Introduction to Constitution and
Its Components
CONSTITUTION COMES FROM LATIN WORD “CONSTITUERE'
WHICH MEANS; TO SET UP, TO ESTABLISH, TO FORM OR
MAKEUP, TO APPOINT, TO GIVE, BEING TO.
1.
Constitution is defined as persons physical and
mental makeup which is revealed through his
physical built, his characteristic desires aversions
and reactions as well as emotional and intellectual
attributes

B.Diathesis

Constitution is the characteristic way of a person
to live in the world.
While diathesis is the characteristic way of a
person of getting sick.
Where temperaments is the characteristic of a
person that which predispose him to act and
behave in a particular manner
Temperament and Diathesis are
integral part of constitution, which
helps in individualisation.
Man recieves some features from his ancestors and
some from the environment factors he has been
brought up .
The effect of exogenous and endogenous factors is at
emotional intellectual and physical plane the sum
total of individual characteristics in the 3 planes is
known as “constitution”.

Genes transmit traits according to Mendelian principles (dominant, recessive, and sex-linked
traits like hemophilia).
- Health represents equilibrium; disease signifies loss of equilibrium or disharmony which are
indicated through signs and symptoms . their removal leads to restoration of lost balance and
helps in constituting ,according to hahnemann , cure-rapid gentle and permanent restoration
of health.
-this loss of balance often leads to Disturbed emotional states , impacting organ functioning,
leading to structural alterations over time contributing to additional symptoms.
TRANSMISSION OF HEREDITARY
CHARACTERISTICS
- Disease is a total response of an organism to adverse environmental
factors ,external or internal : conditioned by constitutional factors
inherited and acquired and its manifest in emotional, intellectual, and
physical spheres.
- Responses are divisible into individual (denoting constitutional type)
and group (diagnostic symptoms) response.(seen in many)
HISTORICAL PERSPECTIVES ON
CONSTITUTION
Hippocrates taught that all diseases (excluding injuries) were initially
general in nature and only become local to provoke a crisis at a later
stage. All natural diseases are originally functional and then proceed
toward pathological damage over time.
He also taught that there was no such thing as a single cause in a natural
disease.
He taught that causation was of an interdependent origin rather than
any one isolated factor.
There is always the merging of the susceptibility of an individual or
group with a sympathetic pathogenic influence. Therefore, the
aetiological constellation includes the predispositions of the physical
constitution and mental temperament, the nature of the disease state,
as well as environment conditioning factors.
Hippocrates classified constitution into 2 types, short and
thick – is prone to apoplectic conditions and Long and thin –
is prone to tubercular affection.
The concept of constitution is widely applied in the practice of traditional Chinese
medicine (TCM).
Constitutional consideration is an important process for clinical diagnosis and
treatment, and it also lays the foundation for health promotion and disease
prevention.
This concept has been in China for thousands of years. It originated from the famous
Chinese Medicine book, Huang Di Nei Jing.
The body constitution concept states that every individual has his or her unique
body, which reacts in a specific way to environmental influences.
As per this concept body constitution can vary from person to person, some are
strong, some are weak, some tend to be heat, and some tend to be cold.
According to the China Association for Traditional Chinese Medicine (CACM), body
constitution can be divided into nine types, named as neutral, qi deficiency, yang deficiency,
yin deficiency, blood stasis, phlegm & dampness, damp-heat, qi stagnation and special
constitution.
Generally, the classification of body constitution is based on physical outlook, personality,
common health problems, and adaptation to external environment .
There is certain traditional Chinese Medicine concept quite similar to the Homeopathic
concept.
One such concept is that advocates that different remedy for the same disease and same
remedy for different disease.
Individuals may have the same disease, but their unique body constitution necessitates the
use of different remedies that not only work with the symptom but also deal with the root
cause, the imbalance in the body constitution.
This is why you will see so much variation in Chinese Medicine herbal remedies to treat the
same common illnesses, such as flu.
Ayurveda, describes a unique concept of constitution, genetically determined,
categorising the population into several subgroups based on phenotypic
characters like appearance, temperament and habits.
According to this concept, the basic constitution of an individual is determined at
the time of conception thus constitution is called Prakruti which is a Sanskrit word
that means, "nature," "creativity," or "the first creation."
One of the very important concept of Ayurveda is that one's basic constitution is
fixed throughout his lifetime.
The combination of Vata, Pitta, and Kapha that was present in the individual at the
time of conception is maintained throughout his lifetime. This is your base point.
Notice that different persons can have different combination of
Vata, Pitta and kapha as their basic constitution or Prakruti.
This is how Ayurveda can explain the subtle differences between
individuals and explains why everyone is unique and that two
persons can react very differently when exposed to the same
environment or stimuli.
One individuals Prakruti is unique to that individual just as the
fingerprint and DNA. Thus, in order to understand a person, it is
necessary to determine his or her Prakruti. Identifying the prakriti
helps in individualising therapy or predicting proneness to a
disease.
Hahnemann
concepts of
Constitution
Hahnemann gives a fair idea on the importance of the constitution in Aphorism 5
of Organon of Medicine.
wherein he states “Useful to the physician in assisting him to cure are the
particulars of the most probable exciting cause of the acute disease, as also the
most significant points in the whole history of the chronic disease, to enable him
to discover its fundamental cause, which is generally due to a chronic miasm. In
these investigations, the ascertainable physical constitution of the patient (and
intellectual character, his occupation, mode of living and habits, his social and
domestic relations, his age, sexual function, ..etc., are to be taken into
consideration”.
DR.VON.GRAUVOGL’S
CLASSIFICATION OF
CONSTITUTION
A genuine constitutional theory was put forth by a German homoeopath Von
Grauvogl in his textbook of Homoeopathy published in 1865.
Though there are different systems having their own concepts of constitution,
we will discuss the one which we follow.
That is Dr Von Grauvogl classification which is fairly explained by JH Clarke in
his book Constitutional medicines says he is a good chemist and have very
good knowledge on chemistry and systematically classified the Constitution
into three types.
Every living organism is made up of oxygen, carbon, nitrogen and hydrogen.
The Excess or deficiency of these elements in the tissues causes the
Development of different individual constitutions
it is
characterized by
excess of
hydrogen Which
leads to
retention of
water in tissue.
HYDROGENOID
CONSTITUTION
(a) Slow intake of nutrition.
(b) Puffiness, retention of water in the
tissues.
(c) Easily affected by humidity.
(d) All of its disorders have periodicity .
(e)apathetic, indolent, heavy
Examples:
(I) Natrum sulphuricum
(ii) Thuja occidentalis

