The document discusses cohabitation agreements, also known as "No-Nups", which are legal agreements for unmarried couples living together. No-Nups set out how property, finances, children and other assets will be divided if the relationship ends, as the laws for unmarried partners are different than for married couples. They can help minimize costly disputes by clarifying things like property ownership, financial contributions, child support, and pension benefits. The document recommends couples consider a No-Nup if they buy property together, have children, or hold pensions, to avoid uncertainty and potentially expensive litigation.

"He's never going to leave me..." and other myths - RKWN event - Wednesday 3 ...Russell_Kennedy
This document discusses myths about marriage and divorce rates, and the benefits of financial agreements. It notes that the divorce rate has been increasing and is higher for second marriages. Many marriages do not last long, with the median duration being 12 years. There are many reasons why marriages break down. Separation can have significant financial consequences as assets are often required to be divided. Financial agreements allow couples to avoid costly court battles and determine how assets will be divided upon separation outside of the family court system. They provide certainty and can help protect inheritances and gifts.
There are several forms of business ownership including proprietorships, partnerships, and corporations. Proprietorships are owned and run by one person, partnerships are owned by two or more individuals who share profits and losses, and corporations are separate legal entities with stockholders and boards of directors. Other forms include limited liability partnerships, joint ventures, S-corporations, limited liability companies, nonprofits, franchises, and cooperatives. Each form has different legal requirements, levels of control, responsibility for profits and losses, and liability.

This document discusses the financial aspects of relationship breakdown and divorce. It explains that during a divorce, the court will consider all assets of both parties and try to redistribute them fairly based on factors like needs, length of the marriage, and contributions made. The court has powers to make orders around property, payments, and pensions that can significantly impact one's wealth. Whether the family home needs to be sold, how pensions will be treated, and how inheritances are considered are complex issues that require legal advice. Blaser Mills' family law team can provide guidance on rights and obligations during this process.

The document discusses the factors used to determine alimony in Florida according to Florida law. It examines 13 factors that courts will consider, including the standard of living during the marriage, the duration of the marriage, the age and health of the parties, the financial resources and earning potentials of both parties, contributions to the marriage such as homemaking, the responsibilities for children, tax treatment of alimony, all sources of income, and any other factors required for equity. Courts aim to use these criteria to properly evaluate each case and decide what constitutes a fair alimony award.

An arranged marriage is a marriage where the parents or guardians of the individuals getting married make the selection of the spouse rather than the individuals themselves. The process typically involves the parents creating lists of potential spouses and negotiating with each other before deciding on a match. While in the past the children had no input, modern arranged marriages often allow the children to choose from the list of potentials selected by their parents. Arranged marriages can provide social, political and economic advantages for families but they also risk an unhappy or abusive match between the spouses if they do not get along. However, some couples in arranged marriages report that love developed between them over time even if it did not begin that way.
it is significant to note that the legal rights and entitlements of same sex couples in relationship disputes (with or without children) are the same as those for heterosexual couples.

Cell Therapy Expansion and Challenges in Autoimmune DiseaseHealth Advances
There is increasing confidence that cell therapies will soon play a role in the treatment of autoimmune disorders, but the extent of this impact remains to be seen. Early readouts on autologous CAR-Ts in lupus are encouraging, but manufacturing and cost limitations are likely to restrict access to highly refractory patients. Allogeneic CAR-Ts have the potential to broaden access to earlier lines of treatment due to their inherent cost benefits, however they will need to demonstrate comparable or improved efficacy to established modalities.
In addition to infrastructure and capacity constraints, CAR-Ts face a very different risk-benefit dynamic in autoimmune compared to oncology, highlighting the need for tolerable therapies with low adverse event risk. CAR-NK and Treg-based therapies are also being developed in certain autoimmune disorders and may demonstrate favorable safety profiles. Several novel non-cell therapies such as bispecific antibodies, nanobodies, and RNAi drugs, may also offer future alternative competitive solutions with variable value propositions.
Widespread adoption of cell therapies will not only require strong efficacy and safety data, but also adapted pricing and access strategies. At oncology-based price points, CAR-Ts are unlikely to achieve broad market access in autoimmune disorders, with eligible patient populations that are potentially orders of magnitude greater than the number of currently addressable cancer patients. Developers have made strides towards reducing cell therapy COGS while improving manufacturing efficiency, but payors will inevitably restrict access until more sustainable pricing is achieved.
Despite these headwinds, industry leaders and investors remain confident that cell therapies are poised to address significant unmet need in patients suffering from autoimmune disorders. However, the extent of this impact on the treatment landscape remains to be seen, as the industry rapidly approaches an inflection point.

