percent reduction strategy for
readmissions for this fiscal year based on
the base number from Measure Factory.

The Solution
 Covenant’s first users are in the quality department:
– Nurse managers are being trained to access DivePort – Dimensional
Insight’s browser-based access to dashboards – to see the
readmissions numbers for their unit each month.
– Each unit has a target to help contribute to the overall reduction in
readmissions.
 Covenant spent significant time during the rollout process
on data governance and data validation:
– The validation process allowed the IT department to uncover
workflow errors that the organization hadn’t previously realized.