OXYGENOID CONSTITUTION
Word itself says that there is excess of oxygen
which causes breakdown of the hydrocarbons,
nitrogens and albuminous , tissue and bones,
causing destruction of tissues.
(a) thin with complain of weight loss in spite of
having good appetite and eating well.
(b) nervous individuals who have increased
physical and mental activities
(c) they have prominent glands , weak bones and
are anaemic .
(d) white spots on nails
(e) Hypohydric aspect with dryness of the skin
and mucous membranes.
Examples: Acidum nitricum, Carbo vegetabilis
Few clinical conditions OXYGENOID constitution suffer from
?
Ulcers

3.CARBONITROGENOID
These people have excess of carbon and nitrogen
and are deficient in oxygen
a) Tissues do not absorb sufficient oxygen due to retarded
nutrition .
b) After a period of obesity thinness follows
C)mentally anxious , restless , irritable
(i) Lycopodium clavatum
(ii) Cuprum metallicum

CONSTITUTIONAL DIAGNOSIS
This method is useful in understanding the peculiarity of a
person in his healthy state as well as diseased state. This
also helps in the "individualization of a person.
Constitutional diagnosis is done under 3 headings:
Basic
Developmental
Environmental
This can be understood better by learning the past history of the
patient. The circumstances and the conditions the patient had
gone through are studied. Developmental constitution
determines the person's psycho-somatic state.
It includes the peculiarities of an individual in his healthy state.
No two persons are alike in this world, either in health or in
disease. The basic constitutional symptoms of the person are
influenced by endogenous factors.
This determines the person's reactions to the current situations, time and
circumstances, either friendly or troublesome. How the person behaves
during a challenging situation, how he copes up with pressure? In short it
helps in learning what the person's behavioural pattern, reaction to the
given environment
TEMPERAMENTS
Temperament
The word Temperament is a Latin derivative of
"TEMPERARE" which means "TO TEMPER" or "A PERSON'S STATE
OF MIND".
Temperaments are the tendencies of an individual to act or behave
in a particular manner .
According to Aristotle, "Temperament is the particular nature of a Person,
Body and Soul".
Hippocrates (469 BC- 377 BC)
• He believed that body is made up of four Humours.
• BLOOD, PHLEGM, YELLOW BILE and BLACK BILE.
• These Humours circulate in the body and determine the unique physical,
mental and emotional qualities of a person.
• If any imbalance in these Humours occurs, they can cause disease
condition.
Claudius Galenus
• He was a Greek physician, surgeon and philosopher in Roman
empire.
• He believed that these Humours are formed in the body
(rather than ingested).
• According to him these humours can be influenced by the
type of food we ingest.
• Galen used the term "Bodily disposition" because
temperaments can determine a person's susceptibility towards
particular diseases and emotional or behavioural tendencies.
There are mainly Four types of temperaments
has been observed in a person.

