Chemical Dependency Professionals Board as a Chemical Dependency Counselor Assistant
(CDCA). This certification is effective 03/28/2016 and will expire on 04/28/2017. Enclosed

The preliminary CDCA is a thirteen (13) month credential. At the end of this thirteen month
period, professionals are required to submit a Phase II CDCA Application documenting the
additional 30 hours of education required to obtain a renewable CDCA certificate. Phase II
applications may be submitted up to three months prior to expiration of Phase I certification.
