Electronic Health Record (EHR) is the computerized storage and sharing of patients’ health information to help in continuous monitoring of the patients’ health (Shickel B., 2017). This is a system developed to enable health clinics share information that can help in providing effective medication to the patients with different kinds of health needs. The data on patients is stored and accessed by the clinics during visits from the patient which will help in care management of the patients. An electronic health record system can be helpful as the information stored consist of medical history of a patient, laboratory tests, treatment plans, immunization dates and various allergies of the patients. This is helpful when the patient visits different clinic health providers where they will not need to explain the situations over and over again.
Electronic health record system automates information sharing and reduces the traditional paper work which was tiresome and had a great risk of losing information. With the HER, information on patients is kept in a secure system where only authorized persons can access it. Errors are minimized in provision of health care since the information kept can be more accurate and available at any given time.
Wasatch Family Clinic will greatly benefit from this strategy of recording, keeping and sharing of information on patients. The nurses can use the system to easily record the patients’ names, numbers and all other critical information required during scheduling for clinical attendance of any patient. Tracing of the information will be easier compared to using the traditional form of papers in storing information for a patient.
Need to share data
Information on health status of a patient has to be kept with care and only authorized persons can be able to access them. This helps in building ethical handling of patients’ information which creates their trust on the health care providers (Drazen J., 2015).
Wasatch Family Clinic needs to share their health data with the patients for them to understand their health issues. The clinic also needs to share data with other health facilities in order to increase the patient’s safety and a great care.
Duplicate registrations will be avoided by sharing data in the different departments of the health care center. A real-time link can be created for the patients from registration, through consultation, testing and final medication. Electronic Health Record (EHR) is continuing to evolve in today’s medical facilities. The American Recovery and Reinvestment Act mandated health organizations to transition to Electronic Medical Records (EMR) by January 1, 2014 in order to maintain Medicaid and Medicare reimbursement (USF Health, 2019). For this week’s discussion I will reflect on the module resources on digital information tools and technologies. I will discuss the healthcare technologies used in the health organization I work for. Lastly, I will reflect on any possible health technologies and the impact it has on nursing practice and healthcare delivery.
The health organization I work for switched to EPIC software for EMR two years ago. EPIC is user friendly for nurses as well as patients. EPIC corresponds to MyChart giving patients access to on their medical records (MyChart, 2019). I work in outpatient Endoscopy clinic where part of patient After Visit Summary (AVS) we educate patients on the use of MyChart and provide them with a code so they can access MyChart from home. Patients can access their medical records, and view lab results, make appointments, communicate with their provider, and pay their medical bills with MyChart (2019). ProVation is another form of technology used in the facility I work for. ProVation is used by the physicians where they document the Endoscopy procedure outcomes and results noted by the physician (ProVation, 2019). The physicians can document their findings as well as list out orders or management solutions for the patients (ProVation, 2019).
EHR has made it possible for nurses to provide efficient patient care as it has given us the ability to share patient information with other providers and health care organization departments, such as pharmacy, laboratory, etc (HealthIT, 2018). Patient care and experiences are improving because patients are being included in their care and they can make decisions in the plan of care. Having quick access to medical records and information results in increased patient satisfactions. Also, by combining patient portals inpatient healthcare facilities has resulted in a decrease of medical errors and adverse events (Dyes et al, 2017).
EHR poses a list of challenges with the main one being security safety. Web-based technology does put us at increased risk of breach of information by hackers. It can also be challenging for individuals to navigate web-based health technology, resulting in decreased patient satisfaction. Our older population is reluctant when it comes to using technology, they prefer paper written information. Electronic health records (EHRs) are useful, but did you know that they
could be even better?
It’s true. But usually, this improvement isn’t a solo effort.
Artificial intelligence (AI) could provide the tools your practice needs to fully
use and appreciate your EHR. Here’s how.
Extract data
First and foremost, electronic health records are records that contain data.
Despite all their potential applications, EHRs are worthless if medical
providers aren’t able to access the data within them.
Different software systems incorporate artificial intelligence in different
ways. Some use machine learning (ML), a component of AI that uses
algorithms and technologies to locate patterns and develop determinations.
Researchers have found that processing EHR data could help machine
learning systems recognize and use such information.
For example, ML tools could use algorithms to find and analyze information
about social determinants of health, which are environmental conditions
that could influence people’s health, well-being, and quality of life.
Document information
Finding data is important. So is documenting it.
After all, if medical practices don’t record information, it’s very difficult, even
impossible to use. Professionals won’t know where their patients are in
their treatments, won’t be able to reference data in the future, might end up
repeating their work, or might unintentionally do things that hurt patients
because they lacked any warnings.
Some EHR systems use AI speech-to-text (also known as speech
recognition) functions. Speech-to-text tools convert language spoken out
loud into written language on a page.
AI and EHR tools might record interactions between medical providers and
patients. The AI applications automatically add information into the patients’
records even as their conversations continue.
2. Such recording processes gather and store information, enabling
practitioners to use it in the present moment and in the future. By shifting
some work to technology, it also eases notetaking and paperwork burdens
and could help prevent professional burnout.
Maintain accuracy
Recorded conversations also provide other benefits.
They ensure accuracy. Clinicians could check recordings to learn what
patients actually said during their appointments instead of trying to
construct their conversations from their memory.
It works the other way, too, as professionals could study recordings and
transcripts to see what they said and did (or didn’t say and do). This could
motivate them to contact the patients again and take further action.
Using AI to record conservations and gather health data compiles data
directly from patients with no one in between. Such direct contact
eliminates the possibility that people mishear, misunderstand, or mis record
vital health information.
Recordings from appointments might also contradict other information in
patients’ files. If there are differences, medical professionals could correct
inconsistencies in patients’ records or investigate if there are issues, they
need to address further.