DIATHESIS
Diathesis
Greek word DIATHESIS = DIATITHENAI = TO ARRANGE , TO DISPOSE
DEFINITION
Diathesis is an inherited or acquired organic and systemic weakness which can
cause predisposition towards certain disease condition.
In other words,
The disease disposition arising from the defects of constitution can be called as
the diathesis.
A condition of the body which makes the tissue react in the special way to certain
extensive stimuli and thus tends to make the person more susceptible to certain
disease.
TYPES OF DIATHESIS-
1. Lymphatic diathesis/Exudative diathesis
2. Uric acid /Rheumatic/Gouty/Hydrogenoid
diathesis (constitution)
3. Dyscratic diathesis
4. Tubercular diathesis
5. Scrofulous diathesis

Constitutions in different
repertories
The constitutions chapter was first introduced by Murphy in the second edition of
repertory.
This chapter consist all the genetic, dispositions, and body types from the generals
chapter.
Extensive additions were gathered from Hering’s guiding symptoms.
The constitution chapter contains: constitutions, temperaments, body size, body type,
hair in general, complexions, age, growth, gender, habits, defects, miasms, infants,
inheritance, children, boys, girls, elderly people, men, occupations, women, young
people.
“Constitution” chapter in the Homoeopathic Medical Repertory several types of
constitutions other than what is mentioned by Grauvogl. Many of the rubrics are
useful in our clinics. The rubric -Occupation with some subrubrics are not seen in
other repertories.

In Knerr’s repertory the rubrics related to the constitution is included under the
chapter stages of life and constitution.
In this chapter rubrics related to various age groups are included under headings
like children; old age etc. the rubric constitution has many sub rubrics.
Most of the rubrics of the Murphy’s constitution chapter are based on the
constitution rubric of the stages and constitution chapter of Knerr’s repertory.
Dr. Calvin Knerr in his repertory, “The Repertory of Hering’s Guiding Symptoms of Our
Materia Medica” gave the rubrics related to Temperament and Diathesis where
number of medicines were more than any other work related to subject and the choice
of rubrics were precise but profound. He gave due recognition to this aspect.
ii)Repertory of Hering's Guiding
Symptoms of our Materia medica
Constitution section contains mostly diathesis, tendencies,
constitution types, miasms
Subrubrics involve subjective and objective symptoms of those
constitutions and diatheses, and also some age groups or persons who
are affected

Boger-Boenninghausen's Characteristics and repertory
mentions nine rubrics related to constitutional types in the
chapter sensations and complain in general such as
Carbonitrogenous, Dyscratic, Hydrogenoid,
Lithaemic/acidosis, Neuropathic, Oxygenoid, Psoric, Sycotic
and Syphilitic.
It covers concept of Hahnemann, Grauvogl and his own.
Boger-Boenninghausen's
Characteristics and repertory
In clinical repertory by J.H.Clarke is divided into five chapters.
One of them is repertory of the temperaments, dispositions,
constitutions and states.
In this chapter are given the remedies which have been found to
act most beneficially in certain types of persons, temperaments,
sex and age.
There are also included complaints and conditions of particular
type of persons and constitutions.

  • 6. Introduction to Constitution and Its Components
  • 7. CONSTITUTION COMES FROM LATIN WORD “CONSTITUERE' WHICH MEANS; TO SET UP, TO ESTABLISH, TO FORM OR MAKEUP, TO APPOINT, TO GIVE, BEING TO. 1. Constitution is defined as persons physical and mental makeup which is revealed through his physical built, his characteristic desires aversions and reactions as well as emotional and intellectual attributes 2.

