This document is a logsheet template used by a school in the Philippines to track student progress in subjects on a weekly basis. It includes fields for the school name, subject, grade, section, student enrollment and attendance breakdown, quarter, week, competencies and accomplishments for each class including the number of students who achieved proficiency, and space for remarks or interventions. The logsheet must be prepared by a teacher and checked by an observer on a regular weekly basis to monitor student performance.
