3. INTRODUCTION
This paper titled “Health information card system”, submitted by Musammat Samina to the
Bangabandhu Sheikh Mujibur Rahman Aviation and Aerospace university, has been accepted as
satisfactory for the partial fulfillment of the requirements for the Microsoft CMT of IEEE CS
BDC Summer Symposium 2022 and team spark approved as to its style and contents. The
presentation has been held on 11th
June 2022.
Most of the people don't understand where to go and which file to show in doctors chamber.
Some time doctors can't find out what's the problem patient that could be recover easily. When
all information record in one card and we can search easily patient problem that makes it
different from the world we live in now.
We all encounter research in our daily lives. The results of research on many topics are presented
to us in the form of newspaper articles, books, reports, and television programmes. My focusing
area are unique health record card for all people. Which are particularly used for recorded health
information in one place. That are easily find information because it will be paper less. Day by
day our journey and time are so much pressurized.

4. PROBLEM STATEMENT
A critical part of any developing paper is the requirements developed for a card system.When the
customer finds it difficult to articulate their requirements, a health information card provides a
low cost way to help them visualize a potential solution. Often customers are not entirely sure
what they want and an health information card can help tease out their requirements. The aim of
our research is to develop a card like debit or credit where the health information document
activities will be centralized. The health information card will be maintained for monitoring and
writing about any information documentation. The patients and receptionist can create and
maintain their own pages for health information document and doctors can review patients report
to check health information document status and give feedback. Once you have established and
agreed a clear set of requirements, the next step is to model a software solution and this stage
helps the team to create a clear and unambiguous health information card design document.
The current system has many problems. In this section we discuss those problems-Each and
every information document paper creating manually.

❖ Sometimes patients and doctors cannot get proper information about the treatment.
❖ The patients and doctors cannot find the health information document details easily.
❖ This system is not efficient and takes long time
❖ Long time to wait to visit doctors chamber
❖ Long time to get medical report
❖ Patients need to medical test several time

5. OBJECTIVE
Health information card as like debit or credit card. That can easily punch with sensor fingerprint
anywhere. Also easily give and take information from any hospital.
The health information card in medical sector for sustainable rural development
An health information card system can reduce overuse of report or paper less the way for
doctors, for instance, while adding smart managing capabilities to existing health management
systems can identify when report are unoccupied and wa1
iting lots to visit doctors room then
patients are uncomfortable for sitting outside in those systems. The use of card can further
contribute to this efficiency by driving down costs, lowering waste, improving quality etc.
The goal of our paper is to make the entire health system efficient and user friendly to the
doctors, patients, manager and administrator. The objectives focusing on our attempt are mainly
concerned:
❖ To increase the flexibility of the doctors and patients..
❖ To make an easier time consuming system to review the report.
❖ To encourage administrators to upload patients' report image.
❖ To monitor report progress.
❖ To facilitate the doctors so that he can easily access patients information from anywhere.
❖ To eliminate the paper work of the chat system like skype.

6. RELATED WORK
After the birth of any child, the hospital can give a card with birth information. Which are used
for her future health information in any hospital. Thus, through various media we have become
accommodate to seeing, reading and hearing about health information cards and although we
may not be aware of it, we are used to making our own judgements about information file
findings. If we take the example of diet and health, many of us have taken into consideration
information that has been presented on ‘healthy eating’ and have made our own choices about
diet within the context of our own lives. We assess the information health record which we are
presented with, form an opinion as to the validity and relevance of the information card.

7. METHODOLOGY
Methodology is defined as a collection of procedures,
techniques and tools that are used for the development.
The methodology is not only a set of methods but it also
includes the ideas, rationale and assumptions which is
underlying the research or investigation. Software
development methodology helps us to contribute our
effort to develop and implement a system by the collection
of theories and concepts, comparative studies of different
approaches and by development of individual methods.

health records to electronic health records, the health care sector is increasingly relying on
technology to maintain the integrity of and update patients’ data. This reliance on technology
requires an acute level of protection from technological disasters and/or threats of human error or
sabotage.
Research has shown there are inadequacies in the installation and use of security controls for
health information records and that current methods of security analysis lack the techniques to
analyse the technical and social aspects of security.
This paper reports on progress towards development of a health information security evaluation
methodology based on Unified Modelling Language techniques, and discusses an imminent case
study that will be used for validation of the methodology.

8. SIGNIFICANT RESULT
Time management, cost effectiveness, paper less and most of the innovative thoughts at a time
doctors can see old records easily. So these are some of the innovations and advancements made
in our country.
Doctor, receptionist and patients are the key players in this card system.
At the same time, HIC is enabling greater connectivity in cities, rural areas, and homes. Our role
is to support our customers’ innovations by enabling them with the most appropriate connectivity
for their services and devices wherever they are in the world. When these gains compound
together, they add up to a substantial impact. Our connectivity performs an important part in
enabling sustainability helping customers develop innovations for a healther, better future.

9. CONCLUSION
The main contribution of this paper is health information card for the requirement gathering
system, using the software development method of modern technology. This model including
various services and tools in the context of a patient's requirement, such as: uploading documents
and reporting images, useful links, assessment, and simple report search. My interest in making
this paper stems from the desire to facilitate all the key interaction patterns in portable device,
debit or credit card. A list of the technologies used in the implementation of web-based system
includes PHP Platform, Apache Web Server, Laravel Framework, and MySQL database.
One is that the proposed card, which contains a hierarchical content structure and papers
relationships between concepts, can provide related useful information for searching and
sequencing learning resources in web-based communication platforms.
The other is that it can help an instructor to develop a communication by helping the structure
understand the how and why of the requirements gathered by developing a health information
card. This card will give the instructors a complete view of paper progress by each developer and
allow instructors to provide targeted feedback and help to the client.
Future Scope
❖ In future I will extend the whiteboard solution for documentation present easily.
❖ Presently we are working on health information card in future it will be more features
with the other modern health information storage concept