  • 9. Constitution is the characteristic way of a person to live in the world. While diathesis is the characteristic way of a person of getting sick. Where temperaments is the characteristic of a person that which predispose him to act and behave in a particular manner Temperament and Diathesis are integral part of constitution, which helps in individualisation.
  • 10. Man recieves some features from his ancestors and some from the environment factors he has been brought up . The effect of exogenous and endogenous factors is at emotional intellectual and physical plane the sum total of individual characteristics in the 3 planes is known as “constitution”.

  • 13. Genes transmit traits according to Mendelian principles (dominant, recessive, and sex-linked traits like hemophilia). - Health represents equilibrium; disease signifies loss of equilibrium or disharmony which are indicated through signs and symptoms . their removal leads to restoration of lost balance and helps in constituting ,according to hahnemann , cure-rapid gentle and permanent restoration of health. -this loss of balance often leads to Disturbed emotional states , impacting organ functioning, leading to structural alterations over time contributing to additional symptoms. TRANSMISSION OF HEREDITARY CHARACTERISTICS
  • 14. - Disease is a total response of an organism to adverse environmental factors ,external or internal : conditioned by constitutional factors inherited and acquired and its manifest in emotional, intellectual, and physical spheres. - Responses are divisible into individual (denoting constitutional type) and group (diagnostic symptoms) response.(seen in many)
  • 16. Hippocrates taught that all diseases (excluding injuries) were initially general in nature and only become local to provoke a crisis at a later stage. All natural diseases are originally functional and then proceed toward pathological damage over time. He also taught that there was no such thing as a single cause in a natural disease. He taught that causation was of an interdependent origin rather than any one isolated factor. There is always the merging of the susceptibility of an individual or group with a sympathetic pathogenic influence. Therefore, the aetiological constellation includes the predispositions of the physical constitution and mental temperament, the nature of the disease state, as well as environment conditioning factors.
  • 17. Hippocrates classified constitution into 2 types, short and thick – is prone to apoplectic conditions and Long and thin – is prone to tubercular affection.

  • 20. The concept of constitution is widely applied in the practice of traditional Chinese medicine (TCM). Constitutional consideration is an important process for clinical diagnosis and treatment, and it also lays the foundation for health promotion and disease prevention. This concept has been in China for thousands of years. It originated from the famous Chinese Medicine book, Huang Di Nei Jing. The body constitution concept states that every individual has his or her unique body, which reacts in a specific way to environmental influences. As per this concept body constitution can vary from person to person, some are strong, some are weak, some tend to be heat, and some tend to be cold.
  • 21. According to the China Association for Traditional Chinese Medicine (CACM), body constitution can be divided into nine types, named as neutral, qi deficiency, yang deficiency, yin deficiency, blood stasis, phlegm & dampness, damp-heat, qi stagnation and special constitution. Generally, the classification of body constitution is based on physical outlook, personality, common health problems, and adaptation to external environment . There is certain traditional Chinese Medicine concept quite similar to the Homeopathic concept. One such concept is that advocates that different remedy for the same disease and same remedy for different disease. Individuals may have the same disease, but their unique body constitution necessitates the use of different remedies that not only work with the symptom but also deal with the root cause, the imbalance in the body constitution. This is why you will see so much variation in Chinese Medicine herbal remedies to treat the same common illnesses, such as flu.

