Case Study: The Avian Flu Threat Projections are that Avian flu could cause problems in the U.S. in approximately one year. We must ask the question, are we ready? The virus is not new in this country. It has been around in chickens for over 20 years. A virulent strain of the virus named H5N2 recently surfaced in Texas and has been curtailed. We know from news reports and interviews with health officials that the disease is not passed from human to human, only from birds to humans. The World Health Organization (WHO), which counts only those deaths which they document as verified in a lab, gives the number as 98 of 177 who contracted H5N1, the Avian bird flu virus that infects poultry in Asia. The number of deaths seems small, but the medical community 's fear is that the virus will mutate into a strain that can be transmitted by humans to humans. If this happens WHO says, "The world is unprepared." Their statistics were given in mid-march and they are expecting to confirm another three deaths shortly. Members of the medical community, emergency managers and emergency communications managers are highly concerned about this new risk to public safety. With regard to the November 2014 Ebola scare in the United States, do you think the public health and medical professionals involved in the Ebola cluster investigation followed proper protocols? If so, what best practices were used? If not, what could have been done differently? Remember to cite references where necessary. When responding to classmates, provide additional critiques on approaches used by public health and medical professionals during the U.S. Ebola cluster investigation. Response must be 300 words. 1st Discussion post for response: Communities depend on public health organizations to “come to their rescue” and prevent epidemics from becoming pandemics. It is important that proper protocols are followed to ensure the safety of the public and to maintain trust from the community. When public health officials “drop the ball” community members are less likely to be cooperative during a state of emergency which inhibit proper handling of cases. According to one study, there is a conspiracy among certain individuals who believe the government created AIDS to kill minorities, and another 10% did not trust their provider to give them appropriate medical care (Whetten, et al., 2006). With that said, there is a lack of trust within certain populations. Adding to this distrust, I don’t believe the Ebola scare was handled as well as it could have been, and I’m not convinced that proper protocol was followed by the medical professionals. The crisis management plan was not followed appropriately and there were many improvisations made which lead to deaths that could have been prevented. According to one article’s documentation of the timeline, the first patient, Mr. Duncan was initially sent home from the hospital with antibiotics because the diagnosis of Ebola was missed, he was then formally diagnosed with Ebola days later on September 30th; Within the first five days after diagnosis there were 15 people being monitored for symptoms, after the first five days there were reports of over 80 people were being monitored and by 10 days after Duncan’s diagnosis he was dead (Andrew, Arlikatti, Chatterjee, & Ismayilov, 2018). There were 76 medical personnel that came into contact with Duncan and it was reported that they wore so much protective equipment that they actually increased the risk of contamination, for this reason the Centers for Disease Control and Prevention (CDC) had determined that it was the Hospital’s fault for the spread and admits that they did not have enough capacity to contain the crisis despite their earlier claims (Andrew, Arlikatti, Chatterjee, & Ismayilov, 2018). The WHO declared a level 3 emergency, which is the highest level of emergency in an attempt to cease the spread of Ebola, there were reported “overwhelming demands” due to the outbreak that the WHO could not contain without assistance, they would need help of other organizations (decisive government action such as bans on travel and trade) to meet all of the .

I. Introduction The recent reports of a new lethal strain of avian influenza spreading through Asia has raised the specter of a global flu pandemic on the scale of the 1918 Spanish Flu outbreak. Should this occur, the impact will be felt throughout the U.S., and ultimately on every campus in Maryland. The University must prepare itself for this possibility in order to mitigate the impact on our students, faculty and staff while maintaining University essential functions to the extent feasible. A. Status of the Avian Flu Pandemic Threat A flu pandemic is a global disease outbreak. Three conditions are necessary for a flu pandemic to occur: 1. A novel virus subtype emerges for which people have little or no prior immunity 2. The virus is capable of infecting humans 3. The virus changes and becomes efficient, causing sustained human-to-human transmission These conditions permit the disease to spread easily from person-to-person, cause serious illness, and sweep across countries and around the world in a very short time. Pandemics have occurred throughout history on a periodic basis. In the last century, there were three influenza pandemics, in 1918, 1957, and 1968. The deadly 1918 pandemic killed approximately 50 million people worldwide. At the present time, the first two of these three conditions have been met. 1. A novel virus subtype, H5N1, has emerged and, despite increased surveillance and large- scale culling of infected wild and domestic birds, is now persistent (endemic) in the bird population. The H5N1 virus has raised particular concerns about a potential human pandemic because it is especially virulent, and it has striking similarities to the deadly 1918 H1N1 flu strain. 2. H5N1 virus has now crossed species and is capable of infecting humans and other mammals. Since 2003, a growing number of human H5N1 cases have been reported in Thailand, Vietnam, Cambodia, Indonesia, and Turkey, and more than half of the people infected with the H5N1 virus have died. While the majority of the human cases thus far have been caused by exposure to infected poultry, there has been limited human-to-human spread through extremely close household contact with the- initial case. An "act utilitarian" makes an ethical decision by evaluating the consequences of an act for a given situation. CASE STUDY RUBRIC MICROBIOLOGY For the Case Study assignment the current pathogen selections may be requested by sending an email to your instructor! Assigned Case Study Problem: You will create a case study for a microbial infection selected from the current pathogen list. Your case study will be assembled using a detailed rubric (see below). Upon completion, you will submit your case study to the Blackboard gradebook in Unit 5 and to SafeAssign. How to create a case study The case studies are meant to be an enjoyable, interesting, and informative assignment. This is your chance to show that you understand the key teaching points about a microbe and to communicate these points in a written format. What information belongs in my case study? Have at least 3-4 key referenced points in each of the five areas shown in the Case Study Information Chart (see below). The left-hand heading in the chart suggests the type of information requested for the pathogen. Outlines can be in whatever form you prefer (bullets/charts/outlines/diagrams or a mix). Be sure to include two discussion questions (and provide complete answers) that you can incorporate into your case study (place them at the end of your write-up). These questions should help connect your case to other material in the course. For example, what other microbes have an A-B toxin? What other viruses are transmitted by fecal-oral spread?
