Introduction:
 This is a practical assessment.
 The student must provide record of use of Core Isolators in an exercise environment.
 The student must instruct a user to exercise on the Core Isolators.
 The user must verify via this form – the Exercise Log.
Practical Assessment:
1. The student will instruct a user on a single Core Isolators exercise listed in the 7
Week 7 Stage Core Isolators Exercise Program.
2. The user will sign-off on the Exercise Log confirming.
3. To pass the student must complete 5 entry logs.
4. Any exercise can be selected.
5. A single user can only complete two (2) logs. This means at least three (3) people are
required to achieve 5 entry logs.
6. Scan and submit this form.