  • 23. Ayurveda, describes a unique concept of constitution, genetically determined, categorising the population into several subgroups based on phenotypic characters like appearance, temperament and habits. According to this concept, the basic constitution of an individual is determined at the time of conception thus constitution is called Prakruti which is a Sanskrit word that means, "nature," "creativity," or "the first creation." One of the very important concept of Ayurveda is that one's basic constitution is fixed throughout his lifetime. The combination of Vata, Pitta, and Kapha that was present in the individual at the time of conception is maintained throughout his lifetime. This is your base point.
  • 24. Notice that different persons can have different combination of Vata, Pitta and kapha as their basic constitution or Prakruti. This is how Ayurveda can explain the subtle differences between individuals and explains why everyone is unique and that two persons can react very differently when exposed to the same environment or stimuli. One individuals Prakruti is unique to that individual just as the fingerprint and DNA. Thus, in order to understand a person, it is necessary to determine his or her Prakruti. Identifying the prakriti helps in individualising therapy or predicting proneness to a disease.
  • 26. Hahnemann gives a fair idea on the importance of the constitution in Aphorism 5 of Organon of Medicine. wherein he states “Useful to the physician in assisting him to cure are the particulars of the most probable exciting cause of the acute disease, as also the most significant points in the whole history of the chronic disease, to enable him to discover its fundamental cause, which is generally due to a chronic miasm. In these investigations, the ascertainable physical constitution of the patient (and intellectual character, his occupation, mode of living and habits, his social and domestic relations, his age, sexual function, ..etc., are to be taken into consideration”.
  • 28. A genuine constitutional theory was put forth by a German homoeopath Von Grauvogl in his textbook of Homoeopathy published in 1865. Though there are different systems having their own concepts of constitution, we will discuss the one which we follow. That is Dr Von Grauvogl classification which is fairly explained by JH Clarke in his book Constitutional medicines says he is a good chemist and have very good knowledge on chemistry and systematically classified the Constitution into three types. Every living organism is made up of oxygen, carbon, nitrogen and hydrogen. The Excess or deficiency of these elements in the tissues causes the Development of different individual constitutions
  • 29. it is characterized by excess of hydrogen Which leads to retention of water in tissue. HYDROGENOID CONSTITUTION
  • 30. (a) Slow intake of nutrition. (b) Puffiness, retention of water in the tissues. (c) Easily affected by humidity. (d) All of its disorders have periodicity . (e)apathetic, indolent, heavy Examples: (I) Natrum sulphuricum (ii) Thuja occidentalis

  • 44. CONSTITUTIONAL DIAGNOSIS This method is useful in understanding the peculiarity of a person in his healthy state as well as diseased state. This also helps in the "individualization of a person. Constitutional diagnosis is done under 3 headings:
  • 45. Basic Developmental Environmental This can be understood better by learning the past history of the patient. The circumstances and the conditions the patient had gone through are studied. Developmental constitution determines the person's psycho-somatic state. It includes the peculiarities of an individual in his healthy state. No two persons are alike in this world, either in health or in disease. The basic constitutional symptoms of the person are influenced by endogenous factors. This determines the person's reactions to the current situations, time and circumstances, either friendly or troublesome. How the person behaves during a challenging situation, how he copes up with pressure? In short it helps in learning what the person's behavioural pattern, reaction to the given environment
  • 47. Temperament The word Temperament is a Latin derivative of "TEMPERARE" which means "TO TEMPER" or "A PERSON'S STATE OF MIND". Temperaments are the tendencies of an individual to act or behave in a particular manner .
  • 48. According to Aristotle, "Temperament is the particular nature of a Person, Body and Soul". Hippocrates (469 BC- 377 BC) • He believed that body is made up of four Humours. • BLOOD, PHLEGM, YELLOW BILE and BLACK BILE. • These Humours circulate in the body and determine the unique physical, mental and emotional qualities of a person. • If any imbalance in these Humours occurs, they can cause disease condition.
  • 49. Claudius Galenus • He was a Greek physician, surgeon and philosopher in Roman empire. • He believed that these Humours are formed in the body (rather than ingested). • According to him these humours can be influenced by the type of food we ingest. • Galen used the term "Bodily disposition" because temperaments can determine a person's susceptibility towards particular diseases and emotional or behavioural tendencies.

In other words, The disease disposition arising from the defects of constitution can be called as the diathesis. A condition of the body which makes the tissue react in the special way to certain extensive stimuli and thus tends to make the person more susceptible to certain disease.

In this chapter rubrics related to various age groups are included under headings like children; old age etc. the rubric constitution has many sub rubrics. Most of the rubrics of the Murphy’s constitution chapter are based on the constitution rubric of the stages and constitution chapter of Knerr’s repertory. Dr. Calvin Knerr in his repertory, “The Repertory of Hering’s Guiding Symptoms of Our Materia Medica” gave the rubrics related to Temperament and Diathesis where number of medicines were more than any other work related to subject and the choice of rubrics were precise but profound. He gave due recognition to this aspect. ii)Repertory of Hering's Guiding Symptoms of our Materia medica
