2. History
Mr.Sugumar ,71 yr/M Patient came with c/o Obstipation for 3days
H/O Abdominal pain Left hypochondriac and left loin pain for 2day
No H/O vomiting
No H/O malena, haematochezia
3. Examination
Local Examination of abdomen
Abdomen distended, BS sluggish,
Tenderness present in epigastric, left hypochondrium, left loin.
No guarding, no rigidity.
No palpable mass, no organomegaly
P/R roomy rectum, normal fecal stain present.
4. Investigations
X ray abdomen erect and add supine – multiple air fluid levels noted
CECT ABDOMEN AND PELVIS DONE
Rectosigmoid growth, multiple peritoneal and omental deposits,
lymphnodes, significant ascitis, B/L Pleural effusion, lung nodule present
5. Diagnosis : Acute intestinal obstruction , Rectosigmoid growth.
Procedure done: Emergency exploratory laparotomy proceded to diversion
transverse colostomy done
Ometal, peritoneal biopsies and peritoneal nodules sent for HPE , ascitic
fluid for cytology. Report awaited.
