Cycle 7: A Avenue: Community Pharmacy Rotation Jan – Feb 2015
Preceptor: Shelly Hewitt Pharm D.
§ Completed patient interviews through the Medicaid PCM program, Mirixa, and Outcomes. Worked with
doctors to initiate drug regimens that would improve patient health. Assisted the current resident run a
pharmacy in-service to educate individuals with hypertension and diabetes. Went to the Free Medical
Clinic once a week to volunteer.

Cycle 6: Cass Lake Indian Health Service: Underserved Population Rotation (Elective) Nov – Dec 2015

§ Worked in a health clinic on an Indian reservation and assisted in daily actives that included counseling
and referring within the over-the-counter clinic, completing drug information requests from doctors and
pharmacists, completing pharmacy-to-dose Vitamin D, and educating within smoking cessation and
Coumadin clinic.

§ Completed a Medwatch for possible adverse reaction while using Stelara

§ Completed a two year chart review for a patient diagnosed with fibromyalgia who is abusing pain
medicine

Cycle 5: International Rotation: Medical Mission Trip to Eastern Africa (Elective) Oct – Nov 2015
Preceptor: Ron Herman PhD
§ Created a drug formulary pocket card for the healthcare team before leaving for Africa. While in Africa,
facilitated and coordinated daily pharmacy operations with peer. Daily duties included writing and filling
prescriptions, working with the pharmacist to verify orders, dosing antibiotics that were sent “dose to
pharmacy”, and counseling patients on medication indication, administration and side effects with the
help of a translator. On average, we counseled 200 patients per day and filled 500 prescriptions per day.
Additional responsibilities were to document the experience and provide a blog and presentation for
classmates, friends and family.

§ Reviewed approximately ten patient’s and prepared for morning vascular and medicine team rounds.
Recommended medication dose adjustments based on renal function or drug interactions, suggested
monitoring parameters, documented pharmacy recommendations, and completed medication
reconciliation for new patients.
§ Assisted preceptor with CPS course at the University of Iowa College of Pharmacy
Projects:
§ Journal club presented to pharmacy preceptors and peers
§ Formal case on pulmonary arterial hypertension presented to pharmacy staff, residents, and peers.

Cycle 3: Unity Point St. Luke’s Hospital (General Hospital) Jul – Aug 2015

§ Reviewed and prepared 20 patients for morning rounds with pharmacist and medical team. Assisted in
pharmacy to dose vancomyacin, TPN, and warfarin. Followed various pharmacy specialists and assisted
in their daily activities.

thrombocytopenia and colonoscopies.

§ Went to the free medical clinic twice a week to learn and gather tools to overcome language and health
literacy barriers

§ Created a questionnaire to gather information during the interview and a brochure for the individuals to
take home.
Cycle 1: One Point Patient Care (Elective) May - Jun 2015

§ Provided an in-service to doctors and nurses at hospice inpatient units via one-minute consults.
Researched drug information requests for doctors, nurses, and pharmacists. Completed clinical
assessments by removing unnecessary medications/matching medication coverage and collected new
prescription requests from nurses in hospice facilities.

§ ‘New + Views’ article for OPPC newsletter on antipsychotic use in end of life care
§ ‘Clinical Assessment Guideline’- initial assessment for hospice patients, provided nurses and doctors with
written recommendations, recommendations included medication coverage by hospice, appropriate