Breast cancer: Post menopausal endocrine therapyDr. Sumit KUMAR
Breast cancer in postmenopausal women with hormone receptor-positive (HR+) status is a common and complex condition that necessitates a multifaceted approach to management. HR+ breast cancer means that the cancer cells grow in response to hormones such as estrogen and progesterone. This subtype is prevalent among postmenopausal women and typically exhibits a more indolent course compared to other forms of breast cancer, which allows for a variety of treatment options.
Diagnosis and Staging
The diagnosis of HR+ breast cancer begins with clinical evaluation, imaging, and biopsy. Imaging modalities such as mammography, ultrasound, and MRI help in assessing the extent of the disease. Histopathological examination and immunohistochemical staining of the biopsy sample confirm the diagnosis and hormone receptor status by identifying the presence of estrogen receptors (ER) and progesterone receptors (PR) on the tumor cells.
Staging involves determining the size of the tumor (T), the involvement of regional lymph nodes (N), and the presence of distant metastasis (M). The American Joint Committee on Cancer (AJCC) staging system is commonly used. Accurate staging is critical as it guides treatment decisions.
Treatment Options
Endocrine Therapy
Endocrine therapy is the cornerstone of treatment for HR+ breast cancer in postmenopausal women. The primary goal is to reduce the levels of estrogen or block its effects on cancer cells. Commonly used agents include:
Selective Estrogen Receptor Modulators (SERMs): Tamoxifen is a SERM that binds to estrogen receptors, blocking estrogen from stimulating breast cancer cells. It is effective but may have side effects such as increased risk of endometrial cancer and thromboembolic events.
Aromatase Inhibitors (AIs): These drugs, including anastrozole, letrozole, and exemestane, lower estrogen levels by inhibiting the aromatase enzyme, which converts androgens to estrogen in peripheral tissues. AIs are generally preferred in postmenopausal women due to their efficacy and safety profile compared to tamoxifen.
Selective Estrogen Receptor Downregulators (SERDs): Fulvestrant is a SERD that degrades estrogen receptors and is used in cases where resistance to other endocrine therapies develops.
Combination Therapies
Combining endocrine therapy with other treatments enhances efficacy. Examples include:
Endocrine Therapy with CDK4/6 Inhibitors: Palbociclib, ribociclib, and abemaciclib are CDK4/6 inhibitors that, when combined with endocrine therapy, significantly improve progression-free survival in advanced HR+ breast cancer.
Endocrine Therapy with mTOR Inhibitors: Everolimus, an mTOR inhibitor, can be added to endocrine therapy for patients who have developed resistance to aromatase inhibitors.
Chemotherapy
Chemotherapy is generally reserved for patients with high-risk features, such as large tumor size, high-grade histology, or extensive lymph node involvement. Regimens often include anthracyclines and taxanes.
2. Family Limited
Partnership
• A “Family” Limited Partnership (FLP)
is a Limited Partnership where most
of the partners arc members of the
same family group. In 2004, a U.S.
Court of Appeals held that family
members can enter into valid Family
Limited Partnerships. A variety of
other courts have also noted that
there is no distinction between a
Family Limited Partnership and any
other type of Limited Partnership,
and FLP’s can hold and protect a
home.
3. Continued
• In a typical FLP, someone in the
family, or corporation or
limited liability company they
control, acts as the general
partner so that control is
retained by one or more family
members. If a FLP operates a
family business, the parent
that manages the business
could serve as the general
partner and the children,
spouse, or other family
members could be limited
partners.
4. Continued
• If the FLP is used to own family
investments, the spouse with
the least risk and the children
are often limited partners,
while both spouses, or their
trusts, would otherwise be the
general partners. You should
make certain that in placing a
residence into a FLP that only
those holding title to the
residence are the initial
owners of the FLP, and that
they own both in the same
percentages of ownership.
5. Conclusion
• This is because in recent years
the IRS has scrutinized the FLP
for valuation, estate, and gift
tax purposes. These questions,
however, only arise if there has
been a “transfer” as defined in
the federal tax laws. No gift tax
is imposed if a taxpayer makes
a transfer to himself. And,
where a taxpayer contributes
property to a FLP which he
owns in the same percentage
as he owned the contributed
property, there is also no gift
tax, since there is deemed to
have been no “transfer.”
