude – “But we have always done things this way”. We can all play a part in thinking, and doing things differently
There is a ‘generation issue’. People may think they are being a burden, so don’t talk honestly about their conditions, how they are managing them, and how they are feeling.
People manage their bank accounts online… why not their health?
Let’s shift the power! “OK Doctor… the patient will see you now”.
People have expectations. If you can shop online and track your parcel the whole way, why can’t you manage your health conditions in a similar way?
It’s so important that people are involved in being informed about, and managing their conditions. It makes them feel more in control.
People who live with their conditions are experts on their symptoms and their feelings.
Information should be tailored to the individual to enable them to be informed about their conditions. Different people will respond to different ways of presenting information. Some will want to read leaflets, some will want to go online, some will want to talk to experts.
Patient advocates are key. They know about the condition, how they manage it, and how it affects their lives.
Use plain English. People don’t always understand medical phrases, jargon and acronyms.
It’s vital that care co-ordination is about the person – and not the diagnosis of their condition.
If we really want to understand long term conditions, it is important to stand in the person’s shoes.
Hope is vital. Everyday achievable goals matter to people – it’s about hope for the future.
