TYPE OF THE VESSEL : Tanker (2000 tonnes)
VOYAGE:Coastal Area Around the United Kingdom.
DETAILS OF OFFICER’S : Master – British.
Chief Engineer –British.
Officer’s –Spanish.
Crew’s – Filipino.
WORKING LANGUAGE : English (on board).
SCENARIO : 1
VOYAGE : Hull and South Shield’s .
TYPE OF INCIDENT : Disposal of oil.
REASON BEHIND THE OIL DISPOSAL:
On board in that case the main problem is understanding and the language
problem .The total crew member on board ship 12. Most of the crew member Spanish . For
that reason when they communicate each other, there must have a lack of understanding
because all the crew member’s are Filipino .when 3rd officer gives job or instruct the able sea
man , he just nodded . He can not give reply. For that reason he can not give a better out put
in his work. And the ship face this type of problems in that case oil spillage .
On the other hand able sea man should need to informed (Duty Officer’s or
Bosan ) when he was instructed by the 3rd officer ,He failed to do that job .The AB should not
do his job properly for that reason the bosan or cheif officer did not come to know.
In that case it is also 3rd officer mistake to informed chief officer so that he
could have checked how the work will be done. (This scenario basically shows the lacking of
communication in able sea man and as well as the 3rd officer also. ) the 3rd officer have poor
communication skills and as well as the AB also.
HOW THIS PROBLEM CAN BE SOLVED?
1:When the 3rd officer instructed the AB , again he should ask him while he
understand or not that particular job.

man.
SCENARIO:2
TYPE OF INCIDENT :Lack of checking IG system or it not work properly.
RESPONSIBLE PERSON FOR THAT INCIDENT : Chief officer and 2nd Officer.
REASON BEHIND THE INCIDENT :
It is very clear in front of us that lack of planning and as
well as lack of commitment .As a sea man they must know about the safety of ship ; safety of
crew; safety of environment and marine life .They also need to concern about the company
benefit and business policy.
It is also the mistake of cheif officer that he must check the
IG system while it is working properly or not. And as well as all the feature of IG plant
system in a tank. Cheif officer need to check the percentage of oxygen in a particular tank
because he is the incharge of cargo operation in properly but he failed to do his job .the
oxygen contents in a particular tank is maintaining 5% or less but in that case it exited the
limit.It is the vital job of cheif officer that to ensure every tank is inerted during cargo
operation.But the cheif officer also did not check the inert gas system.
At least the master need to check or monitor all the job
because he is the incharge of whole ship before in the log book entries .Master must need to
arrange meeting before loading or discharging cargo and discusses about the type of cargo
and preparation or precautions in the time of cargo operation .He discusses each part of
loading and discharging , how to do it more safely.
Also risk assignment need to do and check all the safety feature as well as all the operation..
HOW CAN WE SOLVED THAT PROBLEM?
1.Master need to explain briefly about cargo operation and all safety
features.
2.All the safety meeting must be held at a time of intervals.
4. MARINE LAW ANDMANAGEMENT
In the term of safety all the crew members must familiarized and must correct him if there is
any doubt to regarding safety.
SCENARIO 3
INCIDENT : 3rd officer can not do his job properly( failed to do new stock)
REASON OF THE INCIDENT: 3rd officer did not know his responsibility and he did not
have proper knowledge to do so.
REASON BEHIND THE INCIDENT :
The 3rd officer do not have proper knowledge about Shipboard
oil pollution emergency plan drills .He does not read the discription of plan .The 3rd officer
did not have proper training and he is not concern about his responsibility and duty. The 3rd
officer need to aware about his duty and responsibility.And before carring any drills , he need
to read the discription about that drills.
Though 3rd officer is a new officer rank this can happen .Then
the master gives him a verbal warning because his un-responsibility .The 3rd officer have
poor management skills and don’t show any awareness .Other hand the master have a better
management skills ,so he can do it.
He need to motivate the crew so that the working condition
remain good .If the 3rd mate do not change his attitude and if he de-motivate then the working
atmosphere will be change badly that is not good and it effect on board working level.
As it is the mistake of 3rd officer and he do not know about
his duty and responsibility, in the before the master need to check the 3rd mate wheather he is
proper trained or not .This is master responsibility to find out fault of officer and motivate
them. The duty officer must know about :
In the ship the 3rd
officers duty or responsibility to
maintains and ordering the lifesaving appliance.
Firefighting equipment and all other equipment related to
the drill.
Regularly check all the drill equipment and make a list of
the equipment.
Re orders all the absence equipment as soon as possible.
Refill all the extinguisher in case of after use it
5. MARINE LAW ANDMANAGEMENT
Master is the main manager of the ship and he is act as a
leader so that he take some action to keep the working condition of ship remain good .He
takes some action and all the officers need to help him to get a better out put.
SCENARIO :4
INCEDENT : The ship is detain by port state control.
REASONS : Lack of knowledge about equipments and poor management.
This scenario is basically about the drills which is carried out
time to time on board ship .In the muster list it says about all the drills and its intervals of
time .It is very important that all the drills carried out as per regulations because in case of
emergency everybody should know what to do .All the crew members must know how to
lowering lifeboat and need to familiar about all the features of lifeboat.
They must need to know about equipments and drills .The
master should check wheather the crew member practice the drills at least the safety officer
should check wheather the crew know about equipments and practice in a time of intervals
.The safety drills like as lowering life boat drills is very necessary in case of emergency.
On the other hand all the crew members think about their
wages ,if it is low and the crew work extra periods, that stresses their mind which is not in the
shipping industry. So by giving them proper wages , they can be motivated.
HOW CAN THIS SOLVED ?
1 .Drills should be carried out in the proper time to time as per
regulation.
2.Must give them proper wages and be motivate them.
SCENARIO: 5
6. MARINE LAW ANDMANAGEMENT
REASON : Lack of co-operation ,planning; monitoring and poor management.
Though the master is the in charge of the ship ,he should manage everything
and motivated the crew to remain in a good working atmosphere .He should treat everybody
equally so that he can expect a better out put from them .The master should need to think
always ahead for the safety of the ship.
This scenario is mainly on the lack of planning and a result of poor
management on board ship. There is no good communication between deck department and
engine department .In the shipping industry this is not gonna be happen .Everybody should
aware about their duty and concern about their responsibility .During any specific operation
the deck department and engine department must carried a radio watch by using portable
VHF set .From the bridge or cargo control room should maintain good monitoring throughout
the operation.
The captain is the leader of a ship, he can not give blame to others .He must
check everybodies duty and work .Everything is proper planned and as a master he should
checked their job. As a team the deck department and engine department should maintain
always a good communications .But when inform engine room ,they told that they
maintaining engines, the deck department do not know about the engine room happening that
gonna be happen .And the master can not say like this. He should know about his duty and
others also.
HOW THIS CAN BE SOLVED ?
1.Master should have a good leadership and ahead thinking
qualities.
2 Both two department need to keep good communications
with each others for the safety of the ship.
